TX Podiatrist Pioneers Flatfoot Surgery Without Fusion
Flatfeet can cause lifelong pain and a loss of mobility that can lead to other medical complications like diabetes and heart disease, but a Southeast Texas podiatrist has developed a procedure that corrects the problem - even in kids. If you have flat feet, you've probably been told that not much can be done, but Dr. Mark Bruyn of Victory Hospital has pioneered a surgical procedure to correct this problem.

"Over the past 5 years, we have developed a procedure that corrects flatfeet without fusing the bones, which was the standard of practice when I started out 30 years ago," says Bruyn. He still treats 90% of flatfeet patients conservatively, but says for the 10% of patients who do not respond to treatment, this procedure can have amazing results.

Community Scholarship Named in Honor of Barry Podiatry Student
The highlight of the Citizens Academy event was the honoring of the five non-profits, which include the Reichert House, which received a donation and a certificate of appreciation. The Reichert House is an after-school program that targets elementary- to high school-aged boys. Its mission is to help students transition from boyhood into manhood.

John Alexander, executive director of the program, said the donation will be put to good use. "We are very fortunate to have received such a generous donation, and it will be used to establish the Michael Johnson Educational Scholarship that will be presented for the first time at our graduation ceremony in May," Alexander said. Johnson is a former Reichert House student who is now studying podiatry at Barry University in Miami.

LA Podiatrists Establish Hospital-Based, Outpatient Foot and Ankle Clinic
Dr. Daniel T. Hall, IV and Dr. Mallory M. Przybylski, Co-Founders of Louisiana Foot and Ankle Specialists LLC, have joined Lake Charles Memorial Hospital and Moss Memorial Clinic to establish the first hospital-based, outpatient Foot and Ankle Clinic in Southwest Louisiana. The Foot and Ankle Clinic is designed to focus on diabetic foot education, wound care, limb salvage, and reconstructive foot and ankle surgery to those patients within the Moss Memorial Clinic system in Lake Charles, LA.

Query: Reimbusement for Treating Expert Witness
I treated a patient several years ago following an injury. She is involved in a lawsuit and her attorney contacted me to testify. Since I am the treating doctor, do I get reimbursed for testimony and how do you set your rate? Is there a deposition? What should I expect?

Editor's comment: PM News does not provide legal advice. Expert witnesses are generally reimbursed for time lost in treating patients. This includes time writing reports, attending depositions, and court appearances. Fees are generally determined on an hourly basis. This can be approximated by dividing your gross income by the number of hours worked per year. Thus, if you grossed $350,000 last year and worked 1,000 hours, your hourly gross rate would be $350.

Query: AFO Replacement After 2 Years
I am presently treating a patient who was fitted with an AFO after a CVA around 2 years ago. He was fitted in a hospital after the stroke. The brace is still in a good shape, but the patient's leg is more swollen now and as a result, he developed pressure blister in a few places. This makes him uncomfortable. I tried using padding but it doesn't help. In my opinion, this patient needs to be re-cast for a new device. Would Medicare DME pay for a new brace if it has been less than 5 years since the old one was fabricated? Do I need to send medical records in order to prove my point?

Response: While, yes, it is true that Medicare says the brace should have a lifetime for a period of 5 years, if there is medical necessity (you mentioned that the AFO no longer conforms to the patient's leg anatomy) and if the device cannot be adjusted, prescribe a new one. Have the patient sign an advance beneficiary notice (ABN) because of the 5-year rule. As long as you document why a new device is medically necessary, you should be reimbursed for dispensing a new AFO.

Have no fear! Your patient will run and most likely run with much less pain than before! I have fused distance runners and professional athletes in the past with good results. In fact, for those demanding types of activities, I have come around full circle. As a neophyte, I thought that it was smart to try to save all joints. 25 years of surgical experience has pushed me all the way around that circle. In my hands, fusions tend to last longer and give more predictable results. I suggest changing gait styles of running to shorten stride and perhaps I would add a rocker bottom. I suggest asking Kevin Kirby about those new maximalist shoes, and if there is any alteration to them that a podiatric "biomagician" would recommend in conjunction with arthrodesis.

Our practice went live with Athena in September. As with any EMR, the initial setup is daunting. However, you are assigned an implementation guide with weekly live conference calls to work out any difficulties for three months prior to your go live date. The customer support is the best I have ever worked with.

Get a flexible tool which will enable you to make this transition. An electronic super-bill is the most valuable tool in the hands of treating providers. An ideal super-bill is one which also enables a bridge feature to port over (your) data from the existing EHR and/or practice management and/or billing. Super-bills are typically used to operate an office from day-to-day.

John Francis Durkin also known as “JD” was a sports podiatrist in the suburbs of Chicago, Illinois who recently passed away. He graduated from the University of Illinois, where he ran track and afterward graduated from the Illinois College of Podiatric Medicine. He was Board Certified by the American Board of Podiatric Surgery and was a fellow of the American Academy of Podiatric Sports Medicine.
Dr. Durkin was my mentor, with whom I completed a podiatric sports medicine fellowship. While working under him, we treated great athletes like Sebastian Coe, ...
