Your Feet Hurt? Blame the Pandemic
There was a sharp decline in foot trauma early in the pandemic, said Robert K. Lee, DPM, chief of podiatric foot and ankle surgery at UCLA Santa Monica Medical Center in California, but his practice was soon inundated with patients with foot pain. For those who started running, Achilles tendonitis has been common.

With spring came less stringent regulations and people have started taking to the streets, said James Hanna, DPM, past president of the New York State Podiatric Medical Association. Many are aggravating existing injuries or creating new ones. “People thought they could just go back to their routine or try something they haven’t tried in a couple of years, but their feet weren’t ready for what their bodies want to do,” he said.

Foot Ankle Health Hosts Successful Foot Screening at Annual APHA Scientific Meeting
The Foot Ankle Health Section of the American Public Health Association (APHA), co-chaired by Rachel Albright, DPM and longtime committee member Janet Simon, DPM, presented at the annual scientific meeting in Boston this past Monday and Tuesday. Aside from giving four talks on the latest research by students, and manning an information booth for all attendees to receive information on podiatric medicine, the section celebrated their 50th anniversary of its existence by hosting a foot screening offered to all attendees.

David Alper, DPM and three podiatric residents examined the feet of over 75 eager folks, turning away over 100 attendees due to the three-hour limit. A computer was manned by other committee members to make referrals to APMA member podiatrists for all seeking additional care across the country, from Seattle to Fort Myers.

Chlorhexidine Gluconate Yielded Positive Results for Irrigation of Bone Tumors
According to published results, use of a 0.05% chlorhexidine gluconate solution may serve as a chemical adjuvant to intralesional curettage for patients with chondrosarcomas and giant cell tumors. Chris Moore, BS, and colleagues at the University of Washington School of Medicine analyzed the in vitro cytotoxicity of commonly used irrigation solutions on human chondrosarcoma and giant cell tumor (GCT) cells. Cells were irrigated with several independent test solutions for 2- and 5-minute intervals.
Researchers found the 0.05% CHG solution was non-inferior to the HCC solution when chondrosarcoma cells were exposed for 5 minutes (mean cytotoxicity, 99%) and when GCT cells were exposed for 2 minutes and 5 minutes (mean cytotoxicity, 102%). They also noted sterile water was superior to the LCC solution when chondrosarcoma cells were exposed for 5 minutes (mean cytotoxicity, 28%) and when GCT cells were exposed for 2 minutes (mean cytotoxicity, 37%). Sterile water and the 3% hydrogen peroxide solution were both inferior to the HCC solution, whereas the 0.9% saline, 70% ethanol, and 0.3% povidone iodine solutions were all deemed non-superior to the LCC solution under both time intervals.

Five Telephone Etiquette Tips for Medical Practice Staff
Meeting patient expectations is the surest way to increase their satisfaction with your practice. Telephone management is a good place to start.
3) Don't let the phone ring half a dozen times.
A prompt rollover to voicemail also reduces the caller's frustration. Each ring is a promise that someone will answer the call, raising expectations that are not going to be met. If no one can get to the phone within two or at most three rings, staff is obviously otherwise occupied. All the extra rings do is add to the noise and sense of stress in the office, which is not good for the patients onsite or the staff.

Query: Recoupment from Retired Podiatrist
I am a retired DPM and closed my business. Medicare is wanting recoupment on some DME items from 3 years ago. They can’t recoup it through the claims obviously since I’m not practicing. They are asking me to directly send in the money. Do I need to pay this and then appeal? Should I appeal and not pay? What are the rules in this situation?

Response: It does not matter that you are retired; you received money from Medicare and they can go back more than 3 years if they find you at fault. If you have records to substantiate your dispensing, then all that is needed is to create a brief that outlines each patient's care and why the device was medically necessary. In this situation, you do not pay anything back yet. If you don’t have experience in these audits, I recommend contacting an experienced expert, healthcare attorney, and/or malpractice carrier (PICA provides ADC coverage if you had them as your prior carrier). If you don’t want to appeal, then pay them back. But know this repayment could lead to more requests by not fighting this initial request.
