For PA Podiatrist, There is No Pivot During the Pandemic
Margie Plon, DPM has weathered the storm of COVID-19, but for the first time in nearly 40 years of practice, the Elkins Park podiatrist cannot do any long-term planning because she doesn’t know how the pandemic is going to play out. Dr. Plon is dealing with two sets of headwinds. First and foremost is that she has less patient volume than she did pre-Covid - and it's not due to lack of demand. The issue is due to safety protocols; she is not allowing patients access to her waiting room.

There has also been an increase in the cost of supplies. Plon says she is paying three to five times more than she did pre-Covid for necessities such as gloves, masks, and wipes, as supplies are more limited. So, if she orders 10 boxes of gloves, she might only be able to get five. "And I can't pass that additional cost to my patients," Plon said. "There is no pivoting with my practice. I can't switch to something else."

Albert Hyunjae Lee, DPM, DC has opened Integrative Foot & Ankle in NY. Dr. Lee holds two doctorates - in chiropractic and in podiatric medicine. As a practicing chiropractic physician, he saw prevalence of lower extremity diseases in patients with chronic spinal conditions. He wanted to further specialize in treatment in diseases of the lower extremities. So he attended the New York College of Podiatric Medicine.

Continuous Diffusion of Oxygen Therapy Cost-Effective for Advanced DFUs
Treating patients with advanced diabetic foot ulcers using continuous delivery of oxygen (CDO) therapy cost less and offered better outcomes compared with negative pressure wound therapy, according to a study published online in the International Wound Journal. “The results of this economic evaluation suggest that CDO therapy may reduce healthcare economic burden with a modest increase in quality of life outcomes,” researchers wrote. “Healthcare decision-makers should consider the inclusion of CDO for the treatment of diabetic foot ulcers.”
In a simulated cohort of 10,000 patients, the model estimated 32% would have wound closure after CDO therapy compared with 27% after negative pressure wound therapy, according to the study. The average 5-year cost per patient receiving CDO in the base case was an estimated $78,500 compared with $83,300 for negative pressure wound therapy, resulting in a savings of $4,800 with CDO.

STARK LAW UPDATE |
CMS Finalizes Changes to Physician Self-Referral Regulations
CMS announced it has finalized changes to the Physician Self-Referral Law. The law prohibited physicians from making referrals to an entity for certain health care services if the physician had a financial relationship with the entity. The changes made to the Physician Self-Referral Law, also known as “Stark Law,” will create clearer paths for providers to serve patients through enhanced coordinated care arrangements, resulting in better access and outcomes for patients, according to CMS.
Proposed policies finanlized in the rule include the following: 1) additional guidance on key requirements of the exceptions to the physician self-referral law to make it easier for physicians and other health care providers to make sure they comply with the law; 2) protection for non-abusive, beneficial arrangements that apply regardless of whether the parties operate in a fee-for-service or value-based payment system, such as donations of cybersecurity technology that safeguard the integrity of the health care ecosystem; and 3) a reduction in administrative burdens that drive up costs by taking money previously spent on administrative compliance and redirecting it to patient care.

RE: Therapeutic Shoe Update

As of November, there is an update to the Therapeutic Shoe Policy which some of you will be happy with and others may be troubled by. A summary is provided below. Both nurse practitioners (NP) and physicians assistants (PA), with certain limitations, are now afforded the ability to certify patients as diabetic with respect to their need for therapeutic shoes/inserts. There are two pathways, both of which have limitations which are summarized below.
Pathway #1: NP/PA Working Incident to an MD/DO Effective November 5, 2020 If an NP or PA is working both under the direct supervision of an MD/DO and billing under the NPI of the MD/DO who was/is supervising the DM care, then the NP/PA can perform the examination to demonstrate that the patient is diabetic and needs therapeutic shoes. They may also sign the certification form that the patient requires therapeutic shoes. The exam records of the NP or PA, however, must be...
