OH Podiatrist Conducts Diabetes Webinar for VA
As part of Diabetes Awareness Month, the Amputee Coalition and Organogenesis Inc. convened an educational webinar for U.S. veterans, their families, caregivers, and clinicians on preventing limb loss for patients battling diabetes. "Patient education on proper care of the diabetic foot is an absolutely critical component of diabetes management and prevention of lower limb amputations," stated Jeffrey Robbins, DPM, Director of Podiatric Services at the Veterans Administration and Lois Stokes Cleveland VA Medical Center.

During the webinar, Robbins advised diabetic patients to check their feet daily for any cuts or sores, wear appropriately fitting shoes and socks, and avoid going barefoot at all times as even the smallest cut or blister has the potential to turn into a chronic non-healing ulcer. Robbins also discussed the Amputation Prevention program at the VA and how patients can best navigate the system to get the care they need.

Aussie Sports Podiatrist Returns to Her Roots
Toowoomba born and bred Jessica Hannah has launched her own podiatry center in Middle Ridge. Sole Focus Podiatry is dedicated to achieving the best possible outcomes for all patients. With an extensive background in the region's sporting industry, Hannah felt it necessary to give back her expertise to the city which gave her so much. Hannah's upbringing was all about hitting the sport courts and fields.

"Sports is huge in Toowoomba; it's exciting, dynamic, and at quite an elite level here," she said. "I grew up playing netball and basketball here in Toowoomba so ... was always really interested in the body, how it worked, and the sports injury side of things." So coming into it, I just wanted to be in the sports industry and in health/physical activity. I wanted to be able to help people from high performance athletes all the way down to general footcare."

Challenges Ahead for Portals
With the rush to implement electronic health record systems and meet meaningful use requirements, there have been mixed results in getting patients to use patient portals. As the preferred approach for engaging patients and giving them access to their medical records, portals were developed as an additional technology linked to EHR systems by system vendors. However, clinicians and federal regulators are re-assessing their value and utility.
Simply offering patients online portals does not mean they will use them. Raj Ratwani, scientific director of MedStar Health's National Center for Human Factors in Healthcare, believes portals are not as usable as they could be. "In most cases, patient portals and other health IT have not been designed to support patient needs and do not present information in a manner that is understandable and useful," he says. "Consequently, these technologies are underutilized by the public."

Five Steps to Improve the Patient Experience - Part 3
2. Honor patients with respect and attention
Physicians, managers, and staff must equally commit to give patients the attention they deserve throughout the touchpoints of their visit; from the time the patient arrives for a scheduled appointment until the time he leaves at the end of the visit. Make sure all staff are trained to consistently provide a specific standard of service established by your practice manager, and make sure to hold them accountable. This means measuring how well you are delivering on your promise to meet a standard of care and service. It can be accomplished by implementing patient post-encounter interviews, establishing patient dispute procedures, and/or conducting patient satisfaction surveys.
Provide staff training through an annual customer-service workshop that will reinforce your practice's commitment to patient service. It can be conducted during an extended lunch hour where meals are provided and there is time to share concerns and ideas on how to become a best-service practice.

Not only are we not getting incentives or payment for the work we do in the form of PQRS measures, but it costs us not to be penalized. I checked with the PQRS Registry today recommended by my EMR vendor, and it will cost $500 for the first NPI/tax ID combination that we bill under, and $250 for each additional combination. What's wrong with this picture?

As healthcare policies evolve, it begs the question of “What type of insurance does one have?” In this recent post, there is reference to ACA exchanges which states “[they] … now provide a path to insurance for 10 million Americans.”
When a person contracts with a healthcare insurance plan under the assumption they are purchasing coverage for necessary healthcare needs, are they really? Many plans come with high monthly premiums, co-pays, co-insurance, and deductibles. Now, the average person over an annual period of urgent visits to providers is paying the lion’s share of their healthcare costs, in ADDITION to the premiums. This equates to one “contributing” to a catastrophic plan, not an insurance plan that meets their routine or urgent healthcare needs. It wasn’t long ago that type of catastrophic plan cost the average person about $100 a month.
If one is able to get low or no-cost insurance, what type of insurance do they have, if many services are not covered or most providers don’t accept that plan? And, what about the person able to obtain “premium” coverage that is affordable to them? It is not uncommon for that person to find their insurer denies the recommended medications, investigative studies, or treatments.
It can certainly be contended that these individuals “have” healthcare coverage, however, what does one contend when the so-called coverage is not accepted or not truly “covered?” If one looks at the top five insurance plans in this country, it becomes obvious that even catastrophic coverage costs to the insured can easily become catastrophic to that person’s finances. I'll leave the so-called "coverage" of Medicare for another time.

In response to the articles cited on the link between dementia and diabetes: At the 72nd Scientific Convention of the American Diabetes Association in Philadelphia, I attended a fascinating lecture on the separate glucose metabolism system in the human brain and the derangement of that system being the cause of Alzheimer’s dementia and possibly Parkinson’s disease. This researcher (whose name I don’t recall) decided this constituted a new type of DM and termed it Type IV Diabetes. I believe he chose this number in the chronology, as Type IIIa DM is associated with PCOD (polycystic ovary disease) and Type IIIc DM is associated with PDAC (pancreatic ductal adenocarcinoma).
As the research and findings expand, we need to have the nomenclature cleared up so that the water isn’t muddied by different pathological manifestations of the same disease state sharing the same name.
