AAPPM Fall Conference Addresses Healthcare Reform 
 
The American Academy of Podiatric Practice Management (AAPPM) completed its 2014 schedule of events with another packed Fall Conference at Lago Mar Resort in sunny and warm Ft. Lauderdale, Florida. The nearly 200 physicians, assistants, executive managers, and exhibitors experienced an agenda that featured a full day of workshops followed by a weekend agenda with three different tracks that offered nearly 15 hours of continuing education credits.

Speakers touched on topics from healthcare reform to the latest in wound care. The conference also included a full-day devoted to topics and challenges facing practice executive managers and medical assistants that also had credits available for the PMAC credential.

Amoxicillin Adverse Effects Under-reported, Under-recognized
 
The adverse effects of amoxicillin appear to be under-reported in clinical trials. In particular, treatment with amoxicillin-clavulanic acid commonly results in diarrhea, and treatment with amoxicillin-clavulanic acid or amoxicillin commonly results in candidiasis. Malcom Gillies, PhD, from NPS Medicine Wise, Sydney, New South Wales, Australia, and colleagues published their systemic review and meta-analysis of randomized trials online November 17 in the Canadian Medical Association Journal
 
The authors found that 10% of courses of treatment with amoxicillin were associated with diarrhea. They calculated an odds ratio for diarrhea of 3.30 (95% confidence interval, 2.23 - 4.87) for patients receiving amoxicillin-clavulanic acid compared with control patients. The odds ratio for candidiasis was significantly higher, at 7.77 (95% confidence interval, 2.23 - 27.11).

I had not previously heard of them and am not familiar with their reputation. I was also unable to find anything on the web that was not linked to their sites. Has anyone had any experience with them?

Which code would be most appropriate to bill when the prominent head or base of the phalanx is partially removed due to a painful overlying corn, CPT 28126 or CPT 28160?

Response: Let's look at the two CPT codes you asked about 
 
1) CPT 28126 - resection, partial or complete, phalangeal base, each toe 
2) CPT 28160 - hemiphalangectomy or interphalangeal joint excision, toe, proximal end of phalanx, each 
 
So, if you are removing an interphalangeal joint, meaning the base of the middle phalanx and the head of the proximal phalanx, then it is CPT 28160. 
 
If correcting the hammertoe, then it would be CPT 28285 -correction, hammertoe (e.g., interphalangeal fusion, partial or total phalangectomy). This procedure allows for removal of part of the phalanx, i.e., the head of the proximal phalanx. If just doing the phalangeal base, then use CPT 28126.

In yesterday's "Podiatric Products in the News" section, the story spoke about doctors discussing the Hyprocure procedure and described "The procedure is nearly pain-free." What is nearly pain-free? What a dangerous use of wording when discussing a surgical procedure. I think using "rosey" language is blatant deception and unethical. 
 
I am a strong advocate of the HyProCure procedure and find it can be a life-changing procedure for some of the patients who have it done. But, "nearly pain-free" - come on guys, let's keep it real. All patients respond differently to any surgery, some have pain, some don't. There are varying degrees. The procedure does not differ and is not magical in any way that it is "nearly pain-free."
