Best Collection Tactics for High-Deductible Patients - Part 2
High-deductible health plans are on the rise. Here are some methods to ensure your practice collects from patients with these plans.
2. Let patients know in advance how much they will need to pay.
Most Clearinghouses permit real-time and batch eligibility processing. It is important to know in advance whether you should collect more than just a co-pay.

CODINGLINE CORNER |
Query: Intermetatarsal Nerve Decompression
I performed an intermetatarsal nerve decompression of a neuroma for a patient in our clinic. We submitted code CPT 64726 (Decompression; plantar digital nerve) to Medicare. It was rejected as “treatment was deemed by the payer to have been rendered in an inappropriate or invalid place of service.” Is there a different code or another way this should have been submitted to obtain payment?
PM News Subscriber
Response: I would really be interested in finding out the name of the Medicare Administrative Carrier that made the decision to reject the claim for CPT code 64726 since it was performed in an office setting based upon the premise that “treatment was deemed by the payer to have been rendered in an inappropriate or invalid place of service.” I find this determination to be just plain wrong.
CPT code 64726 is defined, as stated in the above post, as: Decompression; plantar digital nerve. The appropriate ICD-10-CM codes that qualify this CPT code for reimbursement are: G57.61 (Lesion of plantar nerve, right lower limb) and G57.62 (Lesion of plantar nerve, left lower limb). I find it ironic that I have performed this procedure in an office OR in the past and I have been reimbursed by the Medicare Administrative Carrier. I took the liberty of accessing the website of First Coast Service Options, the Medicare Part B Administrative Carrier for the state of Florida. CPT code 64726 is reimbursable in BOTH a facility setting and in a non- facility setting, the office. The reimbursement for performing this procedure in either setting is exactly the same amount, 278.40 or 8.11 RVUs. I am making the assumption that if one Medicare Administrative Carrier reimburses this CPT/procedure code in an office setting, Place of Service Code “11,” the other Medicare Administrative Carriers should follow suit and not only reimburse this CPT code in Place of Service Ambulatory Surgical Center (POS code “24”) or Outpatient Hospital (POS code “22”).
There is not another CPT code that should have been billed; or is there another way that the claim should have been submitted to obtain payment? Unless the Medicare Administrative Carrier of the posting physician confirms that CPT code 64726 cannot be performed in a non- facility setting, this physician should have received reimbursement.
My suggestion is to go onto the website of the Medicare Administrative Carrier in question and access the “Fee Schedule.” When you enter CPT code 64726 and the date of service in question, I am extremely confident that CPT code 64726 will be reimbursable in a facility setting AND in a non-facility setting. If this is indeed the case, you need to file a Level One Appeal (i.e., Redetermination) with the Medicare Administrative Carrier and present them with all of the appropriate documentation to support your position. Unless a Place of Service Code other than “11” was entered on the claim, you should be reimbursed.

RESPONSES/COMMENTS (OBITUARIES) |
RE: The Passing of Dennis Weber, DPM
It is with great sadness that I announce the passing of my good friend Dennis Weber from pancreatic cancer. Our lives mirrored each other in the respect that we lived in suburban Maryland outside of Washington, DC. and after college attended podiatry school - he in Cleveland and I in Philadelphia. We didn't know each other then, but I believe it was divine intervention when we both attended a Pennsylvania CME seminar which "starred" Drs. Earl Kaplan, Irv Kanat, Bob Weinstock, and Sol Luft. We came away from that meeting and agreed that Civic-Kern was the place to go after school. Luckily, a few years later, our dreams came true and our residency programs overlapped and we became good friends. We then went back to suburban Maryland and opened our practices in different locations but became good friends.

Dr. Dennis Weber |
We attended meetings together - served on some committees together - opened our own ambulatory surgery centers and played a little golf and poker too. Dennis was my "go-to guy" - if I had a question - whether personal or business - he had the ability to communicate with me in a "soft" way that was non-criticizing and always positive. We both retired to Southern Florida and lived near each other so we made a plan to have monthly breakfasts together which were a lot of fun and informative. He lived and died with dignity, which I will never forget. To his loving wife Ellen and their two children Marc and Julie and their grandchildren - I send my deepest sympathies.
