OH Podiatrist Discusses Diabetes and Hypertension
 
According to Jane Graebner, DPM, "If your diabetes is mismanaged, then you might be setting off a  chain reaction of events such as hypertension (high blood pressure), which can lead to heart disease and peripheral neuropathy. In fact, two-thirds of people with type 2 diabetes experience hypertension."

"Hypertension and diabetes have many of the same risk factors, and the Journal of the American College of Cardiology (JACC) in 2015 concluded that having high blood pressure leaves you at greater risk of developing Type 2 diabetes. Similarly, diabetes leaves unprocessed insulin in the bloodstream where it creates blockages in blood vessels (often in your heart) and damages your kidneys, which are vital organs in maintaining healthy blood pressure. This means diabetic patients are twice as likely to develop hypertension as compared to non-diabetics," says Dr. Graebner.

UT Podiatrist/Navy Reserves Rear Admiral Has Best of Both Worlds
 
This Veterans Day had a special meaning for a Millcreek doctor after he reached his forty year anniversary of military service back in August. Mark Moritz, DPM is a rear admiral with the U.S. Navy Reserves and a podiatrist at MountainStar Healthcare’s St. Mark’s Hospital. “I think I have the best of both worlds. I have my private practice and I still get to work for the Navy,” he said.

Dr. Moritz joined the military back in 1981. He has served in Beirut and in the Gulf War as a combat corpsman. The Navy had a need for podiatrists and helped pay for his education. Moritz made his way up to Navy Chief and was promoted as Rear Admiral in 2019.

As Population Declines, Commuting Increases for MO Podiatrist 
 
It has been said that people vote with their feet, but it might be more accurate to say that a viewpoint is expressed with four wheels. Every evening in St. Joseph, vehicles carry a sizable portion of the local workforce back home to suburban Kansas City. It’s a trend that benefits St. Joseph employers that tap into a larger labor pool, but it has an impact on the city’s declining population.

Russell Grimes, DPM, a podiatrist, commutes every morning from Platte City to St. Joseph, where he has an independent practice. “It’s all highway miles,” he said. “It allows me time to set the tone for the day as well as allows me to decompress on the way home.”

1. Establish Your Motivation and Vision - want to work as a solo provider. Is there a financial reason? A desire for better work-life balance? Or do you wish to provide a different type of patient care, perhaps with a more personalized touch?
 
An appreciation of your motivations will help define your mission, which sets the framework for the kind of practice you will build and map out the steps to launch it. Even more importantly, your mission will serve as a grounding point, an ideal upon which you can continually reference to measure whether your practice is fulfilling its purpose.

Query: House Calls and Follow-Up Via Telemedicine
 
Our practice primarily does house calls and I would like to know if we can do follow-up appointments via telemedicine? Due to extremely high demand and influx of new patients for the housecalls in our area (and shortage of doctors willing to do them), we are trying to minimize follow-up visits and use technology to determine if doctor’s presence is needed after the procedure. For example, a post-operative patient sends me pictures of the incision, blister, post-operative site, etc. to a secure website. After reviewing them and speaking with the patient on the phone, I discuss the progress and give recommendations. Would I be able to bill a telemedicine visit if this encounter was outside the global period? What if this telemedicine encounter was not a post-operative follow-up? Examples would be a follow-up for treatment for -plantar fasciitis or Achilles tendonitis.

Response: The answer to your question must be determined by (1) your state law regarding telemedicine services, and (2) your provider agreement with the payer. If the payer is Medicare, then you look at Medicare rules on such services. All of the above must be consulted in order for you to be able to bill for the service.

QUERIES (MEDICAL-LEGAL)
Query: Demand Letter from National Government Services
 
I just received a rather chilling letter from National Government Services saying that there will be a demand for overpayment letter coming from incorrect payments to Medicare Advantage patients in regards to COVID-19 vaccine payments (which we did not do any) but the letter also states there were other payments paid incorrectly. This is for 2020 and 2021. It says not to do anything until the demand letter comes and there is a seminar for improper payments on 12/1 and 12/8. I am asking for advice on what to do.
