Prevention of Diabetes is Cheaper Than a Cure: OH Podiatrist
Nearly every table was full Saturday at the PACCAR Medical Education Center as people gathered at the first Diabetes Expo. As part of Diabetes Month, officials at Adena Health Systems hosted more than 100 people to learn more about managing the condition. Dr. John Boyle, a podiatrist at Adena, gave free evaluations Saturday for those who attended the event. While diet and exercise are important in the management of diabetes, he said the health of a patient's feet are as well."Patients with diabetes are at risk for loss of sensation in their feet. When that happens, that puts them at risk for injury and infections that could go unnoticed," Boyle said.

Boyle recommends that patients with diabetes look at their feet or have someone do it for them everyday. "It's easier to keep out of trouble; prevention is easier and cheaper than a cure," Boyle said.

Five Firing Tips for Medical Practice Managers - Part 4
• Have a witness. Always include a third party when telling a staff member her time is up. Others recommend recording the exit interview. "There are bad employees out there and you have to use safe hiring and firing practices," says Keith Borglum, a medical practice consultant with Professional Management & Marketing in Santa Rosa, CA, recalling one employee of a former client who feigned a back injury after being fired (for failing to disclose she was on work release from prison) and later filed a worker's compensation claim.

Query: Encounter with a Painful Normal Nail
When a Medicare patient presents complaining of toenail pain without any signs (i.e., no class finding and no paronychia) other than pain, can a low level E/M service code be billed if the nail was debrided, dressed, and topical antibiotics and soaks advised?

Response: It all has to do with 1) what was actually done and 2) documentation. If you have a normal nail, with no class findings and no paronychia, how do you know that the nail is causing the problem? Normal nails are trimmed, not debrided (this implies thickness), so be careful of your terminology. Why would you apply a topical antibiotic to a nail that has no infection?
Yes, you can evaluate any problem and bill an appropriate E/M level if there are symptoms present. Where the problem arises is when doctors try to embellish the presentation as a way to get non-covered routine foot care paid. Otherwise, it is a CASH service.

RE: Forced to See Patients for Free?

I just received a letter about Health Republic. Health Republic is one of the larger insurance companies in our area, and apparently they just stopped processing claims on Nov. 5 without telling us. This letter appears to say that we are not allowed to balance bill the patients – that they are only responsible for the co-pay and deductible even though it looks like we will not be paid by the insurance company – or might get “modest payments.” Furthermore, it appears that NY State law says that we have to see these patients for free for the next 60 days if they go on a plan that we do not accept. The new plans start December 1, so all of these patients must be seen for free until then.
Could that be the correct interpretation?

Wow, Stating that smoking is EVIL is pushing it a little for me! We pay taxes (invest in our country). I wonder what “EVIL” is done with that money!

It has become a standard ancillary question on forms, "Do you smoke(?)". Dr. Rosenblatt's post invokes the ancillary answer that even podiatrists should relate to their patients. The podiatrist should tell patients; "Smoking is bad for your health. You should quit." The "you should quit" order will have a powerful impact, and it is the physician's duty. People do not like the rising costs of healthcare. However, under the ACA, all taxpayers are paying the price for people who smoke. I recall a day in risk management when we were instructed that technically the physician who does not "warn" patients about the health risks of smoking is liable. Yes, years later, the patient can allege that you never told him to quit. As a podiatrist, are you investing in the rising costs?

I do not invest in companies with products that are harmful to our health, such as soft drinks and tobacco. Even though I like to support local companies, I clearly told my financial adviser that I will not invest in Hormel, General Mills, or Kellogg.
Consider poor lifestyle choices like smoking decades ago when doctors were smoking and even prescribed smoking. It took 7,000 scientific publications for the government to say that smoking is harmful to the health. How many people died of related problems waiting for the government to act on it? If we as physicians cannot recognize and adopt the concept, how do we educate patients? Developing new surgical techniques and medical technology to treat diabetic ulcers will not help if patients keep eating unhealthy foods.
