APMWA Announces 37th Annual Student Writing Competition
The American Podiatric Medical Writers Association (APMWA) has announced its 37th Annual Student Writing Competition.
-
All papers MUST be non-technical in nature. Appropriate subjects include practice management, ethics, law, education, healthcare, or any topic that would be suitable for a lay publication.

There is no word limitation. Papers will be graded for content, style, grammar, neatness, and overall impact.
-
First prize one thousand dollars ($1,000), Second prize seven hundred and fifty dollars ($750) Third prize five hundred dollars ($500) sponsored by an APMA Educational Foundation endowment from Dr. and Mrs. Steven Berlin, and recognition in the APMA NEWS and the APMWA Newsletter. Honorable Mention Certificates may also be awarded.

Proper Footwear Can Prevent Pickleball Injuries: DC Podiatrist
Pickleball has exploded in popularity in recent years, especially among those 50 and older. And, as is often the case when a new sport catches on, injuries are growing along with participation. DC podiatrist Sheldon Laps, DPM says he commonly sees inversion ankle sprains from pickleball, injuries that occur when the foot twists upward and the ankle rolls inward, often the result of inappropriate shoes. Many runners who take up pickleball assume they can play in running shoes: Don’t.

“Many running shoes today have a ‘rocker’ sole, that is, viewing the shoe from the side, the forefoot of the shoe is curved up,” Laps says. “This aids push-off off during running and walking. But rocker-type soled shoes should not be used to play a court sport. The side-to-side motion on a pickleball court increases the likelihood that the player will twist the foot at the ankle and thus, sprain the ankle.’’

MA Podiatrist Participates in Webinar Panel on DFUs
David Alper, DPM, a member of the APMA Board of Trustees, was an invited guest panelist on a webinar/radio show combination hosted by the patient advocacy organization, The Way to My Heart, on November 12th. Broadcast on SRO radio station in California, the discussion revolved around the importance of off-loading and footwear to allow proper healing and prevention of recurrence of DFUs.

The panel consisted of an interventional cardiologist, a wound care nurse from the United Kingdom, a post-TMA patient, and a designer of an off-loading walking boot. It was joined by over 5,000 listeners.

HEDIS® measure Can Reduce Diabetes-Related Amputations: NY Podiatrist
To combat diabetes-related lower extremity amputation, Arche Healthcare has founded LEAP 2030 and the HEDIS PedisTM initiative. The goal of LEAP 2030 is to establish a formal HEDIS® measure for diabetic foot assessments, called the HEDIS Pedistm. The HEDIS PedisTM would ensure the early detection of risk factors for diabetic foot ulcers, leading to a reduced incidence of lower extremity complications of diabetes. A similar HEDIS® measure already exists for diabetes-related retinal exams, preventing blindness in millions of patients with diabetes.

Scott Kantro, DPM, CEO of Arche Healthcare, founded the LEAP 2030 initiative because he believes adding a formal HEDIS® measure is imperative to significantly reducing diabetes-related lower extremity amputations by 2030. "The problem of diabetes-related amputations has only gotten worse in recent years," says Dr. Kantro. "It's time for action. We can enormously impact people's lives by implementing a HEDIS® measure. Healthcare providers can start now by performing an annual three-minute foot assessment."

Five Telephone Etiquette Tips for Medical Practice Staff
Meeting patient expectations is the surest way to increase their satisfaction with your practice. Telephone management is a good place to start.
4) When taking a message, tell the caller when there will be a response.
The objective here is to minimize uncertainty and manage expectations. If the caller knows that there will not be a callback until the clinic session is over, she won't expect one sooner. That makes it less likely she will call back three more times before noon - an obvious boon for staff productivity. Of course, she also needs to know that she can expect a call no later than the start of the afternoon clinic at 1 p.m.
This only works when physicians and staff members keep the practice's promises to patients. Some will object to this practice because there may not be an answer available by the promised time. That's OK. Call the patient back within the promised window, explain the delay, and tell him that you will call again when you have an answer to his question.

Query: Treatment for Plantar Warts
I have been unsuccessfully searching for articles on comparison of success rates of different treatments for plantar warts. Specifically, type of wart i.e.: plaque-like vs. punctate, location weight-bearing or not. There seems to be some articles on using different methods: laser, bleomycin, curette, cryo, acid, and even oral meds, but I can't find a comparison for well designed studies on success rates. Any help out there?

Query: Qutenza
Has anyone had experience with Qutenza? This is the 8% capsaicin application that is performed in the office. I'm looking for clinical thoughts, e.g. did the patient tolerate it well? Did it help? Also billing advice. It is quite expensive. Does the company do a pre-authorization and stand behind it?

Great video! Here are a few pearls I’ve learned through the years:
1. I have had success with this procedure, even with rigid deformities. My thinking is that release of the FDL tendon removes the dynamic force driving the toe down into the ground while the patient is standing/walking - not completely, but just enough pressure reduction to heal the ulcer, particularly in patients who are poor surgical candidates that I don’t want to do arthroplasty or amputation on.
2. I prefer to perform this procedure while the patient is awake, so they can contribute and actively flex the toe to confirm release of the tendon with a gratifying “pop.” If I’m doing the procedure in the O.R., combined with other procedures, then I do the FDL release first while the patient is awake.
3. I have evolved to do all 3 middle toes at the same time, regardless of which one has the ulcer. Because, as Dr. Kilfoil showed, inevitably the middle toes you DON’T release will eventually develop a transfer ulcer. We affectionately call this trifecta procedure the “Rice Krispy Special…Snap, Crackle, Pop!"

RE: Podiatrists and the Prevention of Back Pain

I feel we, as podiatrists, can have a big input in caring for the elderly in preventing the need for back surgery. The back surgeons may not be happy, but I do not care. This is for the betterment of outcomes. My whole life, I have worn orthotics with lift therapy for a pelvic rotation, allowing me to subsequently become a runner and lose 60 lbs. I have had the unique opportunity working for a back surgeon. After about 9-10 months, the orthopedic back surgeon came up to me asking what I was doing with his patients? After being puzzled by this, I responded, “I hope helping your patients.” Then he said, “Not the patients I wanted to do surgery on."
Then I started realizing that as a podiatrist I had future many back patients walking into my podiatry office. I was helping so many elderly people with orthotics and biomechanics that I feel we are missing out on a population that we can save by eliminating debilitating back surgery. What I discovered is that you either have a limb length or pelvic rotation, or as you age, you develop one of these conditions. With heel lifts and custom orthotics, I was able to help a lot of people from needing back surgery.
