GA Podiatrist Discusses Marathon Injuries
Former AAPSM President, Jay Spector, DPM discussed marathon injuries with Ben Pearl, DPM on his podcast, Fit Foot U. Dr. Spector, a marathon runner and coach, talked about the injuries he has overcome. He recently discovered he had a limb-length discrepancy which he addressed with a full-length lift built into his orthotic.

Dr. Spector described what he calls phantom injury after marathons. He said it is not unusual to have real pain that can migrate to different parts of the lower extremity that is a result of temporary muscular fatigue or transient post-race inflammation.

Ozempic Sparks Controversy on Surgery Prep
With the increase of prescriptions for glucagon-like peptide-1 receptor agonists like Ozempic, Mounjaro, and Wegovy, physicians began seeing a startling number of regurgitation and aspiration in surgeries. GLP-1s delay gastric emptying, meaning normal fasting rules weren't cutting it. In response to anecdotal reports and case studies, the American Society of Anesthesiologists published a guideline in June recommending patients stop taking GLP-1s within the half life period — such as skipping one week's semaglutide injection — before undergoing anesthesia.
There's one area resulting in a "clinical dilemma," according to the AGA. Christopher C. Thompson, MD, who helped draft the gastroenterological group's Nov. 7 statement, said after the ASA's June guidance, "many of our cases were being canceled, and this was problematic for patients."

Query: Sufficiently Detailed Exam
Can you describe what is a “sufficiently detailed exam” to confirm the diagnosis of peripheral arterial disease? I was audited and the payment for debridements of calluses were denied, even though documentation of every single class finding was listed. Are we supposed to do ankle brachial indexes, Buerger’s test, etc? Why have class findings if they do not qualify as “sufficiently detailed?”

Response: I have a problem with the phrase “minimum documentation.” I believe the correct phrase should be “appropriate documentation.” With respect to the above post, the issue is a “sufficiently detailed exam.”
So, a patient qualifies for “at risk” routine foot care and is returning regularly (i.e. every 61 days) for follow-up “at risk” routine foot care encounters. Medicare Administrative Contractors do not find it appropriate to run one date of service into the next, in effect “cloning” the information from one “at risk” routine foot care encounter to the next. Medicare Administrative Contractors expect all encounters to stand upon their own documentation as if this was the only time that the patient was...

You are the best one to know the potential valuation of your practice. You talk to your accountant and review at least the last 10 years of gross and net income, also any properties and equipment at today’s market value. If you have multiple offices don't leave out those secondary offices and their equipment. Do you have privileges at local hospitals, and are they open to a new doctor joining the staff? The gross income may be less now but also you may be working fewer hours due to your success and/or your age now. Younger DPMs, just starting out, usually work more hours than older established DPMs.
Ask your accountant and other docs at the hospital who they recommend. YOU and your accountant are the most knowledgeable to set the price. Don't pay a fee you may not need to! Most likely, the purchaser and/or company of DPMs (there are large groups of doctors and/or hospital conglomerates) will also have their experts do an evaluation of it. Good luck in today’s medical market.

In her interview with Very Well Health, Dr. Lobkova said that onychomycosis is an undertreated condition. She further points out various treatments that often work to control or cure the condition. Many are not covered by insurance. The issue with mycotic nails is interesting. When we as doctors see these thick, yellow, and often odorous toenails, we assume that we are treating a medical condition. The dichotomy is that in most cases the patients present to our offices because they don't like the cosmetic appearance of the nails. Very often their insurance companies will agree that the condition is purely cosmetic.
What we need to do is to first take a culture of the nails and send the specimen to a good dermatopathology lab that will spend the time to identify fungal elements when present. Then we need to prescribe the proper medications that can be used to address the medical problem and to this end point, there are many therapeutic options and only the patient and his or her doctor can develop a good plan for treatment. The problem with thick, yellow nails has been around for a long time. Michelangelo, carved in thick yellow toenails in his sculpture of Moses. Michelangelo used elderly Jewish men as models for his sculptures and apparently many in Rome had mycotic toenails.

There is no doubt that the elders in our profession deserve credit for pioneering ideas that are being used today. Oftentimes, we forget those that laid the initial ground work. That said, I don’t believe Dr. Siddiqui was trying to be disrespectful nor did I feel he was misleading anyone. I read his statement as giving an endorsement to the procedure Dr. Cowin has been advocating.
Truthfully, I was also under the impression that the decades-old MIS bunion technique used a "higher speed burr". It’s good for our profession and the community when decades-old thoughts and principals are revisited and refined by subsequent generations of surgeons.
