This weekend, during the annual Live Well with Diabetes conference in Vancouver, British Columbia, Dr. Joseph Stern, President of the Canadian Podiatric Medical Association (CPMA) announced that the 3040 New Balance shoe was awarded the CPMA Seal of Acceptance. Dr. Stern presented a framed copy of the seal to Eric Vassal, the designer of the 3040 New Balance shoe.

Dr. David G. Armstrong, Professor of Surgery and Director of the University of Arizona's Southern Arizona Limb Salvage Alliance (SALSA) provided keynote plenary lectures and standing room only surgery workshops at the Endocrine Society of India Conference (ESICON) in Chandigarh this past weekend. The symposium also heralded the 50th anniversary of endocrinology in India. More than 1,300 physicians attended the lectures.

Six Ways to Avoid a RAC Audit - Part 2
Small- and medium-sized practices are not below the RAC radar and should heed the following succinct steps to avoid and/or minimize exposure.
3. Focus on medical decision-making — The OIG and RACs are focusing on the complexity of medical decision-making as your primary determinant for selecting an E&M code. Do not use a higher-level code when the complexity is not there, regardless of how great your documentation may be.
4. Don't rely on your EHR's E&M code selector — I have yet to meet anyone (other than EHR salesmen, of course) who believes these code-selection engines are consistently accurate. This especially holds true with the rising emphasis on medical decision-making. These engines are good training tools but nothing more, in my opinion. They are particularly weak with medical decision-making.
Consider that the use of Level 4 and Level 5 codes for E&M services has increased more than 60 percent since 2001, as EHRs have become commonplace. These higher level codes are a prime RAC target. Don't let your EHR's code-selection engine be your coding muse.

Query: Venous Insufficiency, Lymphedema and Tendonopathy
I have not seen anything published regarding the venous return nor lymphatic drainage of the Achilles; and I wonder if there is a (published) connection with concomitant venous insufficiency and lymphedema with respect to tendonopathy. Does anyone have any references on this topic to point me to?

Query: Use of Multiple Skin Substitutes
If you apply Apligraf to a wound 5 times (separate surgical sessions over time), the maximum allowed applications by my Medicare contractor, but fail to get closure, would you be allowed to switch to a different skin substitute product (e.g., Dermagraft) to achieve closure? If so, when would it be acceptable (reimbursable)?

Response: I think this depends on your LCD. Make sure your documentation supports the need and explains why you think switching to a different product will make a difference. I would try to pre-certify or contact Medicare before moving forward.

Dr. Krawetz asks, "someone please recommend an excellent digital x-ray system that is reasonably priced." I assure you that the phrases "excellent digital" and "reasonably priced" cannot be used in the same sentence (Depending, of course, on what your definition of "reasonable" is.).
My suggestion is that you call numerous companies and see if they have refurbished systems, or if they have clients who are upgrading their systems and want to, or need to, sell their current digital units to help offset the cost. I have done this three times over the years, and have not been disappointed.

The Dr. Richard Schuster Seminar at the New York Seminar was a rousing success and exceeded my expectations. Presentations were made by doctors from the East and West Coasts. This was one seminar that made me want to stay to the end. The underlying theme to me, paraphrasing Dr. Louis Pack, was that we were on top in the 1970s in biomechanics and maybe the compendium has swung too much away from then. Congratulations to Dr. Joe D'Amico for a job well done.
