LA Podiatrist Provides Holiday Shopping Foot Health Tips
 
According to Susan Girardo, DPM of the Ochsner Health System, "Holiday shopping is like a marathon, and it is essential to wear the appropriate shoes. The ideal shoes are comfortable and well-fitted, providing support similar to that found in walking or running shoes. Shoe inserts can be used for added support if needed. Avoid wearing heels, flip-flops, or shoes that are too tight or constraining. Heels may look great, but they tire out the feet quickly and could increase the risk of trips and falls. Flip-flops do not provide sufficient foot and ankle support.

"If you have diabetes, you should take special care during lengthy shopping trips. Remember to pay attention to your feet throughout the day to ensure that blisters or cuts do not develop, as these could lead to lasting problems. Examine your feet after a day of shopping to look for signs of any problems and consult your podiatrist if you discover a problem.

Podiatrists can provide help in a way that general practitioners often cannot. “Podiatry is a cool field. It encompasses a little of everything: diabetes, skin lesions, ingrown toenails, bunions,” Sarah Haller, DPM said. “We do a lot of sports medicine; we cover all of the bases. I love it.”

The most common sports maladies affecting the feet and lower legs, Dr. Haller said, are “Ankle sprains. They are No. 1 all day, every day, among athletes. That’s actually a statistic. By far. Stress fractures are up there. And Achilles ruptures, especially in the spring. Achilles tendons need to be stretched. Type 1 collagen accounts for most of the dry weight of an Achilles tendon. It is supposed to bend and contract, and when we don’t use it every day, it’s like running in sand.”

The flexor tenotomy is definitely a wonderful and minimally invasive procedure for distal calvi and ulcers in diabetic and otherwise circulatory compromised patients. However, with a semi-rigid or rigid hammertoes, I find it to be less successful. My go-to procedure in these cases is an MIS proximal or middle phalangeal (and occasionally both, depending on the degree of deformity) osteotomy performed with a plantar incision using a 67 blade and an Isham 44 rotary osteotome.
 
I wrap a thin layer of Betadine-impregnated gauze around the toe and bandage it in a slightly dorsal position. The gauze hardens and becomes a mini-cast holding the toe in proper straightened position, and I leave it bandaged for 2 weeks. Results are amazing.

RE: The Power of Your Patients and the Media

The power of the media and your patients working together can reverse payment denials. A suburban Chicago area resident recently went on TikTok in order to obtain a reversal of a $40K payment denial for kidney tumor surgery. This, even though the hospital and physician had obtained pre-authorization from Anthem. This recent story shows the power that you and your patients along with the media can leverage over these giant monopolies.
 
One cannot expect to generate much empathy over small denials typically seen in many physicians’ offices. However, lumped together, a union or group of patients filing a similar action against an insurance giant may be able to leverage equivalent results.

With all my patients I always include a question if they have any back problems. For those patients, I almost always recommend orthotics. Most all who get them are very satisfied and I would say 25% of the patients coming in with back pain order the orthotics. I think all podiatrists should do this. You would be surprised how many patients you can help.

I believe one must be very careful when billing for this product. Their own billing guide online states: "No existing CPT code is specific to the QUTENZA application." However, their website lists some of the following CPT codes:
 
64620 Destruction by neurolytic agent, intercostal nerve 
64632 Destruction by neurolytic agent, plantar common digital nerve 
64999 Unlisted procedure, nervous system 
64640 Destruction by neurolytic agent procedures on the somatic nerve
 
I don't believe billing for one of the above CPT codes after performing a topical application of Capsaicin would be appropriate (except possibly 64999). And to do this three times? So, the first two times weren't "destructive"? I found a billing and coding article from CGS Region J15 Part B regarding Qutenza: My advice is "provider beware".

Focus on the current state of economics of medicine. Hospitals and corporations are taking over private practices. Solo practitioners in all facets of medicine are becoming rare. Now let’s follow the money. The majority of what we do are not total ankle implants and other major surgeries. The bread and butter of what we do is podiatric medicine. It's warts, neuromas, heel pain, onychomycosis, etc. 
 
Nurses could easily be trained to render this type of foot care. By training and certifying nurses to perform foot care, the money will be flowing from the pockets of podiatrists to corporations and hospitals via these certified nurses. Make no mistake. Don't think that these nurses will be working for DPMs. They will be working "down the corner" at medical centers, replacing the good work of podiatrists.
