NJ Podiatrist 'Who Wants To Be A Millionaire?' Contestant Wins $100K

Having used up all her so-called "lifelines" and faced with the question, "What Saturn moon resembles the infamous 'Death Star' from the movie 'Star Wars?'" Jersey City resident Dr. Denise Bonnin passed up the chance to win $250,000 on ABC-TV's "Who Wants to Be A Millionaire?"

But Bonnin, a podiatrist, hardly left the show empty-handed. She walked away -- happily -- having won $100,000. Her appearance, which was taped at ABC-TV studios in Manhattan on Oct. 25, aired Friday.

OH Podiatrist Warns of Barefoot Running Shoe Dangers

You've seen those trendy shoes, or are they socks? To help you feel like you are running "barefoot." But a new report has a few important warnings for those who choose to wear them. Doctor Raymond Stolarski, a podiatrist at Group Health Associates for Tri-health, explains a warning about barefoot running shoes in a recent report from the American Council on Exercise.

The report says that there are a couple of things to consider when it comes to these minimalist or fivefingers shoes, including technique, how much you weigh, and maybe more importantly whether you have the fit or foot for them. "The basis behind these shoes is that you can increase the blood flow to the forefoot, you can increase your speed with these shoes, and also individuals who use them say you have a better tactile sensation, on the surface you are running on," says Dr. Stolarski.

To determine your foot type: Have a podiatrist examine you, or get an idea yourself by looking at the soles of a pair of worn-in flats. “The wear patterns show where you’re putting pressure when you walk,” says Pack.

RE: Hyaluronic Acid Derivative Injections (Alan Berman, DPM)

I have been using hyaluronic acid fillers for DJD of the 1st MPJ for about seven years with good clinical success. The series of injections are done using interventional radiology. The product used is Suplasyn M.D. 0.7ml X3 over a three-week period.

Bioniche Life Sciences is the manufacturer of the product. The initial study was done by Dr. Robert Petrella in 2004. Dr. Petrella, Associate Professor of Medicine at the University of Western Ontario and Medical Director at the Canadian Centre for Activity and Aging in London, Ontario described the latest findings from two ongoing studies using Suplasyn® to treat osteoarthritis of the knee.

RE: Chronic Wound in Type 1 Diabetic

There are several factors and pathways to treat this patient. First, the wound must be thoroughly debrided down to good healthy tissue. A chronic wound needs to be made an acute wound. Freshen up the wound edges, debride to a 45 degree angle. The obvious also needs to be stated here. The wound does show that there could in fact be an infectious process going on by the extent of the peri-wound erythema. It's tough to tell from the photo.

Start from the ground up: fully and thoroughly debride this wound. Obtain deep tissue for gram stain culture and sensitivity. Obtain TCOM and arterial doppler studies. If warranted, based on the above results, refer to infectious disease, vascular surgery, and HBOT2 for consultation. Based on the above criteria, he may be a candidate for HBOT. Please refer to PCP for blood work as well as HgA1C. Nutrition is key as well.

If arterial studies show a good chance of healing, appropriately off-load the area, start him on a silver calcium alginate or something else, possibly hydrofera blue. If criteria dictate, then tissue replacement may be indicated such as Apligraft, Dermagraft, or Oasis. If this does not help and if ID gets the wound under control, vascular surgery may be indicated. If vascular surgery is in fact indicated, have him revascularized. If vascular states there is adequate blood flow without intervention and tropical/local wound care still fail, perform surgical excision of the offending bone. Remember, ID, vascular, off-loading, HBOT, and good medical and wound care usually heal most wounds, provided there is not advanced microvascular disease.

My sincere condolences to Cheryl, daughter Stacy, son Andrew, Glenn, Bart, and the entire family.

The PQRI findings that Medicare are pushing us to comply with suggest that patients require follow-up exams annually, semi-annually, quarterly, or more often depending on the level of disease that they suffer. Therefore, if you think a patient should be seen more frequently, and they meet the criteria set forth by the PQRI measures, schedule them accordingly. 

I just had a diabetic come for routine care and noticed a gangrenous lesion on one of her toes. With her diminished vision, she thought it was just a bruise and thought it would go away. Had she waited longer, she would have been right; it would have gone away...as well as her forefoot!

RE: Minimalist Shoes May Have Some Benefits: TN Podiatrist

On one hand, Dr. Chatelain basically says he does not have direct experience with minimalist shoes, but then goes on to say, ".....most individuals could use the shoes and not experience any problems, with the exception of people who have vascular diseases or diabetes." Oh, really?

I have to ask Dr. Chatlain on what scientific evidence is he basing his opinion? For so many self-described experts, the story is the same. Oh, minimalist shoes will strengthen the muscles in the foot. Of course, these statements fly in the face of all the reasons. Running shoes have evolved in a manner that has lowered the foot injury rates, allowing so many more people to take up jogging and running, with tremendous health benefits. Yet, now it is a marketable trait to say the opposite! Duh!

RE: Early Diagnosis of Bunions Can Prevent Foot Surgery (Dennis Shavelson, DPM)

Dr. Shavelson addressed concerns regarding the theories proposed by Dr. Merton Root and his colleagues.  In the lectures I attended in the 1980's given by Dr. Root, one of the most interesting statements I heard Dr. Root make on a number of occasions was that he didn't expect his theories to last for more than 10 to 20 years since further scientific research would likely improve on them. Dr. Root’s intention was, I believe, not to teach a gospel of biomechanics that was to be set in stone and left unchanged.

Rather Dr. Root, by creating a new foot classification system, introducing the concept of the subtalar joint neutral position and developing an improved evaluation and orthosis manufacturing process. He was attempting to lay a foundation of knowledge and technical skills so that others who followed him would be able to reach higher levels of understanding regarding the function of the human foot and lower extremity.  My belief is that Dr. Root devoted his life to this cause so that new and improved methods of conservative and surgical mechanical treatments could be developed for patients who suffer from foot and lower extremity pain and dysfunction.
 
I believe we honor the memory of Dr. Root when we also endeavor to improve on the foundation of knowledge that he laid down for all of us. This is what he would expect from us as his podiatric colleagues.
