Holidays Increase the Risk of a Gout Attack: FL Podiatrist
Your feet can sometimes feel the effects of too much holiday cheer. Foods and beverages high in purines, such as shellfish, red meat, red wine, and beer can trigger extremely painful gout attacks, a condition when uric acid builds up and crystallizes in and around your joints. According to foot and ankle surgeon and Associate Member of the American College of Foot and Ankle Surgeons, Josh Sebag, DPM, “Often, it’s the big toe that’s affected first since it’s the coolest part of the body and uric acid is sensitive to temperature changes, but this may also occur in other parts of the foot and ankle.”

If you do experience pain in your big toe or other parts of your foot or ankle, drink appropriate fluids, elevate your foot, and immobilize it. Frequently, non-aspirin equivalents of non-steroidal anti-inflammatory medications are used to treat gout. In severe cases, contact your foot and ankle surgeon.

Harry John Visser, DPM was recently elected to the Board of Trustees (BOT) of Hiram College to a 6-year term. Hiram College is a liberal arts institution located in Hiram, Ohio. It was founded in 1850. Former U.S. President James A. Garfield had served as its president from 1851-1854.

DFUs Require Immediate Action: Bahamas Podiatrist
According to Monique Mitchell, DPM, MPH, "A diabetic foot ulcer is an open sore or wound that occurs in about one quarter of patients with diabetes. They are commonly located on the bottom of the foot. These ulcers increase admission to the hospital and amputations, with about a quarter being admitted to the hospital and a quarter of them having an amputation. Diabetes is the leading cause of non-traumatic lower extremity amputations, and 85 percent of people who get an amputation had an ulcer first. Research has shown, however, that development of a foot ulcer is preventable, in most cases."

"Once an ulcer is noticed, seek medical care immediately. Foot ulcers in patients with diabetes should be treated quickly to reduce the risk of infection and amputation, improve function and quality of life, and reduce healthcare costs. The primary goal in the treatment of foot ulcers is to obtain healing as soon as possible. The faster the healing, the less chance for an infection," says Dr. Mitchell.

APWH Holds Virtual 2021 Scientific Symposium
The 2021 Academy of Physicians in Wound Healing (APWH) Virtual 2021 Scientific Symposium featured the largest hands-on biopsy “virtual” workshop ever held. More than 150 (of the 220) registrants received biopsy kits with surgical instruments and tissue specimens. The two-hour workshop by Dr. Steve McClain serves as a model for future virtual educational events.

Lectures by Pamela Scarborough and Joyce Black discussed skin changes with COVID-19 and early onset symptoms that previously had not been recognized. Most importantly, both speakers indicated many COVID-19 related skin lesions are currently being misdiagnosed as deep tissue injury. This misdiagnosis can lead to several complications in addition to the lack of appropriate treatment. This includes billing and potential legal implications.

Prioritizing Foot Ulcers During Lockdown Paid Off in Belgium
Belgium's efforts to prioritize care at its diabetic foot ulcer clinics during the COVID-19 pandemic lockdown minimized both delays in care and adverse outcomes, new data suggest. The country has had a national diabetic foot care program in place since 2005, with 34 multidisciplinary clinics recognized by the Belgium Ministry of Health for the treatment of diabetic foot ulcers (DFUs). The system has resulted in a decline in lower-limb amputations in the country.
And while all non-urgent medical care in Belgium was postponed during the national COVID-19 lockdown period March 14-May 3, 2020, the clinics followed advice from professional associations to consider all active DFUs as urgent. Thus, nearly all foot clinics remained accessible during lockdown.

Invest the time and money up front to develop a memorable name, a strong logo, and a mission statement that communicates to potential patients what your practice is all about. And remember that rebranding costs significantly more than branding, so it’s best to take a “measure twice, cut once” approach for marketing materials.

Query: Complicated Verruca Treatment
The patient had a large plantar verruca that we chose to excise in the operating room. The lesion was at the left foot and the total size of excised area was 2.5 cm x 2.5 cm x 0.2 cm depth (including some subcutaneous tissue). The site was debrided and prepared for grafting. I applied a Grafix graft. Can I bill the preparation of wound bed? Can I use CPT 15004? What other options are there for billing this plantar verruca removal and graft application?

Response: There is no indication as to the insurance carrier involved as each may have their own rules on the use of products such as Grafix. There is also no indication as to the medical condition of the patient. Is this a healthy young person or a debilitated person? Products such as Grafix tend to be covered for certain wounds, primarily diabetic or venous leg ulcer/wounds. Yes, indications for products can expand over time as new indications are found.
With any wound, there is a potential for the wound to heal on its own accord. It may take a combination of local care, offloading, dressing, antibiotic therapy, etc. to get the wound to heal. When it does not heal, then other modalities such as skin substitutes may come into play. These products are expensive and should be used judiciously. Ultimately, taxpayers pay for these products. The first issue is why is ANY skin substitute required in this case versus just allowing the wound to heal on its own? How much faster will this wound heal with or without the graft? Is there literature proof?
Secondly, wound preparation is removing nonviable tissue (infected, non-viable, etc.) to have a clean, vascular base to then add a skin substitute. In this case, the only “prep” is removing the wart which you were paid for. What else was done beyond that? Is it the standard of care to use skin subs for warts or any other non-wound procedures we perform? This is what you must prove in your charting.

RE: The Passing of Anthony P. Tocco, DPM
Dr. Anthony Tocco, 76, of Daytona Beach, FL went home to be with the Lord, November 2, 2021. Dr. Tocco served as a local podiatrist in Daytona Beach and New Smyrna Beach for the past 45 years. He graduated and was a football star at Cardinal Mooney High School and played college football at the University of Miami Hurricanes. His senior year he played in the 1966 Liberty Bowl.

Tony Tocco went to podiatry school and completed his residency in Cleveland, Ohio. Dr. Tocco moved to Daytona Beach in 1974 and open his practice on Peninsula Drive Daytona Beach and on US 1 in New Smyrna Beach. Dr. Tocco loved his family, golfing buddies, high school teammates, being Italian, and watching his Miami Hurricanes.

Thank you, Dr. Wallace, for all you have done for our profession. You have been a wonderful teacher and mentor for many residents. I have enjoyed our friendship and teaching with you for years. I agree that we all do difficult surgeries that are meant to help patients, but sometimes the deformity is so bad that all we can do is attempt to make it better.
Last week, I did two flatfoot surgeries, one triple arthrodesis, one ankle ligament repair, two bunions, and two amputations. It was so nice to go to the office and correct an ingrown nail. I knew the ingrown nail procedure was easy and so successful and so appreciated by the patient. I think all of us who have practiced for many years still do the complicated surgeries but also enjoy the easy procedures.
I agree that making money should not be your driving force but helping the patient needs to be that driving force. I also think patients have to be educated about the surgical procedures and the risks involved and they must have realistic expectations of surgery. There have been times that I declined to perform surgery on some patients because of their unrealistic expectations of the results of the proposed surgery.
