OH Podiatrist Saves Foot of Woman with MRSA Infection
Lyndsey Montanaro's long nightmare began after she had received several corticosteroids in her heel by a physician, yet still had increasing pain. She was referred to Dr. Michael Canales, a foot-and-ankle surgeon at St. Vincent Charity Medical Center's Spine and Orthopedic Institute. Tests showed Montanaro had developed a severe methicillin-resistant Staphylococcus aureus, or MRSA, infection. Canales hypothesizes that the virulent bacillus, infamously resistant to most traditional antibiotics, was introduced deep into the heel by one of the injections at the urgent-care center. Although infection after a properly administered injection is exceedingly rare, Montanaro's case is a reminder that an injection is an invasive

procedure accompanied by risks. Canales admitted Montanaro to the hospital, where he spent the next 2 1/2 weeks working to save her foot. He ordered intravenous vancomycin, an antibiotic "so toxic that it can induce kidney failure in some patients." He took her into surgery the next day to remove dead skin, fat, muscle and bone, and he repeated the procedure every other day for a week. The doctor also arranged for hyperbaric oxygen therapy to help speed wound healing.

ASPS Holds Fall BVoard Meeting
The ASPS Board of Directors held their annual Fall board meeting November 15th and 16th in Bethesda, MD. At the meeting the board reviewed the association’s 2012-2013 Operational Plan noting that a majority of the planned initiatives were successfully accomplished. The board went on to outline an operational plan for 2013–2014, which includes the launch of a journal, new hands-on surgical training opportunities for residents, and the restructuring of ASPS committees.

Dr. Chad Friedman was named Top Doctor in the Main Line Today December 2013 issue. Dr. Friedman is a partner in Podiatry Care Specialists, P.C. with offices in West Chester, Newtown Square, and Audubon, PA. He is board certified by the American Board of Podiatric Surgery and board certified by the American Board of Wound Management in wound care, and he is a Fellow of the American College of Foot and Ankle Surgeons. Friedman was also named a Top Doctor in December 2010 by the same publication.

AZ Podiatrists Discuss "Spray-on Skin" for Wound Healing
UA researchers are studying the effectiveness of a “spray-on skin” intended to treat flesh wounds such as chronic leg ulcers that develop in elderly people. “I’m excited because, for a lot of this stuff, it sounds and it seems like Star Trek, where you’re spraying [skin] on,” said Dr. David G. Armstrong, a UA professor of surgery and director of SALSA, “but the future is now.”

As opposed to a skin graft, which is a flat layer of skin plastered onto the area, the liquid treatment can slip into the cracks and undulations of the wound, which could potentially make it a more effective option, said Dr. Nicholas Giovinco, assistant professor in the Department of Surgery and director of education for SALSA.

20/20 to Highlight New Digital X-Ray at RSNA
20/20 Imaging plans to highlight a new digital x-ray system with improved software capabilities and a higher-frequency generator at the upcoming Radiological Society of North America (RSNA) 2013 meeting. Targeted for the podiatry market, the higher-frequency system offers the potential for lower dose radiation with advanced imaging capabilities. It can also be anatomically programmed, which saves time, improves accuracy, and enhances efficiency by allowing exams to be selected at the technologist workstation instead of at the device, according to the firm.

10th Annual Desert Foot 2013 Conference Offered New Venue and Programming
The 10th Annual Desert Foot 2013 Conference attracted a strong showing of serious wound care experts from 5 countries (USA, Canada, Dominican Republic, India, Norway) and 43 U.S. states this past week. What began as a VA DPM-focused meeting has expanded into a multi-disciplinary conference including MDs, nurses, physical therapists, research PhDs, as well as many podiatry residents and students. This year’s conference, held at the Sheraton Phoenix Downtown included, for the first time, a hands-on Wound Debridement Skills Workshop.

The conference's highlight came when Chairman Robert Frykberg, DPM, MPH presented the 4th Annual Paul Brand Memorial Award to diabetic foot luminary William Jeffcoate from Nottingham, England. FDA Approves New Topical Antifungal Luliconazole 1%
The U.S. Food and Drug Administration (FDA) has approved the azole antifungal luliconazole 1% cream (Luzu, Valeant Pharmaceuticals) to treat fungal infections, the company announced today.
Luliconazole 1% cream is indicated for the topical treatment of interdigital tinea pedis (athlete's foot), tinea cruris (jock itch), and tinea corporis (ringworm), caused by the organisms Trichophyton rubrum and Epidermophyton floccosum, in adults aged 18 years and older.

Query: Podiatrists as a Source for Cosmeceuticals?
Recently, someone came to the office who sells cosmeceuticals (creams, foot moisturizers, etc.). He wanted to buy these items from me in bulk at a premium. The question I have is: what motivation would he have looking to buy from me? Why wouldn't he go directly to the supplier and buy for the price that I or anyone else can get them for? Is he possibly selling them at a premium to someone else? What's the angle?

Everything posted on this forum relative to Medicare, Obamacare, managed care, and all the other extraordinary hoops that they put docs through to get paid for the good that they do for society makes me ever so grateful that I am opted out of Medicare, participate with no insurance companies, and am nearer to the end of my career than not. To be very honest, this is exactly like the cattle being led to the slaughter. It won't take long before most of you realize that all of your cooperation with Medicare and insurance companies, along with your altruism, is going to eventually either put you out of business, or will dramatically reduce your ability to make a decent (and honest) living.
I urge all podiatrists who read this forum to give very strong consideration to looking into other fields of interest so you can slowly transition into something less regulated and much more lucrative. I've never been more serious since it is painfully obvious that the APMA is leading the death march and most of you are too afraid to do take a stand against the tyranny. I guess there is more honor in going down with the sinking ship. I apologize for bailing out 13 years ago.

Reconstructive Foot and Ankle Surgery - This fellowship is a one-year experience during which the Fellow will develop a rational approach to revisional and reconstructive foot and ankle surgery. The Fellow will function as an Instructor / Clinical and participate in the teaching of Podiatric medical students and residents, and have ample opportunity to be involved in clinical and basic research projects. The fellowship will provide the Podiatric Surgeon, further expertise in Charcot reconstruction, trauma and deformity correction.

"ANKLES AWAY", cute name, but a sad reality for so many podiatrists who can't touch the ankle. A few geographic areas are outliers and allow it; many don't. These outlier podiatrists have greater income and professional potential and are more ready to think of themselves as physicians, and call those held back as haters. Even in NY, next year, you can't treat the ankle unless you had a 3-year residency. Think about it. Fighting for the ankle. Nurses, chiropractors, and physical therapist would laugh. It never occurs to them that there is a part of the body they can't touch. Podiatry has limited its scope by focusing on suits without addressing the inferior education that forbids podiatrists from taking exams for a broader license. I think the podiatric leadership enjoys the status quo that keeps them secure and in power.
