NJ, MD Podiatrists Discuss Home Remedies for Athlete's Foot

Cornstarch, a common kitchen ingredient made from corn, can also treat athlete's foot by absorbing excess moisture from feet, says Jason Galante, DPM, a double-board certified foot and ankle surgeon in New Jersey. That's because keeping your feet dry prevents the spread of this fungal infection. If you don't have cornstarch, you can try drugstore foot powders like Dr. Scholl's, which can be helpful as well, says Jon Sherman, DPM, a board-certified foot surgeon and fellow at the American College of Foot and Ankle Surgeons.

(L-R) Drs. Jason Galante and Jon Sherman

Because athlete's foot is so contagious, it's important to disinfect items that may be harboring fungi, Dr. Sherman says. To prevent re-infection, it's always a good idea to disinfect your shoes. One way to disinfect your footwear is with an ultraviolet-C (UVC) light, some of which can kill viruses, germs, and fungi in 15 minutes, says Sherman. These can be purchased at home stores or podiatrists' offices. Another option is spraying your shoes with a disinfectant spray, like Lysol. This can be done once a week for a month and then once a month after that, Dr. Galante says. "You want to reduce any kind of pathogens or fungal spores that could live inside your shoe."

Arterial and Venous Thromboembolic Complications in Patients With COVID-19

A cohort of 1,114 patients with COVID-19 was followed to evaluate arterial and venous thromboembolic disease. The rate of use of prophylactic anticoagulation was 89.4% among patients in intensive care, and 84.7% among those hospitalized but not in intensive care. The frequency of major arterial or venous thromboembolism, major cardiovascular adverse events, and symptomatic venous thromboembolism was 35.3%, 45.9%, and 27.0 %, respectively, in the intensive care cohort, and 2.6%, 6.1%, and 2.2%, respectively, in the non–intensive care cohort. No events were observed in patients treated in the outpatient setting.

The risk of arterial and venous thromboembolic disease is high among patients requiring intensive care treatment for COVID-19 despite a high rate of use of prophylactic anticoagulation.

PRACTICE MANAGEMENT TIP OF THE DAY
Reducing Medical Practice Embezzlement Risk with Cash Controls - Part 6

Front-desk embezzlement schemes are not uncommon and they are typically the result of loose protocols and lack of oversight. Reduce your financial risk by tightening cash controls using these proven practices.

6. Conduct background checks for EVERY employee who handles money.
  • An attorney colleague is currently working on five cases that involve practice or billing service employees accused of stealing. Background checks should be an essential part of the hiring process. You'll be amazed at how many candidates are up to their ears in credit card debt, or who have been convicted of a crime. Both are clues that the candidate should not be hired to handle money.
  • Don't wait until after the hire to learn the facts. Companies such as Trusted Employees offer inexpensive background checks for employees or hospitals and physician offices, as well as other industries.

RESPONSES/COMMENTS (CLINICAL) - PART 1A
From: Richard A. Simmons, DPM

Bako Pathology has a PCR web space panel test designed to detect 1) Staph aureus, 2) Staph aureus mecA Gene (MRSA), 3) Gram negative bacteria, 4) Dermatophytic fungi, 5) Candida species and Corynebacterium minutissimum. The test is easy to collect specimens using a curette to scoop the suspicious tissue and then place it inside a special cardboard container for transport. The final specimen container is two business cards thick. What I appreciate is the ability to do a broad spectrum screen in one test and a quick turnaround for results.

Disclosure: I have no financial or other connection/association to Bako Pathology.

RESPONSES/COMMENTS (CLINICAL) - PART 1B
From: Robert D Teitelbaum, DPM

I use Castellani paint instead of gentian violet because it is less 'stainful'. It stains but it does clean up with an inordinate amount of isopropyl alcohol. If I suspect the maceration to be a chronic issue, I suggest (and dispense) MacerRx web gel from Darco.

Disclosure: I have no personal or financial relationship to Darco Medical.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Fear from Ciox Audits
From: John Moglia, DPM

I just heard from a Ciox Aetna rep that 3 charts out of 30 "failed" the chart audit. I was informed that Ciox does not pay for a failed chart review. Has anyone else had this experience? What are potential consequences? My position is regardless of pass/fail, the fee for chart retrieval stands with no refund, as the service was provided. My fear is that Aetna will now ask for return of insurance payment for office visits if informed by Ciox.

RESPONSES/COMMENTS (RELEVANT RESAERCH)
From: David Armstrong, DPM

I noticed a summary of a study sent over by Dr. Leonard Levy referencing "low frequency" spinal cord stimulation from Erika Peterson and colleagues at the University of Arkansas. It is actually HIGH frequency (10 kilohertz). This is important because low frequency SCS has been around for 50+ years. The current data from Dr. Peterson and co-workers is therefore rather novel.
