ASPS Board of Directors Hosts Annual Board Meeting
The American Society of Podiatric Surgeons (ASPS) Board of Directors held its Annual Board Meeting November 13-14 at APMA Headquarters. The meeting focused on ways the organization can enhance conference programming for the 2016 Annual ASPS Surgical Conference, membership recruitment, and ASPS’ social media strategy. ASPS is excited to announce the launch of ASPS social media in the coming weeks, and is looking forward to engaging student members to assist with social media content.

UT Podiatrist Provides Winter Foot Care Tips
We take a lot of extra precautions to stay healthy during the winter. One thing we don't talk about very often is our feet. Dr. Jason Bruse, a podiatrist with the Ogden Clinic says one of the problems he sees in winter is that people wear shoes or boots which are too small. "In the winter time, we tell people to wear shoes which have plenty of room. In the cold weather a lot of people will wear extra socks. That can be problematic sometimes. You are actually better wearing thinner socks," says Bruse.

I have seen patients with mild frostbite from the toes being too crowded in the shoe. One rule is to make sure that your toes can wiggle around in the shoe. That allows for circulation to the toes. The same is true for ski boots. People will tighten them too much and then complain that their feet are cold," says Bruse.

Modern Surfaces Make Minimalist Shoes Dangerous for Most Runners: PA Podiatrist
Podiatrist Brian Crispell, DPM says, "Impact reduction is essential for the modern day runner. The surfaces that most recreational runners choose to run on are now artificial. Pavement and concrete pathways create a very consistent surface on which a runner can crank out many miles in little time. However, these man-made solid stone pathways do not absorb any of the impact imparted on them by the foot. All of the impact is absorbed by the human body. Additionally, the consistent nature of the surface multiplies repetitive stresses on the musculoskeletal system. For these reasons, minimalist running on a pavement or concrete surface is very dangerous."

While the minimalist footwear and forefoot striking may be beneficial for a select few (for example people with anterior compartment syndrome), the risks usually outweigh the potential benefits. In the vast majority of the population, conventional running shoes and heel striking are more practical.

Diabetic Foot UIcers Largely Preventable: IA Podiatrist
For people with diabetes, taking care of their feet is especially vital. A 2012 study by the American Podiatric Medical Association (APMA) indicates that Hispanics with diabetes are particularly in danger, because more than 90 percent of those with the disease or at risk for it have never seen a podiatrist as part of their healthcare.

“The leading cause of hospitalization among people with diabetes—regardless of ethnicity—is foot ulcers and infections, but most of those problems are largely preventable,” says Mindi Feilmeier, DPM, president of the Iowa Podiatric Medical Society and an APMA member. “It’s important for those with the disease to ‘knock their socks off’ and receive regular foot exams by today’s podiatrists.”

FL Podiatrist Withdraws License
A St. Augustine podiatrist accused of huffing dust cleaner has withdrawn his medical license, according to the Florida Department of Health website. Dr. James Militello, 61, of St. Augustine, was arrested Oct. 29 after police said he crashed into a power pole and left the scene, according to a release from the St. Augustine Police Department.

If you have just cause and you've followed disciplinary procedure, don't be afraid to cut an underperforming employee loose, says Bo Snyder, a practice management consultant in Kalamazoo, MI, noting that many managers hang on too long for fear of litigation. "You need to have your ducks in a row but that's not hard," he says. "You can fire people for lots of great reasons. Often, it just takes discipline and carving out time to go through the progressive process."

I'm part of a large group of about 50 podiatrists. Of our group, the only ones who passed PQRS were the very few who chose to report via a paid registry. All those who reported via claims were told we failed. Something is fishy about that. I think the APMA should survey the membership to see who passed, and if they passed via claims or a paid registry. It's highly unlikely that 45 intelligent humans all failed to include the proper codes. I asked for the informal appeal and was rejected.

One further thought on this topic. There is a famous study where monkeys were trained to throw darts. They threw darts against a wall covered by the listings of all stocks. Stocks were then selected by the darts. Professional advisors then picked stocks based on their criteria. Guess which group was more successful? No one cares about your wealth as you do. Investing in the American economy and partnering with great corporations have been a great arrangement throughout history.

I agree with John Lychak's effort to dispute facility fees. When I was seeing a neurologist in his private office, the typical fee was under $300. He moved his office into a hospital and the only facilities from the hospital I utilized were the elevator, rest room, and hallway. My typical bill from his hospital office had an additional $400 facility fee. No services other than consultation and examination occurred. If the hospital isn't charging the doctor rent, the financial arrangement seems spurious.
Why is this important? Physicians are the first to suffer reimbursement cuts and are held responsible for increased healthcare costs when it's hospitals and labs that are the culprits. When we do hospital surgery, our fee usually is about 1/8 of the total physician, lab, and hospital bill. For elective surgery, that seems like an awful waste of resources.
