Barry Finkelstein, DPM recently presented a poster at the Israel Radiology Association annual meeting in conjunction with Montefiore Medical Center and the Albert Einstein College of Medicine. The meeting took place October 28-31 in Eilat, Israel.

KS Podiatrist Honored For Service in 3 Branches, Three Wars
At a luncheon at the VFW following the observance in Sunset Park, 31 Saline County veterans were named to the newly established national Veterans Corps and presented with pins to recognize them for volunteer service. Retired Salina podiatrist Dr. Blase Sibilla was also among those honored for volunteer service. Sibilla, who is in his 80s, said he still volunteers when he feels up to it, delivering Meals on Wheels with his wife, Judy. “I wish I could do more,” he said. Sibilla has a service record that was unique among those being honored, since he served in three branches of the military.

He said he went into the Coast Guard about a year before the end of World War II and de-activated because everybody was going home. He came back to the states and looked for a job, but when he didn’t find one, he joined the Army, serving for 18 months. Then his uncle talked him into joining the Air Force, where he spent the rest of his 21-year military career. Sibilla, a medic, saw action in World War II and the Korean and Vietnam wars. He trained to be a medical evacuator, dressing wounds, evaluating patients, and loading injured people onto helicopters. Many of the patients had been wounded several days before he saw them. He saw head wounds and chest wounds.

Five Firing Tips for Medical Practice Managers- Part 1
Despite all efforts to recruit effectively, a few bad apples will slip through the cracks. They may be chronically tardy, rub patients the wrong way, repeat costly mistakes, or merely fail to meet expectations. Don't let mediocrity slide, says Keith Borglum, a medical practice consultant with Professional Management & Marketing in Santa Rosa, CA. It sends a message to your best and brightest that your standards are subpar, which results in a "why bother" mentality. Instead, address infractions promptly and put disciplinary processes in place that set your team members up to succeed, but culminate in their termination if performance doesn't improve.
• Three strikes. Many practices embrace a progressive disciplinary process, using verbal instruction and feedback first to correct performance shortfalls, followed by written reprimands on the second occurrence, and finally termination. At every step, managers should set measurable goals, invite input from the employee and provide resources to help them develop professionally. They should also document each disciplinary action in the employee's file.

Query: Unable to Dispense a Custom DME Item
Is there a way to charge Medicare for cost of custom fabrication of a DME item that became ready for dispensing just after a patient died?

Response: This is covered in detail in your DME Supplier Manual. Use the Date of the Beneficiary's date of death as the DOS. Send this claim in manually, along with an attached sheet detailing all your expenses to the claims address of your DME. Materials to send include a paper claim and the following: copies of lab invoices, shipping costs, costs of materials used to cast the patient, overhead expenses incurred in casting the patient (this includes your employees' time in setting up the room, fillng out the lab order, utilities, etc.) and a wage for yourself for the time you spent with the patient.
In many cases, the DME MAC will simply pay the fee schedule; in other cases, your costs will be reimbursed and at least you won't lose any money (other than for the time you took to write up the reimbursement request). DO NOT simply send in a claim electronically, even if you think the reimbursements would be the same as though you actually delivered the device.
In case of an audit, a lack of a Written Proof Of Delivery will guarantee you will need to refund the money. Having all your documentation concerning the beneficiary's death and the paper work you've attached will assure you won't be refunding any money (at least not due to a lack of written proof of delivery). For more information, check out the Medicare Supplier's Manual Chapter 5 Section 4 of The Fall 2015 DME Supplier's Manual. The subchapter is entitled: "Artificial Limbs, Braces, and Other Custom-Made Items Ordered but Not Furnished."

RE: Serious Issue With Some Blood Labs and Coding Issues

A few months ago, I went to LabCorp for blood tests ordered by my physician. My insurance company did not pay for all of the tests because the doctor apparently did not code the orders properly. He rewrote the orders and it appears that the tests requested should all be covered. Meanwhile, I went today for more blood tests and I was informed at the lab that they would not draw my bloods because a small bill was still outstanding, and even though it is in appeal, they would not take my bloods unless I paid for the tests by credit card. I picked myself up and simply went to another lab where there was no problem.
I have heard this scenario from some of my patients who also went to Labcorp and had to pay large out-of-pocket bills because either their insurance company failed to recognize the need for a certain test or the coding was not correct and the patients were intimidated by large bills from the lab.
The point is that we all need to be aware of which labs are doing this to our patients and forewarn our patients if they must use those particular labs, or we will risk alienating these patients.

Dr. Zappan, your stock market prediction of a "Dow" 20,000 is very nice to read and even somewhat encouraging. Your investment acumen, however, needs some help. One should not have 100% of their portfolio in the stock market. As a matter of fact, the general rule for percentage of stock investment is 110 minus your age. A more conservative formula would be 100 minus your age. Therefore, a 50 year old podiatrist may have 50% to 60% of his or her portfolio in stocks. In addition, you write about selling stocks in 2017 when the market may drop to 16,000 (your prediction). This is market timing, pure and simple and is faulty and dangerous advice. If you are correctly invested in large cap, mid cap and small cap stocks (to include value, blend and growth styles), you should never sell as you should be holding quality issues.(Do I hear the argument for ETFs?).
In addition, your stock issues should be distributed in proportions similar to the Dow Total Market Index regarding sectors. Therefore, a certain percentage might be in healthcare, a certain percentage in financial stocks, consumer durables, energy, consumer discretionary, etc. No one on Earth has ever been able to time the market for any extended period so "buy and hold" is an intelligent investor's best friend. By the way, the percentage not in stocks resides in fixed income, including percentages of short, intermediate, and long-term bonds (of quality). A small portion of junk high yielding bonds can be included along with CDs, money market, annuities, etc.
