IL Podiatrist's Memorial Soccer Tournament Continues to Grow
 
One of the region's premier soccer tournaments continues to grow, which makes it a success for all of Saline County, its founder says. Dr. Roger Herrin, a retired Harrisburg podiatrist, founded the soccer tournament in memory of his son Michael Herrin, who died in a car accident in 2001.

This year, 41 area teams participated in the tournament, Herrin said. He said the economic impact of such an event helps bolster Saline County. "As long as we can do that, it will help the schools, the community, and the businesses here," he said. "I hope to continue to do that, with even more support next year."

New Shoe Insole Technology Could Help DFUs Heal Better 
 
Purdue University researchers have developed a shoe insole that could help make the healing process more portable for the 15 percent of Americans who develop ulcers as a result of diabetes. "One of the ways to heal these wounds is by giving them oxygen," said Babak Ziaie, Purdue professor of electrical and computer engineering. "We've created a system that gradually releases oxygen throughout the day so that a patient can have more mobility."

"We typically treat ulcers by removing devitalized tissue from the surface of the wound, and by helping the patient to find ways to take the weight off the affected foot," said Desmond Bell, DPM, a podiatrist in wound management and amputation prevention at the Memorial Hospital in Jacksonville, FL, and the founder of the Save a Leg, Save a Life Foundation. "The gold standard for treating an ulcer is a patient wearing a total-contact cast, which provides a protective environment for the foot. If we could test how well this insole delivers oxygen to the wound site from within the cast, then this could be a way of aiding the healing process," he said.

VT Podiatrist Lectures and Holds Workshops at Schuster Biomechanics Seminar

Howard Dananberg, DPM, one of the world’s foremost authorities on the relationship of foot dysfunction to chronic postural pain syndromes, again lectured at the 6th Annual Richard O. Schuster Memorial Biomechanics Seminar at the New York College of Podiatric Medicine. Dr. Dananberg’s presentation “In-Shoe Pressure Analysis and its Application to Understanding Human Gait” was followed by an F Scan workshop entitled “Orthotic Modifications and How They Affect Gait.”

Dananberg solicited orthotic-wearing subjects from among the attendees to objectively assess and analyze current function, address abnormalities via orthotic modification, and then re-assess function.He noted that minor adjustments can have a major impact not only on the foot, but also on the overall health and function of the locomotor system. Dananberg did add one caveat, stating that before you can address and correct these deficiencies, you first must be able to accurately identify them, which can be achieved through in-shoe pressure testing.

Reasons to Say No to Payer Contracts - Part 2
 
A payer contract is more than a mere fee schedule. There are several other factors to consider when deciding to keep or terminate a payer contract.
 
2. Credentialing issues 
 
There are opposing incentives when it comes to credentialing new providers. The sooner a practice can get a provider credentialed, the sooner it can bill and get paid. Conversely, the longer it takes a payer to credential a provider, the more money they save since the practice may not bill for services delivered by un-credentialed providers. If your state does not have credentialing regulations that include a timeline, your practice could lose thousands of dollars due to credentialing delays. My recommendation is to make it an issue with your payers. Share your expectations in advance of bringing in a new provider and let your provider representative know their performance will be part of your next re-negotiations.

RE: The Danger of Downcoding

A colleague of ours, an immigrant from Russia, was used to working in an onerous medical system and strived to do the best to avoid any problems with Medicare. This doctor downcoded all  I&Ds in order to avoid having to justify the higher code. As a result, the podiatrist’s number of 10060s was much higher than average. CPT 10061 was never billed, thereby saving the government money. The doctor was charged and found guilty for improper billing. Now, I am not sure there were other factors involved; nonetheless, the downcoding was held to be a fraudulent act.  
 
I have never seen an actual description of the difference between 10060 and 10061, except that one is considered greater in nature than the other. Lesson to be learned - 1. the government is not here to help you and 2. Don’t downcode.

While I offer no scientific proof, I can give you what I have observed for most of my practice. I have shared offices with chiropractors for most of my career. They were all hands-on DCs who stretch and adjust the spine. As part of their exam, they look at the patient standing barefoot. We all understand, or at least we should, the additional stresses that hyperpronation places on feet, ankles, shins, knees, hips, and low back. Far more often than not, appropriate orthotic treatment lessens or eliminates much low back pain.

Howard, prior to about a week ago, I had never heard of you. I sincerely apologize to you for not adequately researching your work and accomplishments during your career. It was only after reading your CV online and seeing the extent of your publications, did I understand the extent of your contributions to biomechanics and podiatry over many years. What I did initially find were links on Google that showed the sale of your orthotic products on Amazon, and your involvement in Vasyli. 
 
I was looking to find a direct, reproducible, scientifically-based connection between the use of orthotic devices and the treatment of back pain. In my almost four decades of practice, I have  seen a frightening number of patients who had claimed that their orthotics had been helpful for the treatment of everything from HIV to ingrown toenails. Although I may not necessarily agree with some of your publications and conclusions, you are nonetheless a true scientist and worthy of great praise.
