OH Podiatrist Uses HBOT to Heal Diabetic Wound

Hyperbaric oxygen therapy (HBOT) is the most progressive treatment option for wounds that won’t heal. For Robert Lenz, 73, the treatment allowed his diabetic foot ulcer to heal after a string of related health challenges. To help relieve pressure on his foot ulcer, Mr. Lenz began wearing a customized brace. He also had procedures to remove damaged tissue. Then, a decline in health led to him being admitted to the hospital. An MRI test showed he had a bone infection.

Dr. William Scott

“We needed to prevent the wound from worsening to avoid a possible limb- or life-threatening condition,” says William Scott, DPM, a podiatrist at the Lutheran Hospital Wound Healing Center. “As a result, Mr. Lenz underwent surgery to remove his fifth toe and part of the bone underneath,” said Dr. Scott.

APMA COMPONENT NEWS
APMA and AASPM Warn of Dangers of Playing on Injured Feet

Officials at the American Podiatric Medical Association (APMA) and American Academy of Podiatric Sports Medicine (AAPSM) are warning athletes to take care of their feet when they play fall sports like football, and lacrosse. "Many fall sports athletes look to compete their hardest on the field, even after being diagnosed with a foot or ankle injury by a podiatrist or other medical professional," said Dr. Bruce Williams, past president of the AAPSM.

Dr. Bruce Williams

"However, not taking foot injuries such as plantar fasciitis, sprains, and turf toe seriously, and continuing to play a sport on an injured foot or ankle can aggravate and worsen these ailments and delay or significantly worsen the healing process," he said.

INTERNATIONAL PODIATRISTS IN THE NEWS
UAE Podiatrist Cites Shortage of Doctors to Treat Diabetic Feet

There is a desperate lack of podiatrists for patients with diabetic foot – one of the biggest complications associated with diabetes – which can lead to amputation if not treated. Susan Tulley, head of podiatry services at Abu Dhabi’s Healthpoint, said there were only eight full-time and two part-time foot doctors in the Emirate.

Susan Tulley

“The World Health Organization recommends that for this area and for the problem we need at least 200 specialists, and that is for diabetics,”  Tulley said. “You can see the problem. There are not enough of us.” Diabetic foot is often overlooked as a complication of the disease, and there is fear of amputation in having it seen to, she said. “The three main microvascular complications of diabetes are in the eyes, feet, and kidneys, in that order,” said the British expatriate, who has more than 20 years of experience in the Middle East.

QUERIES (CLINICAL)

Query: Distance Running for Patient Post MPJ Fusion?

I have a 52-year-old patient with end-stage arthritis of the first MPJ. I am planning on doing a first MPJ fusion. He likes to run half-marathons. I do have patients on whom I've performed fusions who will run, however I don't have any who are running half marathons. Can some of my colleagues let me know if they have patients who can run that kind of mileage after a first MPJ fusion?

RESPONSES/COMMENTS (CODINGLINE)
From: Ira Baum, DPM

Chronic inflammatory or degenerative changes to the plantar fascia, I think, are a continuum of the process of plantar fasciitis/fasciosis. I understand there have been studies that “prove” plantar fasciitis is not inflammatory but degenerative, but those studies have limitations, particularly based on timing of the pathological evaluation.

Having said that, placing the foot and ankle in an extended position places passive tension on the plantar fascia. As we all know, the fibers of the plantar fascia are oriented longitudinally, extending from heel to its insertions. Chronic inflammation or degeneration of the fibers over time change the...
