MD Podiatrist Discusses Advances in MIS
 
Noman A. Siddiqui, DPM, MHA, director of podiatric surgery at the International Center for Limb Lengthening and chief of podiatry at Sinai and Northwest Hospitals, said the use of low-speed, high-torque burrs have allowed surgeons to preserve patients’ soft tissues.

“In the past, there were burrs that you could use for MIS; however, the challenge was they were high-speed, high-torque, not as accurate and potentially harmful to the soft tissue and bone if used without strict control,” Siddiqui told Healio | Orthopedics Today. “However, more recently, there has been more focus on utilizing high-torque, low-speed burrs that can specifically allow you to focus on areas that would be more challenging if you were utilizing a burr that was high speed, high torque. Low-speed, high-torque allows you to make an osteotomy and not disrupt the soft tissue,” he added.

Aaron Chambers, DPM is currently an officer in the United States Public Health Service Ready Reserve Corps and is a former officer in the United States Navy. He earned his DPM degree at Western University of Health Sciences. Afterwards, he completed his surgical residency at the Loma Linda Veterans Affairs Healthcare Systems and traveled the country as a uniformed service podiatrist. He is currently board-certified in Podiatric Medicine, Foot Surgery, and Reconstructive Rearfoot and Ankle Surgery.

In the heart of Rock County, there are two generations of podiatrists — four people in one family — who proudly serve their community. Greg Zainer, DPM, a recently retired MercyHealth podiatrist, is at the forefront of his family’s medical dynasty. He said he never expected his children to follow in his footsteps but is proud they did.

CA Podiatrist Issues Pedicure Warning to Diabetics
 
Dr. David Armstrong, Professor of Surgery at the Keck School of Medicine of the University of Southern California, recently told Medscape.com that even a pedicure or massage treatment could inadvertently result in a serious foot problem if you get a cut or bruise. He said, "A minor nick during a pedicure or excessive pressure during a leg massage could potentially spiral into serious complications." However, he stopped short of advising a ban on leisure foot treatments for diabetics altogether, saying that patients should instead consult with a health professional first and follow his four key rules if they are hoping to get pampered.

These include avoiding pedicures if you have active infections, open sores or cuts, requesting that foot baths and tools are properly sterilized before use, and ensuring you visit a properly licensed salon rather than attempting it yourself, as this can heighten the risk of injuries. You should also make sure you are not shaving your legs in the 48 hours before a pedicure, as tiny cuts can potentially act as a gateway to infection.

Query: Class Findings and Absent Pedal Pulses
 
I work closely with a vascular surgeon and have a hand-held Doppler in every room. Now, many times I cannot feel a pulse, but I can find it by Doppler. So, reading Medicare rules on Q8 class findings, if I cannot feel it… is it an absent pulse? Is the definition of an “absent pulse” the fact that you cannot feel it? Or must it be absent by Doppler as well?

Response: the answer is, if you can’t feel it, as far as class findings are concerned, it is absent. The LCD also does not mention if the pulse is absent/present on the left or right foot only. While you should document laterality, the NCD/LCD do not mention it. If the dorsalis pedis (DP) artery is absent on the left and present on the right, as far as I am concerned (and I have won these on appeal) the DP pulse is absent from the perspective of the policy. Of course to qualify as Q8, you would need more (posterior tibial pulse is absent) or 3 other trophic changes to qualify.
 
Advanced trophic changes (at least three of the following):
Decrease or absence of hair growth
Nail thickening
Skin discoloration
Thin and shiny skin texture
Rubor or redness of skin
