Vigilant Foot Care Can Reduce Diabetic Complications: MI Podiatrist
 
The diabetes epidemic has reached more than 30 million Americans to date, and complications in the feet send more people to the hospital than any other complication. Reedsburg Area Medical Center podiatrist Elizabeth Baker, DPM says, “Regular and vigilant foot care can help catch problems before they turn into a health crisis.”

“These problems can turn into ulcers, essentially open wounds on the feet, and can develop serious infections. Unfortunately, diabetic ulcers often lead to amputation. No single case of diabetes is the same as another,” Dr. Baker said. “Our care must be with the patient’s specific needs in mind.

PODIATRISTS IN THE COMMUNITY
AR Podiatrist Featured in "Talk With a Doc" Seminar
 
Dr. Spencer Mortensen, a doctor of podiatric medicine at Sparks Foot and Ankle Clinic, recently introduced himself to the community during a "Talk With a Doc" seminar held at Sparks Regional Medical Center. During his presentation, "Focus on Your Feet," Mortensen addressed topics including how to avoid diabetic foot complications as well as general recommendations for good foot health.

Mortensen also had several recommendations for prevention of problems. Lifestyle changes including diet and exercise, keeping blood sugar within reason and smoking cessation were all suggestions for prevention. In addition, Mortensen gave some general recommendations: visiting a podiatrist regularly, daily foot inspections - recognizing even the smallest abnormality, wearing clean, thick, white socks, investing in good shoes, cutting toenails straight across the top, and not performing at-home surgeries such as removing ingrown toenails or corns or calluses.

Governor Signs Legislation Updating Scope of Practice for MI Podiatrists
 
Governor Rick Snyder recently signed Senate Bill 901 into law. It’s now Public Act 355 of 2018 and will update the scope of practice for Michigan podiatrists. The legislation moved through the entire legislative process without any votes in opposition from legislators.
 
The Michigan Podiatric Medical Association (MPMA) has never sought to expand their scope-of-practice but rather correctly define how they practice on a day-to-day basis with procedures like venus leg ulcers and ankle fractures. The previous scope of practice put podiatrists in a very gray area when performing these procedures.

Reasons to Say No to Payer Contracts - Part 4

4. Excessive denials/higher accounts receivable 
 
Some payers deny more claims than others. Ask your staff to show you the first pass denial rate for your payers—that is, what percent of initial claims submitted to each payer are denied. A payer who denies more claims is cutting into your bottom line since a denied claim requires additional work by your billing department.  
 
While looking at denial rates, take the extra step to look at why claims are being denied. You may find patterns—incomplete/incorrect registration, for example— that can be improved internally.

RE: The Passing of Robert C. Alberhasky, MD
 
Bako Diagnostics and the podiatric community mourn the loss of pathologist, Robert C. Alberhasky, MD, who died on Saturday, November 17th in a single-engine plane accident at Lee Gilmer Memorial Airport in Gainesville, GA. Details of a memorial service will be forthcoming on the BakoDx website.

Dr. Alberhasky had a long history in the podiatric medical community and was part of the Bako family from nearly our inception. "Dr. Bob", as he was known around the lab, was always available to help, be it for a colleague or a friend and will be sorely missed. Our deepest condolences to the Alberhasky family and to his many friends.”

RE: Has Evidence-Based Medicine Gone Too Far?

I was at a cancer conference recently and there was a discussion on how we moved too far to the "evidence-based medicine" camp and lost all common sense. A doctor at the meeting (Brian Alexander. MD) presented this funny abstract: Parachute use to prevent death and major trauma related to gravitational challenge: systematic review of randomized controlled trials
 
Objectives: To determine whether parachutes are effective in preventing major trauma related to gravitational challenge.
Design: Systematic review of randomized controlled trials.
Data sources: Medline, Web of Science, Embase, and the Cochrane Library databases; appropriate Internet sites and citation lists.
Study selection: Studies showing the effects of using a parachute during free fall.
Main outcome measure: Death or major trauma, defined as an injury severity score > 15.
Results: We were unable to identify any randomized controlled trials of parachute intervention.
Conclusions: As with many interventions intended to prevent ill health, the effectiveness of parachutes has not been subjected to rigorous evaluation by using randomized controlled trials. Advocates of evidence-based medicine have criticized the adoption of interventions evaluated by using only observational data. We think that everyone might benefit if the most radical protagonists of evidence-based medicine organized and participated in a double-blind, randomized, placebo-controlled, cross-over trial of the parachute.

In response to Dr. Long's query regarding injection coding for Morton's neuroma, the current CPT code is 64455. This was updated from the CPT 64450 in the CPT manuals and NCCI several years ago. When billed with the "G" ICD-10 codes (G57.61 or G57.62), the 64455 is valid and billable in conjunction with the proper "J" code(s). As Dr. Borreggine stated, the 20550 and 20551 are the proper injection codes for plantar fasciitis.

Mechanically-induced back pain can be reduced/improved or alleviated with better postural mechanics and improved lower extremity function in standing activities and during gait with the help of custom prescription foot orthotics. This usually requires a combination of treatment modalities including stretching, core muscle strengthening, activity modification, and foot orthotics.
 
An excellent test to see if custom orthotics might be successful is to first try a properly applied low-Dye tape strapping(s) and when indicated also concurrently the use of a unilateral measured heel lift inside the shoe for limb-length discrepancy. Podiatrists who dispense orthotics should be checking for LLD on exams. If the temporary taping and a heel lift helps, I have found that this is a good predictor that custom prescription orthotics will help as well. This two-step approach is quite understandable to our patients who are looking for relief of their back pain. These patients will often try anything but unfortunately they become disappointed when poor outcomes and results don't meet the hype and their expectations.

Dr. Simon seems to suggest that I don't do a thorough work-up. He is also of the opinion all chiropractors don't know what they are doing. On both counts, how unprofessional. Do all DPMs immediately refer out their patients with heel and back pain. I highly doubt it, unless maybe if they are employed by hospitals. Are there chiropractors I do not trust, sure, but the ones I know, do not exceed their training, do not do orthotics, and don't sell products except for the occasional back brace and Biofreeze. Chiropractors have been a steady referral source. Here's what I believe: If you want respect, give respect.
