DC Podiatrist Discusses Shin Splints

The most common site for shin splints is the medial area (the inside of the shin), according to Sheldon Laps, DPM, a podiatrist in the Washington, DC, area. Anterior shin splints (toward the outside of the leg) usually result from an imbalance between the calf muscles and the muscles in the front of your leg, and often afflict beginners who either have not yet adjusted to the stresses of running or are not stretching enough.

"Shin splints will be most painful if you forcibly try to lift your foot up at the ankle," says Laps, "If you flex your foot and it hurts, it's probably shin splints." If you are prone to developing shin splints, stretch your calves and Achilles regularly as a preventive measure.

BUSPM Students Awarded Dr. Herbert Feinberg Memorial Scholarships

Barry University’s School of Podiatric Medicine has awarded two third-year students with the Dr. Herbert Feinberg Memorial Scholarship. For their academic merit, Noelis Rosario and Hani Badahdah will each receive a $1,000 scholarship credited to their student account to defray the cost of tuition. Both students achieved the highest grade point average in their class of 2013. Rosario hails from San Juan, Puerto Rico. She serves as the Vice President of the Class of 2013. She is active in several podiatric student organizations and community service activities in the greater Miami area.

Badahdah is from Jeddah, Saudi Arabia. He serves as the President of the Class of 2013. As a class leader, he recently organized the annual residency fair at Barry, where 41 residency programs from across the nation came to represent their programs and provide the students with crucial information which helped them to select their residency upon their graduation.

PODIATRISTS IN THE COMMUNITY

Atlanta Homeless Receive Free Screenings and Donations from Village Podiatry Centers

Foot problems often plague Atlanta’s homeless population due to lack of proper care, exposure to the elements and excessive walking. In an effort to address these foot-related needs, the podiatrists and staff of Village Podiatry Centers recently donated their support to the Atlanta Day Shelter for Women and Children (ADSWC) which serves more than 5,500 clients annually. Village Podiatry’s second annual “Put a Sock in Homelessness” campaign began in October. The campaign is a month-long drive for donations in the practice’s 26 Georgia offices.  The 2011 drive resulted in 205 new pairs of shoes, 461 pairs of socks, and $1,255 in donations given by patients and staff.

On Saturday, November 5, Village Podiatry organized a free foot screening at the day shelter where podiatrists, Drs. Andrea Cass, Allen Lazerson, and La’Genia Mitchell-Smith, evaluated the feet of 60 women and their children. Shelter clients were screened for current foot problems and educated on proper foot hygiene. More than 25 Village Podiatry staff distributed new socks and assisted the clients with shoe selections.

MI Podiatrist Joins St. Mary Mercy Hospital

St. Mary Mercy Hospital has added a new medical staff member. Dr. Jennifer A. Gerteisen. She received her degree at Des Moines University College of Podiatric Medicine and Surgery in Des Moines, Iowa.

Gerteisen completed her residency at St. John Hospital and Medical Center in Detroit, Michigan, and is a member of the American Podiatric Medical Association, Michigan Podiatric Medical Association, and an Associate of the American College of Foot and Ankle Surgeons.

8th Annual Desert Foot Wound Care Conference Hosts Over 900 Attendees

Over 900 Attendees and Exhibitors converged on Phoenix, Arizona this week for the 8th Annual 2011 Desert Foot High Risk Diabetic Foot Conference. Founded for the many clinicians from the VA System dedicated to saving the limbs of our nation's veterans, Desert Foot has grown into one of the largest conferences for the wound care industry. Original research was presented in the 60 strong abstract/poster exhibit, and separate sessions were conducted for nursing and orthopedics.

This emotional presentation honored the magnificent contribution made by Dr. Brand to our understanding and treatment approach to diabetic neuropathy.

RE: Sesamoid Pain Right Foot (Bryan Markinson, DPM)

For the most part, my opinion is that your assessment is spot on. One must take into account the repetitive trauma endured from marathon running, and more importantly, its training. The cystic changes along the lateral side of the fibular sesamoid, along with what appears to be fusion to the 1st metatarsal head, are indicative of an old healed fracture. The fact that it appears fused would likely eliminate AVN.

A sesamoid axial view would be helpful in evaluating the fibular sesamoid/1st metatarsal articulation to see if it is completely fused, partially fused, or totally open. If open, then AVN becomes a possibility. Micro-motion along the portion of a partially open articulation, combined with fusion along another, would create pain during propulsion when the sesamoid attempts to glide against the underside of the metatarsal.

I see nothing in this lone submitted x-ray to suggest an...

RE: Wavelengths for Laser Treatment of Onychomycosis (Elliot Udell, DPM)

Wavelength is just one factor when treating fungus toe nails. The fluence is another factor. This is joules per cm2 over time. The wavelength that has been used in both long pulse and q-switch is the 1064. Another wavelength is 980, which attracts water and hemogoblin. When talking about diode lasers, the 870/930 wavelength is used with the Neovon laser. So, the wavelength depends on the type of laser, i.e., diode vs Yag.

Dr. Udell points out that there needs to be long-term studies and that you don't want to have unhappy patients who spend 1,100 dollars with the nails looking a year later just as bad as they did before treatment. I agree 100% with Dr. Udell's statement. The problem is that...

RE: STJ Arthroeresis Coding

CPT, through its official coding newsletter, CPT Assistant (September 2011 issue, pages 11-12) addressed coding the insertion of a subtalar implant (subtalar arthroereisis procedure). The publication stated, “There currently is no CPT code that specifically and accurately describes a subtalar arthroereisis procedure.” The article further clarified that a subtalar arthroereisis procedure (insertion of a subtalar implant) should NOT be coded as subtalar arthrodesis (neither CPT 28725 nor CPT 28725-52), and it should NOT be coded as a subtalar dislocation (CPT 28585).
 
The article stated that the most appropriate code to report a subtalar arthroereisis procedure is the unlisted foot/toes code, CPT 28899 or, alternatively, the temporary national HCPCS code, S2117 (arthroereisis, subtalar).

RE: Early Diagnosis of Bunions Can Prevent Foot Surgery (Robert Bijak, DPM)

As a CCPM graduate from 1975, I take exception to Dr. Bijak’s comments. Drs. Sgarlato, Root, Weed, Orien, and others who taught us had an excellent understanding of the biomechanics of the foot. Yes, as Dr. Bijak points out, different clinicians could measure slightly different degrees of forefoot varus, etc. Nonetheless, through their teachings and application of podiatric biomechanics, we have differentiated our profession from others.

When MDs were prescribing rest, ice, elevation, NSAIDs, and physical therapy, and runners were subsequently still encountering overuse syndromes, it was the podiatrists who were curing the runners’ ills. By addressing the causes of these overuse syndromes, through their understanding of biomechanics and the use of orthotics that were properly prescribed and fabricated from a cast of the foot, the podiatry profession made significant advances.
