Ankle Replacement Has Significant Advantages Over Fusion: PA Podiatrist
 
Daniel Plesniarski’s ankle problems started when he was a kid, with frequent sprains. By the time he reached retirement age, it had become a real problem. But he put off getting treatment, in part because a doctor at another hospital system recommended he get the joint fused. “I didn’t want an ankle fusion,” Plesniarski says. “I didn’t want to lose the ability to move my ankle.” Today, the pain and swelling are gone and the 72-year-old Andreas man says his ankle feels good as new, thanks to foot and ankle surgeon Elliot Busch, DPM, with Lehigh Valley Orthopedic Institute, along with the team at Lehigh Valley Hospital–Carbon.

“For patients who have ankle arthritis, ankle replacement is a fantastic treatment option that can give people their quality of life back,” says Dr. Busch. Replacing the ankle joint rather than fusing it has several advantages, he says. Fusions remove the pain by removing the joint and letting the bones grow together. But this can affect the way someone walks, Dr. Busch says, causing stress on other joints and possibly leading to “transfer arthritis” and eventual fusions of those joints as well.

NJ Historical Society Museum Holds Program About Dr. William Scholl
 
The LaPorte County Historical Society Museum (located in Northwest IN) hosted a program about Dr. William M. Scholl, who was born and raised in LaPorte. Kelly Reiss, the Archives & Feet First Museum director from Rosalind Franklin University, presented the program. The program detailed Dr. Scholl's unique drive as an inventor, entrepreneur, corporation owner, teacher, author, and philanthropist, all to advance medical education in foot health and awareness.

He knew early on that he had the talent to go pro; but the career path he kept in the back of his mind, in case things didn’t work out, was podiatry, based on the success modeled by a woman in his community.

“You know, a lot of people always laugh at me when I get them that answer. And it’s based on the people that impacted me at an early age,” he explains, recalling the podiatrist in his childhood home of Bessemer, AL. “She worked on my dad’s feet, mom’s feet, my uncle’s feet. She was taking care of two people that I loved very dearly, my dad and his mother, my grandmother. And I wanted to be a podiatrist, brother. Like, I really wanted to be a foot doctor.”

Query:  Coding Procedures with Time-Based E&M
 
I normally code an E&M visit with medical decision-making. I have a new job that codes off of time-based visits. There are many denials for the E&M when a procedure is performed. I am trying to appeal these and am stuck when the denial is upheld, stating it’s inclusive with the procedure. The documentation clearly states they were seen for unrelated services and based on time before, during, and after. Time does not include procedure. Why is it still denied and how do I appeal this?

For example, when billing time, one must state at least several of those listed below and explain each one that contributed to “Total Time”:

ii. Obtaining and/or reviewing separately obtained history
iii. Performing a medically appropriate examination and/or...
