WI Podiatrist Discusses External Causes of Arch Pain
Like many other foot issues, plantar fasciitis and other types of arch pain can come from two primary sources, says Maggie Fournier, DPM, a sports podiatrist in LaCrosse, Wisconsin, and past president of the American Academy of Podiatric Sports Medicine: Intrinsic factors, which are related to your individual anatomy and physiology, and extrinsic factors, which come from outside your body—think shoes, terrain, or training habits.

External causes of arch pain include ramping up your training too quickly. Adding too many fast miles proves especially risky, due to the added pounding, Fournier says. Making a switch from running on softer surfaces to harder ones—say, going from trails to roads or gravel to concrete—can also bring on tenderness in this area, as can always running on the same side of the road if the street is sloped or cambered. Fournier also sees arch pain in runners who hang on to their running shoes well past their expiration date.

7 Financial Reports Your Practice Needs to Run - Part 2
Most physicians readily admit that classes on how to run a successful medical practice were not part of the curriculum in medical school. While "financial reporting" might not have been a class at your medical school, experts say you need to at least become familiar with understanding some of the basic reports. Here are seven to track.
2. Profit and Loss Statement (P&L) - "A P&L tracks revenues and expenses, showing the profitability of your practice. It can help track key performance indicators such as total encounters, number of procedures, total charges, and collections. Red flags could include a sudden drop in collections. Before deciding on a course of action, look for an explanation behind any warning signs. For example, a decrease in collections this month should have been preceded by a decrease in charges last month." - Ada Stewart, MD, AAFP Board of Directors.

Query: ANS and Sudomotor Testing
I send many patients for vascular as well as neuro testing. There is a diagnostic machine available for purchase that allows practitioners to do the testing in our office. One of the machines I have found is called VitalScan which includes autonomic nervous system testing, vascular testing, and sudomotor reflex testing. Does anyone have any experience with these types of machines? Does anyone know if Medicare/commercial insurance allows reimbursement to podiatry for these types of diagnostic tests?

Without viewing other radiology views and clinical pictures, it sounds like your patient has a rigid/semi rigid cavus foot type. The rigid New Balance shoes along with the rigid orthotics may be too supportive for a patient with this foot type and excess weight. Try a New Balance shoe that is not as rigid/supportive with an orthotic material, (i.e., NickelPlasttm) that will cradle/accommodate the foot.

Chuck Ross started an interesting discussion triggered by his attending the always excellent Richard O. Schuster Memorial Seminar in biomechanics last week. He makes the statement that the concern regarding the lack of emphasis on biomechanics should be focused on the 3 years of residency education, where he notes that there is a “dramatic lack of a continuum once students graduate and enter residency programs, with few exceptions as the emphasis is solely upon surgery.”

Be very careful with -RT and -LT modifiers especially with BC/BS. I recommend not using -RT and -LT with BC/BS but make sure the ICD-10 codes are correct. Remember the ICD-10 does specify laterality. If your biller places an -LT modifier and the ICD-10 code indicates a right foot, BC/BS will request money back for laterality issues and you will need to send a corrected claim.
For x-ray and diagnostic ultrasound, use -RT or -LT or a -50 modifier for bilateral procedures. -RT and -LT modifiers are also insurance-specific. Remember that the ICD-10 codes indicate laterality. Healthfirst in NYS likes the Q8 modifier after the XU modifier, not like with MCR which is the opposite. Fidelis in NYS likes the DM code first when performing CPT 11721 or 1105* codes. Ask your local representatives - they usually are very helpful per my experiences in NYS for 1199, Healthfirst, Fidelis, and BC/BS (not as much).

Dr. Rubin, on this subject, states that if there is no symptom or clinical diagnosis, payers won’t pay or may not pay, or we may be demanded to pay money back if we get paid. I reject this simply because, if true, then my original statement that all of our proclamations of benefit of a diabetic foot exam or podiatry involvement in overall diabetic management can be beneficial not only during complications, but also in risk stratification and patient education are for naught.
If I am wrong about this, then there is a gaping chasm in the reality of practice and what we espouse as a core important contribution of our profession. Dr. Jacob’s response and...
