VA Podiatrist Discusses Dangers of Steroids With HS Students

The best way, financially and otherwise, to ward off steroid use among teen athletes is probably through parents and coaches -- and the old-fashioned approach that Arlington County physical education teacher Rocky Belk and Arlington podiatric physician Ben Pearl took last week. They met with about 60 high school students from Sheila Napala's physical therapy and sports medicine classes at the Arlington Career Center to discuss steroids and the 2008 documentary the students had watched, "Bigger Stronger Faster."

Dr. Ben Pearl

"We heard a lot about Barry Bonds and baseball players through [the congressional hearings], but we found that [steroid use] really has to do with wanting to be somebody special," Pearl said of his research. "A lot of us are trying to get somewhere because we're not happy with ourselves. We're not comfortable in our own skin, being what we are."

When Pearl asked how many students had used steroids or would admit to using them, no hands went up. When Pearl asked whether any of the students had taken other performance enhancers, several arms went up.

Editor's note: The January 09 issue of PM will feature an article by Dr. Pearl on the dangers of steroids.

QUERIES (NON-CLINICAL)

Query: Fairest Compensation

I am a new provider in an area that is primarily run in a managed care environment. I only see people from within “the system”, thus I am perhaps not as busy as I potentially could be. Recently, an established and extremely busy podiatrist asked if I would be interested to “moonlight” as a part-time provider. I currently work on a structured salary/benefits package. My employer is amenable to this opportunity as they are hoping that I would bring business back to their facilities (e.g., surgery, inpatient care, other referrals).

My employer believes that I should rbe eimbursed based on the RVU’s that I generate at the new office; the new office would find it easier to simply pay by the hour. What is fairest method to structure compensation? Does anyone have any ideas on where I would be able to gather advice/references for how to structure such a proposal?

RESPONSES / COMMENTS (CLINICAL) ACTIVE - PART 2

RE: Orthotics and Bunions (Simon Young, DPM)
From: Dennis Shavelson, DPM

I would like to pose these theoretical questions:

1) If an orthotic could be dispensed that reduced functional hallux limitus and allowed peroneus longus to leverage and power to the point that it produced renewed stabilizing power upon the first ray, locking it more securely in closed chain with every step, would the IM, HAA and Met primus elevatus of that patient's pathological medial column improve?

2) Could manual therapy, motor control, and additional vaulting of the dynamic arches of the foot then leverage and power the flexor hallucis longus and abductor hallucis, further reducing the development or advancement of a bunion deformity?

Although Neoteric Biomechanics has not yet been double-blind studied, the podiatrists working with this new paradigm are doing just that, every day, in practice.

Disclaimer: Dr. Shavelson is the inventor of Neoteric Biomechanics and has a financial interest in the paradigm.
