More Patients Opt to Replace Ankle Joints Instead of Fusion
Replacing hips and knees has become a common affair. Now, people are increasingly swapping out weakened or injured ankles to get back on their feet. Ankle replacement is an alternative to the more common fix, ankle fusion, restoring more range of motion and flexibility. Newer artificial joints can include parts such as mobile bearings that make moving up and down on an incline easier. Some patients who had fusions in the past may be candidates for ankle replacements. Dr. Shannon Rush, a podiatrist who performs foot and ankle surgery at the Palo Alto Medical Foundation’s Mountain View, CA center, says he sees one or two patients a month whose ankles were fused in a poor position.

Erika Spencer Brose, 44, a former dancer who now works as an emergency 911 operator, had her ankle fused after a car accident in 1992, but it was fused at a slight angle so she could wear a heel or boot. She says she became addicted to painkillers for a time, and was unable to walk very far.Two years ago, Dr. Rush removed the fused bone and put in a new ankle joint. “Overall it has made a huge difference,” says Ms. Brose. “It turns, it flexes, and it does things it never did before.”

A kinesiology taping workshop was given by the New York College of Podiatric Medicine's NYCPM's Physical Therapy Department this month for members of the Sports Medicine Club. Good information, real experience, and good times were had by all! Instructors were Gregory Taylor, DPT, Jessica Botvinick. DPT, and Nicholas Argerakis, DPM ('09).

FL Podiatrist Hosts Sports Injuries Event for UCF Students
University of Central Florida (UCF) medical students joined a group on physicians, orthopedic residents, and professional athletes for an in-depth session on sports injuries and treating athletes as a whole. The event was hosted by sports medicine physician and surgeon, Dr. Gideon Lewis.

“It’s very rare that medical students and residents have a chance to talk with professional athletes and physicians who treat these athletes on a daily basis,” Dr. Lewis said. “I just want them to think outside the box and understand that medicine is not just a science, but it’s also an art.”

NE Podiatrist Provides Winter Boot Buying Tips
Are your winter boots stylish or safe? It's a question many people are wondering about when it comes to finding the best shoes to prevent them from slipping and sliding on icy streets. "The last patient I saw had just a sprain, thankfully for her, but it could have been a hairline fracture," said Dr. Don Buddecke, a podiatrist in Bellevue. Winter footwear can make or break you or your ankles, especially with below freezing temperatures and icy streets.

Dr. Buddecke says that people are slipping and falling and it could be because of their shoes. "We see a lot of broken ankles this time of year," said Buddecke. That's why he says to be more careful, especially when shopping for a winter boot. "Some of the more stylish boots have more of a high heel to them, and it's not going to have much support; it's very slippery, so again it's that rubber type sole that's going to be the best," said Buddecke.

I was disappointed to find that your fine publication ran a press release from a local newspaper about some significant trouble I was tragically caught up with in 2004. At the time of your publication of this article, I was incarcerated and had no knowledge that PM News would print something of this nature, particularly as I, the subject, was given no opportunity to discuss the matter with PM News. While PM News runs a statement regarding the protection of privacy, I found it particularly lacking in this case. At this time I would appreciate the opportunity to reply to your article.

While I agree in principle with what Dr. Brody is saying -- and there is probably no better expert in Meaningful Use than he is. How efficient is it really? I would love to see this: Let's compare the EMR "structured data" from each medical provider that a single diabetic patient sees -- let's say their internist, their PCP, their opthalmologist, their podiatrist, their neurologist, and their orthopedic surgeon. I'll bet a whole lot that it's not all the same. It's probably not even close. But since all of these docs have their own separate EMR, and are getting the stimulus money, there are a whole lot of separate data.
Why didn't the government think about having a patient's PCP be the "hub" of meaningful use? They could keep a master EMR system of all of a patient's structured data. And as ancillary providers, our systems could access it! Then, it would all be cohesive, as Dr. Brody suggests it should be (of which I agree). But we don't have a cohesive system. We have multitudes of different medical assistants, nurses, and techs entering different structured data into different EMRs. I know the government's goal is to achieve cohesiveness. But with all the different systems, I don't see how it can be. Oh, and I have heard that not many doctors are achieving stage 2 Meaningful Use.

Before anyone uses long-acting or slow-release opioids, consider a recent statement by physicians of the American Academy of Pain Management who do not recommend their use in acute or post-operative pain management. Although it is a selling point of manufacturers of medications such as Xartemis, I personally use what works best for my patients and not necessarily what protects those who obtain and abuse opioids illegally.
Recommendations for acute pain are multimodal regimens combining acetaminophen, NSAIDs and opioids. Preparations of LA and SR medications are best reserved for chronic pain management.
