Common Foot Hygiene Problem for 40+s is Not Drying Between Toes: NY Podiatrist
"Many people over the age of 40 have some unhealthy foot hygiene habits," says Bruce Pinker, DPM, a foot and ankle surgeon at Progressive Foot Care in New York City. The most common, he says, is failing to dry between the toes after bathing. "If moist feet are placed inside closed shoes, the feet may become warm and fungus growth may be promoted, due to the wetness and the footwear, which prevents breathability," Dr. Pinker explains. This can lead to blistering and cellulitis, as well as athlete's foot.

"By simply drying between the toes after bathing, one can help reduce the occurrence of such infections and therefore reduce foot discomfort," he says. In addition, Pinker recommends using over-the-counter powders, which can be placed directly onto your feet or into socks to reduce wetness.

7 Financial Reports Your Practice Needs to Run - Part 1
Most physicians readily admit that classes on how to run a successful medical practice were not part of the curriculum in medical school. While "financial reporting," might not have been a class at your medical school, experts say you need to at least become familiar with understanding some of the basic reports. Here are seven to track.
1. Appointment Analysis- "Although analyzing appointments is not considered a financial report in the traditional sense, patients who do not show up for scheduled appointments cost the practice money. The ideal fill rate is usually about 90 to 95 percent. Calculate the actual fill rate and compare it to the desired fill rate to decide if any correction or additional action is needed. This report also shows which time of day or which months are busier for your practice. This helps determine if and when additional help is needed." - Ada Stewart, MD, AAFP Board of Directors.

Query: Modifiers -LT and -RT
I have been having a difficult time understanding when it is necessary to use -LT and -RT to specify laterality. I am being told that it is unnecessary to specify laterality unless I am doing a specific procedure on both feet. Am I getting incorrect information? It seems easier to just use -LT and -RT for every CPT code and let the payers ignore it if that is their protocol. What do you think?

Query: Modifiers can be tricky. Using them incorrectly can obviously get your claims denied. Using them when you don’t need them can also confuse the computer and get claims kicked out. If you bill an RT, the computer system might be searching for the corresponding LT procedure and when it can’t locate it on your claim, the claim gets denied.
If there is ever a question as to which side you did, the ICD-10 code might help, but ultimately your chart note will be the definitive documentation. I use RT/LT 100% of the time when billing x-rays. Outside of that I use them when doing procedures on two feet depending on if other modifiers may be more appropriate.

RE: Plantar Mechanical Stress Tied to Formation of Plantar Melanoma

A study in the Journal of European Academy of Dermatology and Venereology was reported by Dermatology Advisor indicating that “mechanical stress on the plantar surface of the foot, which is typical of normal weight-bearing on the foot, is associated with a higher formation of plantar melanoma compared with other non-weight-bearing areas of the body.” It indicated that “the heel was the most common region with plantar melanoma, followed by the forefoot, plantar aspect of the toes, lateral midfoot, and arch.”

Greenway was fined over $50 million by the DOJ for fraud as it related to their EHR system. As part of their settlement, they were REQUIRED to transition records to an EHR platform of their customer's choice FREE of charge. This doctor should not be paying anything for the conversion.

The normal diabetic foot examination - The discussion of how to submit charges for a “normal foot examination” reminds me of the old adage “the eye sees what that the mind knows.” If the examination is cursory, then I suppose one may lack a diagnosis for the justification of a diabetic foot examination.
I propose that the ability to diagnose relevant foot pathology in the diabetic is only as good as...

Dr. Ron Werter replied to my comments of the other day: "I do not take issue with Dr. Busman's comments about bundling. I do take issue with his surgery...a toe exostectomy taken to the OR and the patient sedated and having a painful recovery period? Does he also take a patient to the OR and sedate them for a P&A. What’s wrong with just a digital block for an exostectomy or even an arthroplasty?"
I fully agree with Dr. Werter and even if I was still in practice, I would never take a patient to the OR for such simple procedures, but would do them in the office under digital block. I retired from podiatry and worked as a circulating OR nurse for 10 more years, during which time I did indeed see podiatrists bring patients to the OR for very minor procedures. Sometimes the anxious patients had insisted on it, refusing the procedure otherwise. Some matrixectomy patients were children who had failed office attempts at digital blocks and truly needed the procedure. There were indeed instances were no such special conditions existed. Sad, but true.
