Podiatrists Meet with Icelandic Minister of Health
The President of ABPM, Lee C. Rogers, DPM (TX), and the President of the International Federation of Podiatrists (FIP), Ivan Farrugia (Malta) met with the Icelandic Minister of Health, Sigurjon Jonsson, in Reykjavik. The trio discussed the looming threat to the health system by diabetic foot complications.

Iceland has one of the lowest prevalences of diabetes in the world, but it has more than doubled since 2005, potentially taxing the national health service with costly complications, including diabetic foot ulcers and amputations. The minister will be present at the Global Podiatry Summit in Reykjavik from March 3-6 to welcome podiatrists from around the world.

IL Podiatrist Participates in Phase 3 Post-Op Bunion Pain Trial
In a recent interview with long-time local radio personality Sam Madonia on WFMB-AM, John M. Sigle, DPM announced that his clinic is involved in an exciting new bunionectomy trial to evaluate an investigational pain medication for post-surgical bunion removal. Bunion removal is provided at no-cost to qualified patients.

This particular trial is in the final phase of research. This new medicine may be released on the market for worldwide use by this time next year if the results are positive and final approvals are granted.

CA Podiatrist Speaks at Red Caboose Dedication Ceremony
Antioch’s historic Red Caboose has made its final stop and it opened its doors to new passengers in last Saturday’s dedication at the Antioch Historical Museum. Saturday’s festivities also included commemoration speeches by James R. Boccio, DPM – the son of Jim and Phylis Boccio, the original owners of the Red Caboose and the former Red Caboose Restaurant that the train car was once attached to – and Doug Debs, the lead docent of the Niles Canyon Railway.

Boccio’s father bought the caboose back in the early '80s, nearly 20 years after he bought the land that the Red Caboose Restaurant sat on. “I was gone for 12 years, off to school and residencies, and I came back in 1982, and he told me he was going to buy a caboose and I thought he was nuts,” Boccio said. “He prepared the place for it with concrete and gravel and the rails and everything. He put it down there and it didn’t move for the next 40 years.”

Query: Diagnoses Codes Regarding Kidner Procedure
I have a patient that we are planning to move forward with surgical correction. The planned surgery will be a Kidner procedure at the left foot. I was hoping to get some input regarding the diagnosis codes. The patient has a prominent navicular exostosis. There is no os tibiale externum or accessory navicular. The patient has insertion pain and tendonitis at the tibialis posterior tendon. She does not have a significant pes planovalgus deformity. Thoughts on coding these?

Response: CPT 28238 is defined as the following: Reconstruction (advancement), posterior tibial tendon with excision of accessory tarsal navicular bone (e.g. Kidner type procedure). This is quite specific. The accessory navicular or os tibiale externum is excised and the posterior tibial tendon is advanced. In the above post, there is not an os tibiale externum present. The patient has an exostosis on the navicular. The posterior tibial tendon demonstrates insertional pain and tendonitis. What would be the most appropriate CPT code(s) to bill and the best ICD-10-CM code or codes to link to the CPT codes that are billed?
To address the prominent exostosis on the navicular, the most appropriate CPT code to bill for the excision or the removal of the exostosis is CPT 28122, which is defined as the following: Partial excision (craterization, saucerization, sequestrectomy, or diaphysectomy) bone (e.g. osteomyelitis or bossing); tarsal or metatarsal bone, except talus or calcaneus.
To address the detachment and the re-attachment of the posterior tibial tendon, the most appropriate CPT code to bill is CPT 28200 which is defined as the following: Repair, tendon flexor, foot; primary or secondary, without free graft, each tendon.
What about the ICD-10-CM codes?
For CPT 28122, the most appropriate ICD-10-CM code is M89.372 which is defined as: Hypertrophy of bone, left ankle and foot.
For CPT 28200, the most appropriate ICD-10-CM code is M76.822 which is defined as: Posterior tibial tendonitis, left leg.
How about the coding scenario?
When the NCCI edits are accessed, the two CPT codes to be billed are not bundled. Therefore, the coding scenario is the following:
CPT 28122 – LT (M89.372)
CPT 28200 – 59, LT (M76.822)

Query: Quick Remit Insurance Payments
I have noticed that more insurance companies are using Visa credit card type payments as an alternate payment method to regular checks in the mail. They offer a change to check payments but warn that there is no allowance to change back to the "Quick Remit" method. Has anyone regretted the change to check payment to avoid the bank charges? Should I just "take the money and run" to avoid delayed check reimbursements?

I read Ms. McCormick's explanation of why we should have a nail extender work in our office. I would say be careful. I have been a proponent of extenders and have coined the term podiacurist which is a mani/pedicurist doing routine care in a podiatrist's office. The major problem with this has been that once these individuals have been trained by you, they get emboldened and then will leave you and start a routine foot care practice down the street. They will be in direct competition with you and usually take some of your patient load with them.
If Ms. McCormick, who I believe owns a training program for these individuals, had the ability to mandate that they must work under a podiatrist or lose their certification, that would go a long way. In reality, a program through a podiatry school could make this a real legitimate profession and would ensure that the certificate had value and protected the podiatrists.
