OH Podiatrist Urges Diabetics to Keep Appointments During the Pandemic
Individuals with diabetes are more at risk for serious foot complications, some of which can lead to the loss of function in a limb or even amputation, especially when they’re not cared for in a timely manner. Since the start of the COVID-19 pandemic, podiatrists across the nation have seen many patients with diabetes delay care due to fear of exposure to the coronavirus.

“If a patient with diabetes ignores a wound or an infection, the possibility of amputation is very real,” said Dr. Ramy Fahim, a podiatric physician and surgeon at NOMS Ankle & Foot Care Centers. It’s crucial for these patients to keep their appointments with all of their healthcare providers, including their podiatrists, who are taking extensive precautions to provide a safe environment.”

CA Podiatrist Discusses the Future of Podiatric Diabetic Treatments
David Armstrong, DPM, PHD recently joined Ben Pearl, DPM on Fit Foot U to discuss topics ranging from the incidence of vascular disease in Covid patients to regenerative medicine for wound healing and treatments for diabetic neuropathy. Dr. Armstrong has been dubbed by Patrick Agnew, DPM as the Elon Musk of podiatric diabetes.

Armstrong said that hyperbaric oxygen is over-utilized with wound healing in some healthcare settings but has a place when it’s not used like an ATM machine. He added that 5 years ago, he was skeptical about topical oxygen for wounds as a separate technology but noted he was part of one of the randomized controlled studies that show efficacy. Dr. Pearl asked Armstrong about the implementation of some of the new expensive technologies such as sensor insoles for ulcer prevention that were once $170,000 per insole. Armstrong cited the example of his Apple phone’s high resolution camera which has gone down in pricing significantly.

NHS Clinic Shows Fluorescence Point-of-Care Imaging Improves Wound Healing
MolecuLight Inc., a leader in point-of-care fluorescence imaging for real-time detection of wounds containing bacteria, announced the publication of results in Diagnostics from a 2-year retrospective study of 229 foot ulcers. Diabetic foot ulcers represent a large and ever-increasing financial burden on the National Health Service (NHS), costing up to £962 million annually.

"Diabetic foot ulcers (DFUs) and their bacterial burden produce a significant strain on the NHS (National Healthcare System) and I wanted to understand and quantify the effects of point-of-care fluorescence bacterial imaging on the costs and patient healing rates since the MolecuLight i:X was implemented at our clinic," says Nadine Price, study author and podiatrist at the Whipps Cross Hospital. "Beyond the significant realized cost savings that the study uncovered, the fluorescence imaging procedure has had a noticeable impact on the quality of wound care provided to patients, without detriment to our clinical workflow," says Price.

11th Circuit Affirms GA Podiatrist's Prescription Kickback Conviction
A Georgia doctor was not harmed by a trial court's erroneous jury instruction over his involvement in a prescription kickback scheme that defrauded the federal government's Tricare military health insurance program, the Eleventh Circuit said in a published opinion Tuesday, affirming his convictions. A three-judge federal appeals panel ruled that podiatrist Alap Shah was rightly convicted in January 2019 of conspiracy to defraud the government and of receiving kickbacks, despite the jury being instructed that a violation occurs when one reason that payments are received is for writing prescriptions. Shah argued that writing prescriptions had to be proven as the main or only reason for accepting payments. The appellate court said Shah was paid $55,340 in kickbacks for writing prescriptions that cost Tricare more than a million dollars.

Query: Podiatrists and Access to COVID-19 Vaccination
In discussions with a colleague, we wondered if podiatrists will be given any priority in having access to COVID-19 vaccines when they become available. The news says that it will only be made available to doctors, nurses, EMTs and other first line responders. Since we also treat patients, will we be able to get this vaccine along with other healthcare workers?

I can only echo the sentiments of Dennis Weber's good friend Mark Sugar. I have known Dennis my entire career and I am also a proud Civic/Kern alumnus. Dennis practiced in Rockville, Maryland not far from my office. There was a fire in his building around 2008. He couldn't practice and I had space in my office so I called him and made arrangements for him to just see his patient in my space until he could move back into his own space. After a few months, he was able to move back into his building. The time we spent in the office together went so well that when his lease was up for renewal, Dennis and I made a deal for him to move his practice into my space as part of a purchase agreement.

Dr. Plon had described tremendous difficulties with practice stabilization over the past year with the onset of the COVID-19 pandemic. That same issue featured a PM editorial that was written by Dr. Barry Block indicating the importance or the significance of wound care becoming part of the podiatric practice and the stability it provided for patient volume and financial viability. This editorial reported on a survey from PM News indicating how wound healing was the least affected area of practice during the pandemic.
I am the president of the Academy of Physicians in Wound Healing. Our membership has indicated generally minimal impact on the wound care practice. I encourage readers who are concerned about this issue to do more investigation and learn more about becoming involved in wound healing. There are a number of steps that can help you achieve this easily, but it does take some time and some effort.
