ABFAS Announces Surgeons of Tomorrow Essay/Video Contest Winners
 
ABFAS  has announced the winners of the “Surgeons of Tomorrow” essay/video contest. The winners were chosen among entrants from each of the nation’s 10 podiatric colleges, who submitted videos and essays articulating why ABFAS Board Certification is important to them as aspiring podiatric surgeons, and how they will use it to benefit their patients.

The winner for the essay component is Faith Carelli, a 2nd year student at the College of Podiatric Medicine and Surgery at Des Moines University. Remi Drake, a 1st year student at Arizona College of Podiatric Medicine at Midwestern University, is the winner of the video component. ABFAS will cover all costs for the winners to attend the ACFAS 2023 Annual Scientific Conference in Los Angeles on February 9-12, where they will join more than 2,000 of the nation’s top foot and ankle surgeons in sessions discussing the latest advances in the profession.

Education Is Essential For Diabetics: CA Podiatrist 
 
The key to preventing amputations is preventing foot ulcers. To do so, Paul Han, DPM, MS, clinical consultant of podiatric surgery in City of Hope’s Department of Diabetes, Endocrinology & Metabolism, emphasizes patient education as the most effective way to prevent diabetic amputations and infectious foot ulcers. A board-certified foot and ankle surgeon, Han specializes in diabetic foot complications and is an expert in limb preservation in diabetes patients. He has treated patients with diabetic foot complications for more than 30 years.

Han estimates that he performs about five amputations each month that “could have been easily prevented. It is important to teach people with diabetes to be aware of and recognize the early signs that might indicate a risk of foot ulcers and infection, so that a timely medical intervention can be made,” Han said, who recently published the third edition of his book, What’s the Foot Got to Do With My Diabetes? “I want to let our patients know that prevention is possible once they  learn about the disease.”

OH Podiatrist Discusses Diabetic Peripheral Neuropathy
 
Numbness and tingling in the feet is a common and concerning symptom of diabetes. "More than 30 million Americans are living with diabetes, and peripheral neuropathy, or nerve damage in the feet, is one of the most common—and most serious—complications of the disease," says podiatrist Thomas F. Vail, DPM. "Nerve damage causes burning, tingling, heaviness, or numbness in the feet and affects up to 70 percent of all diabetic patients. Neuropathy can be a rather scary aspect of diabetes because patients may not be able to feel pain. If you can't feel an injury or sore, it could lead to a serious infection."

Experiencing nerve damage means people with diabetes must take extra care to not get injured. "If you do experience diabetic nerve damage, foot care becomes even more critical. It starts at home with daily checks on your feet," Dr. Vail says. "Check your feet for any injuries and for changes to the skin, hair, or even temperature of the skin. If you can't see your feet well, try propping up a mirror, or ask friends or family for help. I recommend patients with peripheral neuropathy never go barefoot because of the risk of injuries. People with peripheral neuropathy should see a podiatrist regularly to help catch any changes in their foot health early."

The flexor tenotomy is an excellent procedure for diabetic wounds and painful distal clavi. As we were all taught during surgical training and rotations, choose the procedure that accomplishes the most correction with the least amount of surgery. The closed flexor tenotomy as shown in Dr. Kilfoil's video is a perfect example.
 
A couple of pearls I would like to add are as follows: An excellent blade to use is the Beaver 5910 eye blade with a mini-Beaver handle. Make a longitudinal stab incision around the middle IPJ. This type of incision allows it to actually close when the toe is extended during bandaging. A very simple but effective bandaging can be accomplished by taking a 1" x 3" Coverlet bandage and cutting it lengthwise in a way that divides it approximately 2/3 and 1/3. Positioning the padded area plantarly, apply the Coverlet where the 2/3's part is proximal and the 1/3 tab is distal and actually crisscrossing as it attaches to the top of the digit. This pulls the distal aspect of the toe in an extended position.

When I first attended podiatry school, back in the '70s, the main treatment for verrucae, including mosaic verrucae, was topical Sal acid and weekly debridement. Dermatologists and some podiatrists added on liquid nitrogen therapy. Over the years, there have been credible studies showing that Sal acid treatment is 70% effective. Our problem is how to handle the 30% of the patients who are not responsive and are likely to travel to multiple podiatrists seeking a cure.
 
Unless you want to believe the vendors promoting drugs and surgical therapies, who will tout their products as being cure-alls for warts, we have to face the reality that when it comes to warts, we are still playing with the same deck of cards in 2022 as we were in 1978. Some new and old treatments work for some patients and not for others. In our office, when one treatment fails, we try another. We will either succeed or the patient will move on to another doctor.  As Dr. Sullivan writes, there isn't a wealth of literature proving that any one treatment is the be-all and end-all for treating all verrucae.

I had the same question many years ago. I designed a multiplex PCR test with the idea of correlating the type of HPV and treatment efficacy. However, my research went to a different direction afterward. I'd be interested to see whether this assay could be used in the research.

I don’t have any studies to quote on either the bleomycin or Panacos graft procedures for verrucae treatment. Observational studies are—like any form of empiricism—of little scientific value as “occurrence is not causation” and does not adhere to the guidelines of Koch’s postulates, the Bradford/Hill criteria or Falkow’s criteria. Anyway, I’ve treated flat warts with bleomycin and mosaic warts with the Panacos graft technique since 1999. Over several hundred bleomycin procedures, the cure rate has averaged 98%, with the added benefit that you only treat one lesion, and every wart the patient has (regardless of pedal or manual) are eliminated. I’ve rarely encountered mosaic warts, but of the 32 Panacos graft procedures I’ve done to treat these, they have all resolved. It would be nice to do a controlled trial of some sort, but as there is no money in it, I can’t imagine a grant or sponsoring agency. If these studies exist, I’d love to read them.

The company said patients may not experience the benefits until the third application. I have done the third application to my first patient and will follow him next week. He did say it helped after the second application. I also had a first application on the second patient who said he had the worst pain immediately after the application. But he is looking forward to the second application.
 
The company provides billing information and does the benefit verification prior to each application. They also advertise the clinics where Qutenza is available. I have received two patients from its website in the past three weeks.

Certification of nurses in foot and ankle care only benefits the public. For example, in states such as NY, there is no clean-up bill that includes the right for podiatrists without ankle privileges to take punch biopsies or to treat a condition such as a melanoma on the ankle. Currently, there are nurse practitioners licensed to do so. Thus, if nurses become certified to do so, this benefits the public, increasing access for care since most podiatrists are restricted from treating ankle conditions.
 
Regarding foot care, I feel it is not one's degree that is important, but instead one's competence at providing foot care.
