Diabetic Wounds Challenge Physicians to Alter Care
 
The U.S. has a diabetic wound problem. “The problem is often that clinicians don’t look at the whole patient; they just look at the hole in the patient,” said Matthew J. Regulski, DPM, director of the Wound Care Institute of Ocean County and wound care podiatry specialist at RWJBarnabas Health, both in Toms River, New Jersey. The solution? Assess and refer.

As time is of the essence, clinical evaluation, pulse assessment, and probing the wound are the extent of the PCP’s initial role in this primary stage, said Richard Sherman, DPM, MBA, a podiatric surgeon in the Division of Vascular Surgery and Endovascular Therapy at Johns Hopkins Hospital, and assistant professor of surgery at Johns Hopkins Medicine, both in Baltimore. He said that he sees patients daily whose wounds have persisted due to inappropriate or ineffective initial treatment.

CA Podiatric Board Weighs Statutory Change for Approval of Residency Programs
 
The Podiatric Medical Board of California on Nov. 7 heard staff say the Board likely does not have statutory authority to delegate annual approval of California podiatric residency programs to a standing committee and asked staff to investigate legislative or regulatory options. Regulatory staff and the executive officer told the Board their legal review suggests delegating approval of residencies to a committee is not currently authorized by statute. 
 
Regulatory staff said doing the delegation by regulation also looks unlikely. "That does leave a couple options," staff said, including continuing current practice or seeking a statutory change to allow committee-level approvals. Staff noted that the Council on Podiatric Medical Education (CPME), the national accreditor, typically finalizes program approvals in an April–May window, which constrains the Board's timing for any earlier approvals.

INTERNATIONAL PODIATRISTS IN THE NEWS
Lack of Regulation Contributing to Podiatrist Shortage in Newfoundland
 
The head of the Newfoundland and Labrador Federation of Podiatric Medicine is calling on the newly formed provincial government to create a regulated college for its members. Currently, there are only 11 podiatrists in Newfoundland and Labrador, all of whom are in the metro region. Dr. Kathryn O’Rielly says that many doctors are completing their schooling in Ontario and not returning home because they cannot practice to their full scope.

She says Newfoundland and Labrador is the only province that does not have its own college. “We are very under-serviced. Most places would have one podiatrist per 10,000 people and right now we have 11 members here in our province practicing,” she told VOCM News. “However, because of the decrease in the scope and practice here, it does not draw any desire for people to leave other provinces such as Ontario.”

I am writing to address the recent disparaging comments made by Dr. Rod Tomczak regarding me. His experience in podiatry is outdated, as he has not practiced in over 40 years and lost his license due to “personal issues”. One must ask what his relevance is. One must also ask if he has ulterior motives like working for a non-designated board in the hopes of making them become more relevant.
 
It is clear that Dr. Tomczak is out of touch with the current state of podiatry, a field that has evolved significantly in recent years. He lists his MD degree like it means something when in fact he cannot use it to see patients. His comments do not reflect the advances and practices that podiatrists are actively engaged in. We as a profession have become independent with our own institutions that are...

Parity or Parody…A little boy is standing outside a sandbox where the big kids are playing. He is distressed and wants to play with the big kids but they have not been paying attention to him for a very long time. He thinks that if only they would let him in the sandbox that he will become one of the gang. As we all know, probably not true. He will never become one of the “big kids”….and if he should happen to be invited in, most likely he will be assigned to the corner of the sand box.
 
At some point, after a lot of whining about his predicament, he turns around and sees a really nice sandbox where all of his friends are having a grand old time. Maybe if he stopped whining about wanting to be in the big kids' sandbox, he would have gained an appreciation for what he already had and found a new appreciation for his...
