PODIATRISTS AND DIABETES
FL Podiatrist and Vascular Surgeon Saves Diabetic's Foot

Rosemary Vega has been a diabetic for 41 “mostly uneventful” years. But in July, she went on a beachside vacation and noticed her foot was bleeding and her toe was infected. When she returned home, she went to her podiatrist, who tried to treat the infection with antibiotics. The infection persisted and reached the bone, so Vega, 61, went to another podiatrist, who sent her to South Miami Hospital. Dr. Christopher Blanco amputated her toe Aug. 24.

Vega suffers from diabetic neuropathy, a type of nerve damage that most often affects feet and legs. It is caused by high blood sugar that damages nerve fibers throughout the body, according to vascular surgeon Dr. Edgar Galinanes. Galinanes said treatment for the condition has shifted to a multidisciplinary approach only in recent years. Education is key to preventing amputations. All diabetics should have an annual podiatry exam, Galinanes said. And they should make a point to examine their feet on a daily basis and wear more comfortable shoes.

MEETING NEWS
NYCPM’s Schuster Biomechanics Seminar Gathered Experts from Across the U.S.

Sponsored by an education grant from Jerry Seinfeld, the third annual Richard O. Schuster, DPM Memorial Biomechanics Seminar was held at NYCPM on November 14, 2015, and featured a once-in-a-lifetime assembly of pre-eminent speakers from all across the U.S. Dr. Joseph D’Amico professor and past chair of the Department of Orthopedics served with distinction as the Scientific Chair of the Seminar.

(L-R) Drs. Michael Trepal, Guido LaPorta, and Joseph D'Amico

Drs. Guido LaPorta,  Kevin Kirby, John McNerney, James Losito, J. David Skliar; and Jinsup Song, as well as acclaimed experts from NYCPM including Dr. D’Amico, Russell L. Volpe, and Jeffrey Cusack, made presentations. Lectures were geared to reaffirm and re-establish biomechanics as the cornerstone for the practice of podiatric medicine and surgery. Topics focused on lower extremity bio- and pathomechanics from a static as well as a dynamic perspective and its accompanying influence and significance in the pediatric, adult, and sports-participant patient. The importance of phylogenic and ontogenic influences in the foot and ankle and their resultant equinovarus deficiencies were a focus of the presentations.

IN THE COURTS
Lawsuit Challenged Coordinated Health's Facility Fees for Foot Care

John Lychak of Lower Saucon Township knew he would receive bills when he sought treatment last year for a foot ailment. What he didn't expect was to be billed nearly $1,000 for hospital "facility fees" for care he didn't consider to have occurred at a hospital but at a doctor's office. According to the lawsuit, Lychak was treated by a physician in Bethlehem in a facility affiliated with, employed by or part of Coordinated Health and CHS Professional Practice, but not located on a hospital campus.

Lychak had an examination and steroid injection, later went for an MRI and had a follow-up appointment. He was billed $1,342. That broke down to a physician charge of $255, a pharmacy charge of $61, a patient facility fee of $369 and a procedure facility fee of $607, "notwithstanding the fact that the visit was a non-hospital visit and the treatment was performed in a doctor's office and not at or near a hospital owned or operated by CH Hospital," the lawsuit said. Lychak's health plan requires him to pay 20 percent of medical bills. In the end, he was responsible for $39 of the physician's charge and $511 of the facility fees charges. He had the same outpatient treatment procedure performed previously by another podiatrist at a cost of less than $150 for the visit and injection, according to the lawsuit.

HEALTHCARE NEWS
AMA Calls for Ban on Consumer Advertising of Prescription Drugs & Medical Devices

Responding to the billions of advertising dollars being spent to promote prescription products, physicians at the Interim Meeting of the American Medical Association (AMA) adopted new policy aimed at driving solutions to make prescription drugs more affordable. Physicians cited concerns that a growing proliferation of ads is driving demand for expensive treatments despite the clinical effectiveness of less costly alternatives. “Today’s vote in support of an advertising ban reflects concerns among physicians about the negative impact of commercially-driven promotions, and the role that marketing costs play in fueling escalating drug prices,” said AMA Board Chair-elect Patrice A. Harris, MD, MA. “Direct-to-consumer advertising also inflates demand for new and more expensive drugs, even when these drugs may not be appropriate.”

The United States and New Zealand are the only two countries in the world that allow direct-to-consumer advertising of prescription drugs. Advertising dollars spent by drug makers have increased by 30 percent in the last two years to $4.5 billion, according to the market research firm Kantar Media.

PRACTICE MANAGEMENT TIP OF OF THE DAY

Five Firing Tips for Medical Practice Managers - Part 5

• Give them dignity. Unless the employee poses a serious threat of violence or vandalism, allow him to leave with his dignity. "I strongly recommend against escorting them off the premises," says Ricki Roer, an attorney with Wilson Elser Moskowitz Edelman & Dicker in White Plains, NY, noting employees who sue their former boss frequently state that being ushered out the door was one of the most damaging scars of the process. "It makes them feel like a criminal," she says. "It's hugely embarrassing and what you gain will be so minimal compared to what you will lose." That said, you must still take steps to protect the safety of your staff and privacy of your patients' protected health information by taking his keys, work phone, and laptop, and changing any alarm codes and passwords to which he had access.

QUERIES (PQRS)
Query: PQRS Denial

It appears that everyone I have called has received the identical CMS letter indicating that they “failed” to do the required PQRS reporting in 2014 and will suffer the 2% reduction in 2016. We are proceeding with the Informal Review. We thought we did it correctly. Has anyone received a letter indicating successful PQRS reporting?

RESPONSES/COMMENTS (NON-CLINICAL)
From: Philip Wrotslavsky, DPM

The conversation about investing in stocks reminds me of a story that has been told in many ways, but the gist of it goes as follows. Joseph Kennedy (father of JFK) pulled all his money out of the stock market prior to the crash. The story goes that he knew it was time to get out of the market when he received stock tips from a shoe-shine boy. Take whatever message you want out of this story.
