PODIATRISTS AND SPORTS MEDICINE
FL Podiatrist Featured in Orthopedic and Sports Medicine Physician Forum

The College of Medicine’s Orthopedics and Sports Medicine Interest Group hosted the first “Orthopedic and Sports Medicine Physician Forum” on November 14. The panel included a talented group of orthopedists and sports medicine specialists who shared their personal experiences with sports injuries, training, and answering questions from the audience. The panel included Gideon J. Lewis, DPM, UCF alumnus, sports medicine physician, and foot and ankle surgeon.

Dr. Gideon Lewis

Dr. Lewis, a College of Medicine volunteer faculty member and adviser for the student group, hopes to use this collaboration to expand sports medicine interest and awareness in the Orlando community and beyond. “This involves forming connections with other sports medicine and orthopedic specialists, as well as professional sports teams in Orlando,” he said. “Our goal is to host many more events in the coming future to spark interest in orthopedics and sports medicine, promote healthy living strategies, and contribute to the UCF community and greater Orlando area.”

PODIATRISTS AND FOOTWEAR
NY Podiatrist Provides Tips on Shoe-Buying for Feet with Bunions

Dr. Karen Langone, a podiatrist based in Southampton, N.Y., has some suggestions for those suffering from bunions, particularly when it comes to footwear choices. “We can hasten their development with improper shoe choices,” she said, advising patients to visit a shoe store where they can be serviced by experienced shoe fitters who can address their special needs.

Dr. Karen Langone

Shoes should provide ample support underneath the bunion area, advised Dr. Langone. Here, some brands cheat and use the same sole and simply add a wider upper. Make sure both the sole and upper are done in a wider width so the foot is not hanging off the sole. Look for suppler upper materials that will be gentle on sensitive bunion areas. “Look for a soft leather or suede and stay away from patent leather, which is going to be more harsh,” said Langone. “One of the things I tell my patients is, 'you don’t break in shoes.'”

MEETING NEWS
NYCPM’s 2-Day Schuster Seminar Assembles Large Cadre of Biomechanics Experts

A one-of-a-kind gathering of leaders in the field of biomechanics distinguished the 2018 Richard O. Schuster, DPM Memorial Biomechanics Seminar, held November 10 and 11 at NYCPM. The seminar was coordinated and chaired by Joseph C. D’Amico, DPM, a professor and past chair of the department of orthopedics and biomechanics at NYCPM. The seminar was made possible in part by an unrestricted educational grant from a grateful patient of Dr. D’Amico, who the doctor cured of his debilitating foot pain and who wants to make available to all the techniques that led to his cure.

Back row (L-R) Drs. Paul Coffin, Mark Caselli, Russell Volpe, Donald Greene, R. Daryl Phillips, Jeffrey Cusack, Jinsup Song. Front row: (L-R) Drs. Barbara Resseque, John McNerney, Joseph D’Amico, J. David Skliar, Loretta Logan, Robert Eckles.

Held over two days for the first time, the packed daily agendas reflected the seminar’s chief mission: to provide a meaningful historical and clinical review as well as current relevancy of the contributions of Richard O. Schuster, DPM to the field of podiatric biomechanics, pediatrics, and sports medicine. John McNerney, DPM examined Schuster’s concepts, while other speakers looked at various podiatric issues and problems through the lens of biomechanics; they looked at gait, orthoses, pediatric issues, and even the “Legal Aspects of Biomechanics,” presented by Barry Block, DPM, JD.

PRACTICE MANAGEMENT TIP OF THE DAY
Reasons to Say No to Payer Contracts - Part 1

A payer contract is more than a mere fee schedule. There are several other factors to consider when deciding to keep or terminate a payer contract.

1. Overflowing appointment schedules

If you have more patients than you have room for or time to see, you have leverage with any payer who makes up five percent or less of your revenues. Let the payer who pays least and the payer who is the biggest pain know that they need you more than you need them. Make your issues with them—be they lengthy credentialing, a high denial rate, and/or low fees—their issues. These are the issues that must be addressed to your satisfaction if you are to remain in their networks.

CODINGLINE CORNER
Query: Removal of Implant and Metatarsal Resurfacing

What procedure coding would you suggest using for the following procedure: Arthrosurface HemiCap implant removed. Bone graft placed in distal 1st metatarsal head defect. First metatarsal head reshaping and then orthobiologic graft was used to resurface the distal 1st metatarsal head.

William Long, DPM, Simpsonville, SC

Response: Many insurance carriers would not cover the bone graft or the orthobiological dressing. That would include the cost of the graft and the application. Removing the old implant may be considered part of the overall procedure. That leaves you with first metatarsal head reshaping which would be CPT 22888 or possibly the unlisted code 28899.

I would pre-authorize this and be very clear what you are doing so that there are no post-op surprises with either the patient or facility on the hook to the cost of the graft or a denial of part or the whole procedure.

Tony Poggio, DPM, Alameda, CA

RESPONSES/COMMENTS (NEWS STORIES) - PART 1
From: Ira Baum, DPM

Thank you to Rock Positano, DPM for your most important action on the inappropriate CMS proposal. Directly presenting the value of podiatric services to a bureaucracy via its decision makers hopefully will add to the mass communication provided by the APMA and its membership. This allows me segue into the value membership in the APMA is to all podiatrists. Financial considerations almost always come into play when considering joining the APMA, but if the proposal to segregate us and reduce fees to our profession ever passes, over the long-term, the cost of membership will be “a spit in the bucket”

In the past, I have been critical of the APMA on other issues, but Dr. Positano and the APMA’s efforts in response to this attack were commendable. Sitting on the sidelines and benefiting from your colleagues who are members of the APMA, is unethical and immoral, particularly when non-members profit equally. Reconsider your decision and join the APMA.

RESPONSES/COMMENTS (NEWS STORIES) - PART 2
From: James Yakel, DPM, David Kaufmann, DPM

Dr. Dananberg has been instrumental in exploring sagittal plane biomechanics and how it affects gait. If Dr. Kaye was current with biomechanics and orthotics, I’m sure he would be aware of Dr. Dananberg’s contributions to podiatric biomechanics. I applaud Dr. Dananberg’s professional response.

James Yakel, DPM, Longmont, CO

Dr. Dananberg has devoted a lifetime to the science of biomechanics, both clinically and within the research settings. As one familiar with his training and experience, he is at the apex of his specialty. Dr. Dananberg poses interesting questions and theories with solid evidence behind his thoughts. I find it perplexing that someone would question his academic reputation in such a disrespectful way. In Dr. Dananberg’s case, the legal phrase applies: res ipso loquitor (the thing speaks for itself).
