United Arab Emirates Podiatrist Discusses Flatfoot

"Pain on the inside of the ankle or the arch, or a decreased ability to walk or run due to aching feet is often the first sign of a flatfoot developing," says Dr. Caterina Obrador Riera, a podiatrist at the Dubai Bone and Joint Centre.

It is not just the feet that hurt. Flatfoot affects other parts of the body too. Dr. Riera says: "Flat feet can affect the knees, hips, and back. The arch collapsing inwards places stress on the knee, which may rotate the hip and cause back pain. Avoid high heels and flat shoes with no support, such as ballet flats and flip-flops."

Canadian Podiatric Diabetes Event Celebrates 10-Year Anniversary 
 
The British Columbia Podiatric Medical Association recently celebrated the 10th anniversary of the Live Well with Diabetes event. The event started with a two-day diabetes-focused seminar for medical professionals, then a public forum, which was followed by a dinner provided for people with diabetes and those who are underprivileged. This year’s event also incorporated the 2011 APMA Region VII Conference within the seminar portion of the weekend.

The full weekend event was started by two B.C. podiatrists, Dr. Tim Kalla and Dr. Joseph Stern. Over the years, the event has expanded both in terms of involvement and activities. In addition to a growing list of podiatrists involved, the event also includes numerous event partners, such as the Canadian Diabetes Association, Paris Orthotics, Lady Sport, and the University of British Columbia.

Dr. Tom Tilkens is a 66-year-old podiatrist, Green Bay native and game-day fixture on Ridge Road, just west of Lambeau Field. Tilkens grew up on the east side of town within sight of the old city stadium where the Packers played between 1925 and 1956. His parents let fans headed to the stadium park their cars on family property during game day. When he was 5, his dad handed him a sign and put him to work, waving cars onto the front lawn and collecting the nickel-per-vehicle charge from customers. Today, Tilkens practices medicine in the shadow of the new city stadium where the team has played since 1957. And since his children have married and moved, he still parks cars on game day. Some fans come back to park on his property year after year and have become "like family," he said.

Romney Calls for More Medicare Competition

Republican presidential candidate Mitt Romney said he supports offering Medicare beneficiaries a choice between traditional Medicare and "private health plans that provide at least the same level of benefits." Romney spoke at a gathering of the Americans for Prosperity Foundation, a conservative organization focused on economic reform. The former Massachusetts governor insisted that competition in Medicare would lower costs and increase the quality of healthcare for beneficiaries, saying, "That's the answer for Medicare." He did not elaborate on how these new plans would relate to existing Medicare private plans.
 
During the speech, Romney offered some of his most specific proposals yet on Medicare, Medicaid, and the health system reform law. For example, he said that he supports capping federal spending on Medicaid and increasing Medicare's eligibility age by two years to reach 67, and he also opposes tax hikes to pay for increased Medicare spending.

PRACTICE MANAGEMENT TIP OF THE DAY

Choose Your Words Wisely

Don’t let miscommunication hinder productivity and harm the bottom line. When conveying a message, choose the right words and the appropriate amount of words. Follow this strategy:

  •  Go right to the point. Rambling and dancing around the subject forces listeners to waste time searching for the buried message. Prevent listener annoyance and misinterpretations by saying precisely what you mean. Tip: Practice articulating difficult messages in advance.
  •  Minimize the qualifiers. Hedging around the topic with words such as “maybe” and “could be” dilutes the impact and causes confusion when you bury the true message. Example: Saying, “There could be a problem if we don’t get your answer by Friday” makes the message seem less important than, “We need your answer by Friday.”
  •  Simplify and clarify. Employees who receive unclear and incomplete instructions waste time trying to decipher the request and risk making mistakes. Avoid being terse and vague by providing the details needed to accomplish the task. Example: Saying, “I need information on Mrs. Jones” fails to reveal specifically what information you need and when you need it.

Has anyone billed L1971 (ankle foot orthosis, plastic or other material with ankle joint, pre-fabricated) to Medicare? If so, what is the Medicare fee allowance for this device? We have used other similar codes (L1960 and L1970) for the Ritchie and Arizona braces, but never this pre-fabricated one.

I would be happy to share any information about lasers. I believe that podiatry has gotten off to a bad start with these lasers. We are so afraid of just being truthful with a patient when we are charging cash for a procedure. Cash does not guarantee 100% success, but it can guarantee confidence if presented correctly. I have a successful reputable practice and the last thing I would do is perform a service for my patients that would harm my reputation. So, yes I think you can incorporate laser treatment with good results and maintain your standing in the community. 
 
I did extensive research before starting with lasers. When I look at my own success rates, I would say that a fair percentage to represent to a patient is 65-70%. I have had success with...

RE: Wasting Our Time and Money

I have read the opinions of others about why we should 1) aspire for equal footing with MDs, 2) be defined as doctors for Medicaid, 3) jump through all of Medicare's hoops w/EMR, Rx, PQRI, etc. to not get penalized. I see postings for "supergroups" started by some podiatrists who are just praying on their own to make money. They are not true groups with "shared risk" as defined by the FTC, so they will be shut down, but not before the "organizers" of these easy groups make their cut of your revenue. 
 
Enough of the whining. The point is all these efforts, well-meaning as some of them may be, are just getting us from storage to a first class stateroom on the Titanic! The APMA and AMA need to work together to...

I've worked as a partner in two different orthopedic groups and one multi-specialty group. Taking blood pressures has nothing to do with advocating that podiatry is mainstream medicine. Most ortho groups don't take blood pressures. There's nothing medical or mainstream about taking a blood pressure that validates anyone medically. A machine at Walgreens can do one for you.
 
My previous internal medicine colleagues were frustrated that they had to squeeze in a pre-operative visit for my ankle fusion patient. I can only imagine them having to squeeze in an extra patient who had a high diastolic one time in my office. Taking pressures isn't difficult. Filling out a referral isn't difficult. It doesn't prove anything to our allopathic colleagues. It creates liability. It forces extra work. It frustrates our PCPs. I'm all about decreasing liability. We are specialists. We shouldn't test what we aren't going to treat. Should we all be ordering baseline CBC and CMPs? Where does it end? A high blood pressure, in and of itself, is diagnostic of nothing. A documented recommendation for my patients to seek regular check-ups to their PCPs is all I do as their specialist. If they want a BP, they can go to Walgreens.

RE: The Passing of Paul J. DiMeglio
 
I was out of town lecturing when I heard the horrifying news of the passing of my friend and huge supporter of podiatric medicine, Mr. DiMeglio. Not only was Paul a highly successful businessman, but he was a renaissance man. A well traveled wine connoisseur, bigger than life, fast moving, and full of wit. This highly educated individual was an innovative force in the development of medications for the podiatric field. I personally have felt a huge void in his passing. He will certainly be greatly missed. I'd like to send my deepest sympathies to his lovely wife and entire extended family including the employees at The Tetra Corporation. May God bless his soul.
