Jamil Hossain, DPM has joined Ankle & Foot Centers of America. He earned his Doctorate degree in Podiatric Medicine from the Temple University School of Podiatric Medicine. He completed his podiatric surgical residency at Hahnemann University Hospital, a program of Drexel University located in Philadelphia. While at Hahnemann, he received extensive training in rearfoot and ankle reconstruction.

PA Podiatrist Recalls Treating the Lubavitcher Rebbe
“This is not a story I’ve told, other than to close family members and friends… it’s a 28 year old story,” says Warren Joseph, DPM. The news travels quickly – “A doctor who treated the Rebbe is sharing his experience for the first time!” Within minutes, the Zoom meeting of Chabad rabbis has reached its capacity of 1,000 participants.

In late 1992, Dr. Joseph was a faculty member at the Pennsylvania College of Podiatric Medicine. One day he received a call from Elliot Udell, DPM, a podiatrist in New York, who asked him to travel to Brooklyn to see an important patient who had an infection in his foot. That person turned out to be the famous Lubavitcher Rebbe. Dr. Joseph’s encounter with the Rebbe had a profound impact on the way he and his wife raised their sons, and on how the family came to view and observe his religion.

“Make sure the toe box is wide enough, as a round toe is better than a narrow or pointed toe box,” says Dr. Parthasarathy. “Your toes should be able to wiggle. Check the inner sole of the boot; is it removable, is it thick and cushioned? Those are good signs that they’ll provide a lot of support. A removable insole allows you to use your custom orthotics, or more supportive insoles if needed, and a textured sole is important for traction during inclement weather.”

Administration Announces Prescription Drug Payment Model
HHS Secretary Alex Azar has announced a drug payment model through the Center for Medicare and Medicaid Innovation at the Centers for Medicare & Medicaid Services that will lower Medicare Part B payments for certain drugs to the lowest price for similar countries and save American taxpayers and beneficiaries more than $85 billion over seven years.
Starting in January, the model, known as the Most Favored Nation (MFN) Model, will test an innovative way for Medicare to pay no more for high cost, physician-administered Medicare Part B drugs than the lowest price charged in other similar countries. Following the President’s recent Executive Orders to lower drug prices and improve access to life-saving medications, the MFN Model will protect current beneficiary access to Medicare Part B drugs, make them more affordable, and address the disparity of drug costs between the U.S. and other countries.

A great idea is to branch out into a range of different services to allow the maximum possible options for your patients. Options include physio, acupuncture, beauty treatments, etc.

You don' t even need to employ these service providers as extra members of staff; you could rent out space, and they can set up as businesses in their own right.

Taking this approach means the smallest possible outlay for you, but being able to offer the greatest possible range of services and give the patients a great deal of choice.

Query: Global Period for Tenotomy Surgery
Can someone clarify the global period for percutaneous flexor tenotomy for flexible hammertoes? I thought the global for CPT 28010 was ten days. My experience is 90 days! The same as a bunion correction?
I performed an in-office flexor tenotomy successfully on one foot; now the patient requests correction for the other foot. I understand how a longer global would discourage so-called “serial surgeries” for higher reimbursements for multiple procedures on one foot; but the opposite foot as well?

Response: No, the global for “major” surgery is 90 days and CPT 28010 is in that club.

If you are doing the contralateral foot, make sure you document very clearly that it is the contralateral limb, use the digital modifier when possible, a -79 for your procedure. I would expect the need to appeal for payment.

RE: The Passing of Peter M. Kaminski, DPM
Dr. Peter M. Kaminski, 66, of Harveys Lake passed away peacefully surrounded by his family at Geisinger Wyoming Valley Medical Center. Dr. Kaminski obtained a pharmacy degree at Temple University. While working as a pharmacist in Philadelphia, he furthered his education graduating from the Pennsylvania College of Podiatric Medicine.

He completed a two-year surgical residency at St. Mary’s Hospital in Philadelphia and later established an office of podiatry in Plymouth and Dallas where he practiced for the past 35 years. He was a member of the American Podiatric Medical Association.

Eneslow has been successful in helping numerous healthy males who present with fat-grafting to the heel. To relieve impact forces and reduce pain, we use a heel rocker, often with SACH shock-absorbing material under calcaneus. The goal is to delay heel contact until the body is over the foot and increases forces to the midfoot.

Shoes that start with a thick (one inch or thicker) midsole/outsole can be modified. Otherwise, most walking shoes can have material added and modified by pedorthic technicians to accommodate this deformity.

As the old saying goes - K I S S (keep it simple...). First, check for a leg-length discrepancy. If nothing else is helping, start with a 1/4" thick felt or Korex rubber cork heel lift and cut a big enough hole to accommodate the painful area. Bevel the inside edges for comfort. The material you use should be rigid enough so it doesn't compress. This would at least isolate the area and eliminate a few other possible etiologies. Make sure you add the same thickness lift in the other shoe for balance unless there is a leg-length discrepancy.

There is no “best” machine. It depends on what you plan on doing with it, but the hand-held models have too small a screen to be useful for our diagnostic purposes. That being said, if you feel you have a knack for diagnostic imaging and love the intricacies of ultrasound, then look at the 20/20 units which are pricey but amazing. If you just want a very basic unit that you can do most imaging (but not as detailed as the 20/20) and for guided injections, then look at something cheaper like the Mindray DC-30 machine.
I also recommend that you purchase a new machine.
