Onychomycosis is an Under-Treated Condition: NY Podiatrist
Onychomycosis is an under-treated condition. “Treatment is expensive and time-consuming,” Nelya Lobkova, DPM, a board-certified podiatrist and founder of Step Up Footcare, told Verywell. “Some options, such as laser therapy, are not covered by health insurance at all.” Dr. Lobkova said that effective topical medications, such as Jublia (efinaconazole), are expensive and not covered by insurance.

Onychomycosis is not a condition that goes away on its own. Lobkova said if it’s left untreated, the infection can spread to every nail. Over time, the nails become thick, brittle, and ingrown, causing pain. “It is advised to consult with a nail specialist, such as a podiatrist or dermatologist, to assess for patterns in the nail structure in order to properly diagnose and treat the root cause of the damaged nail.”

CA Podiatrist Says Company Used AI Clone of Her Voice for Ad
A doctor on TikTok claims that her voice was used without her permission to sell an ear-cleaning device. "Doctor Dana" Brems (@footdocdana), a podiatrist who has more than 2.2 million followers, typically posts a combination of lifestyle and medical content. But on Thursday, she posted a different video. An older TikTok of hers was allegedly manipulated to make it sound like she’s recommending a product from a company called HearClear.

In the TikTok video, she was originally advising followers not to use even Q-Tips to clean one's ears, because it could “damage the tympanic membrane.” Dana was addressing a video where a man appears to be sticking a small black device in another man’s ear. Then the video cuts to an image of the HearClear Pro, while Dana’s voice continues, “Instead, you should be using something like the HearClear, as it is far safer and more effective.” In a stitch, Dana watches the video in horror. She says in the caption that the company “used an AI clone of my voice to pretend I recommend their product.” Many commenters advised her to take legal action.

NY Podiatrist Discusses What to Look For in a Men's Dress Shoe
Men's Health style editors know a lot about shoes but we’re not doctors, so we called in an expert for advice. “My main thing to look for in dress shoes is proper arch support,” says New York City-based podiatrist Brad Schaeffer, DPM. “Dress shoes can have little to no arch support and are very hard and rigid, depending on which brand you buy.”

So choosing a lighter, more comfortable dress shoe sole is half the battle. That will provide the give and support that your feet need to minimize discomfort. If you prefer a traditional heel, then stick to rubber outsoles over leather and keep heel height to a minimum. Dr. Schaeffer pointed out that you don’t need to drop a lot of money to get comfortable dress shoes. “I have seen very expensive shoes with no support and vice versa,” he explained. “It can be very hit or miss.”

I have been getting information from my wound care clinic regarding restrictions on the application of skin substitutions grafts. I am told that I won’t be able to apply more than four grafts for any given patient. Is this true?

Response: Good news, for now; that is not correct. CMS and a few of its MACs had planned to institute a significant reduction in available cellular and tissue-based products (CTPs) and limit to 4 applications per year. Fortunately, they pulled back that change just before its scheduled start date of Oct 1. The most recent Federal Register made no mention of such change either, which could bode well for the delay in this policy. For now, it is business as usual in the use of your CTPs.

In the article that was posted in the November 17, 2023 edition of PM News, Norman A. Siddiqui, DPM, MHA is quoted as saying, “In the past, there were burrs that you could use for MIS; however, the challenge was they were high-speed, high-torque, not as accurate and potentially harmful to the soft tissue and bone if used without strict control.” Of course, the burr itself does not control the speed, the power equipment that it utilizes to rotate the burr has that responsibility.
