APMA NEWS

APMA to Meet in Hawaii in 2014 & 2021

The American Podiatric Medical Association (APMA) announced Thursday that they have booked annual meetings at the Hawaii Convention Center in 2014 and 2021. The association last met in Honolulu in July, bringing together about 3,500 podiatrists and related professionals.

Statewide spending from the meeting was more than $16.5 million and generated more than $1.6 million in statewide tax revenue. About 28,000 hotel room nights were booked.

NY Podiatrists Featured at Int’l Clubfoot Deformity Symposium

The First New York-International Clubfoot and Pediatric Deformity Symposium was held November 9, 2008 at Elmhurst Hospital Center. Sponsored by the Mount Sinai Department of Orthopedic Surgery and Podiatry, the conference was attended by approximately 150 podiatrists, medical doctors, nurses, and physical therapists.

The international faculty was highlighted by Prof. Henri Bensahel of Robert Debre Hospital in Paris, France; Wallace Lehman, MD of NYU-Hospital for Joint Diseases; Prof. N.S. Laud of Mumbai, India, and John Clegg, MD of Coventry, UK. Wayne Axman, DPM and Steven Yager, DPM, directors of the Podiatric Residency at Mount Sinai Hospital, and Robert Fridman, DPM of Columbia University Medical Center were the featured podiatric lecturers of the pediatric deformity track. The symposium concluded with a hands-on workshop on the principles of differential distraction.

QUERIES (CLINICAL)

Query: Varus 5th Hammertoe in a 12 Year Old

I am interested in surgical suggestions/options for a healthy athletic 12 year old female with painful progressive, severe left fifth varus hammertoe deformity, semi-reducible. There is complete under-riding of adjacent 4th toe with dorsal displacement of 4th toe (reducible so far). No dorsal MTPJ contracture. X-rays show open epiphyseal plates at the proximal phalanx base and fifth metatarsal head.

She has had progression over 4-5 years with previous multiple types of padding, splints, taping, shoe changes, etc. Mom is very concerned and has had conflicting surgical opinions. She now needs surgical intervention, but I am concerned about growth plate disruption if utilizing K-wire (smooth) at proximal phalanx/ fifth metatarsal. Any suggestions, techniques, pearls appreciated.

RE: First and Second Ray Reconstruction S/P Joint Implant (James Breedlove, DPM)

I would remove the implant, debride out any necrotic/non-viable bone from both the met and the phalanx, fill the implant stem defects with DBM, but fuse the 1st MTPJ with an iliac crest graft (autogenous would be best, but can be done with fresh frozen) fixate with a plate, and get an external bone stim on the patient post-op.

With regard to the short 2nd ray, I would find it easier to shorten the 3rd and 4th mets with simple Weil osteotomies. This should bring you back to a more normal parabola with less work than trying to elongate the 2nd. I think this plan will give you a good result without all the additional surgeries and complications associated with the fixator and lengthening procedures.

I have had a 20/20 DR System for over 2 years and I am very satisfied with the system and their service. Last summer, there was a power surge which unfortunately resulted in "frying" my DR system. I called Reina and had a new system in my office the next day before 11am. I have no financial relationship with 20/20.

From the standpoint of speed and functionality, the 20/20 NAOMI DR System works well. Resolution is better than plain films. Laterals and upright ankle films have a grainy appearance along the plantar-most soft tissue margin. This can be corrected with a 1-inch foam spacer under the foot, and by focusing the beam just over the foot rather than what one would do with standard film. Images are readable within 10 seconds from the time of exposure. Software is trouble-free.

The system can be used easily without an assistant as there is NO developing; it is essentially a big digital camera. Remember, even if the plates are digital, you can't take the next patient's films until you have cleared the films from the first patient. With the DR, I can take 10 views of both feet and not hesitate before filming the next patient.

20/20 is a great company to do business with. Tech support is unbelievable. I had a power outage and Cox changed my IP address in the same week. I didn't wait more than 20 seconds to have a tech help me re-establish network connections. They also included the setup/shipping and installation in their price quotes. Make sure you compare apples to apples.

Congratulations to those who have worked so hard to create the ASPS. Our profession is too small for two organizations to represent similar interests, provide similar services while competing for the same resources. I look forward to an organization which will supplement the efforts of the APMA and which will be responsive to the majority of podiatric surgeons. I’d be happy to participate in whatever manner is required to get this organization off the ground.

RE: AAPPM Presents Initial Egerter Award

The Egerter award, presented to Barry Block is more than earned. I was one of those students lucky enough to actually take a course from Dr. Egerter. We tend to take for granted talented and ground-breaking leaders of our profession until they are no longer with us. For this reason, I'm delighted that Dr. Block has received recognition for his achievements now.

Like Dr. Egerter, Dr. Block put his locomotive directly in front of us, helping every podiatrist achieve the personal and financial goals we dreamed about from our first days in podiatry college. In a time when there has been over-whelming criticism against the profession from both inside and out, Dr. Block decided to actually do something to improve our lot, rather than just complain.

I offer my heartfelt congratulations to Barry for a job well done. May he continue a long, healthy, happy life. Believe me, it is Dr. Block who has given to us, not vice versa.
