Western University of Health Sciences faculty, staff, and students were among the volunteers who stepped up to address the homeless problem by providing free medical, dental, vision, and podiatric care to over 1,000 homeless and others in need at the Care Harbor mega clinic at The Reef in downtown Los Angeles Nov. 15-17, 2019.

It's feet's time to shine! TLC is bringing a new reality show into your life all about funky feet. Starring Ebonie Vincent, DPM and Brad Schaeffer, DPM, My Feet Are Killing Me will feature the foot docs taking on all kinds of podiatric conditions. And it's not for the faint of heart. "This program examines podiatric medical conditions and the procedures involved with treating them," the voiceover says. "Due to their graphic nature, viewer discretion is advised."

The nine-part series premieres on Thursday, Jan. 2 at 10 p.m. In My Feet Are Killing Me, the expert podiatrists and surgeons are put to the task of restoring patients' confidence and getting them, quite literally, back on their feet. The doctors juggle all sorts of cases, from wart clusters and fungus to toe amputations and foot reconstructions. Cases range from a paranormal romance novelist who wants her sixth toe amputated and a former professional dancer who has a bunion so big she has problems walking, to a man suffering from Proteus syndrome and a nudist with severe fungus.

Painful Hallux Could Indicate Gout: CA Podiatrist
You notice an enlarged, painful big toe. It could be gout. Gorged on wine and steak? The painful aftermath could be gout, a type of arthritis that often affects the joint of the big toe. Foods high in purine, a chemical compound found in red meats, fish, and certain alcohol, can trigger an attack by raising levels of uric acid in the body. Uric acid is normally excreted through urine, but is overproduced or under-excreted in some people.

“You’ll see the deposition of the uric acid in the joint, most commonly the big toe or the ankle,” says Bob Baravarian, DPM, a podiatric foot and ankle specialist at Providence Saint John's Health Center in Santa Monica, CA. “The patient will wake up with a hard, red, swollen joint. It’s extremely painful.” A doctor may prescribe anti-inflammatory drugs for short-term relief and medicines to reduce uric acid production. You may also need to follow a low-purine diet for long-term prevention.

Over 200 people were on hand in Daytona Beach for the 2010 Fall Conference of The American Academy of Podiatric Practice Management (AAPPM). One of the highlights of the Conference was the presentation of the Egerter-Greiner Award to longtime AAPPM board member and past president Bill McCann, DPM.

Query: EHR Charging for Medical Records
I have been with the same EHR company (Success EHS by Greenway) for over 10 years. They are sunsetting the EHR at the end of the year. They wanted me to stay with their recommended EHR product which I did not like. Now, I switched EMRs with a different company and they want to charge me 14K for all of my patient records. Is this legal? I have paid them a percentage every month for practice management and billing.

Editor's Comment: PM News does not provide legal advice. The answer may lie in the contract agreement you signed when you bought the EHR. The lesson to be learned is to always anticipate the possibilty that you may someday switch to a different EHR system and negotiate accordingly. Among the negotiated items are a liquidated damage clause if you end your contract prematurely and a clause that assures that the data provided to you will be in a "useable" format.

I respectfully disagree with a recent post from my colleague Dr. Werter. A bone surgery of any kind, whether it be a simple exostectomy or joint reconstruction, is much different than a P&A in terms of wound depth and potential for infection. An OR should be much cleaner than anyone's office, and in today's age of drug resistant bacteria, anything we can do to limit post-operative infection (especially osteomyelitis) should be considered and most likely carried out. With respect to anesthesia, light sedation to perform the digital block and keep the patient comfortable during the surgery is a good thing.
In patients who have very high anxiety, it is proper and a necessary course of action. What about the pediatric patient who is screaming, crying, carrying on about "the shot" before doing a toenail procedure? I have taken quite a few of these patients to the OR simply to make sure that I can actually carry out the procedure safely and efficiently. In those cases, sedation for a P&A is absolutely necessary! Lastly, pain is VERY subjective. We all know this.
Patients' pain varies widely from the simplest skin closure or P&A to the most complex reconstructive procedure. Post-operative pain, in my opinion, tells you very little about the surgery, the technique, or the surgeon. I never tell patients in absolute terms how much or how little pain they should expect after any procedure I perform. Manage the patient, the pain, the wound, and move on.

The point people miss with the name plates is that the title of all the representatives always says “Dr., Mr., Ms., Miss, or Mrs.” Name plates don’t show the degree for anyone. I’m surprised by Dr. Jacobs’ comments that may have been interpreted in a negative tone by some about Dr. Wenstrup. I am so proud of Dr. Wenstrup. I have met him and hope he is President one day. I also feel Dr. Jacobs has been a great icon in our profession.

I believe the many comments and responses made on this subject amount to trying to answer this question: Can you bill a new patient Evaluation and Management (E/M) service for a preventive foot examination of a diabetic patient referred by a physician if there are no present diseases or disorders of the lower extremities? In addition, there would presumably not be medical record documentation of the required E/M "Chief Complaint" or "Nature of the Presenting Problem."
On the issue, while a podiatric comprehensive diabetes foot examination is universally accepted to be...
