Comfortable Shoes are an Investment: CA Podiatrist
How do you define "comfortable shoes," exactly? It's not a sexy answer, but podiatrists say defining a comfortable shoe is totally based on individual preference and the purpose they're trying to serve. "It depends on your foot structure, as well as the purpose for when or how you're going to be using them," says Diane Koshimune, DPM, a podiatrist based in San Jose, CA, and a spokesperson for the American Podiatric Medical Association (APMA).

For example, shoes for plantar fasciitis should have ample arch support (likewise, people with flat feet have fallen arches and need flexible medial posts for stability). People with bunions should look for shoes with a wide toe box and soft, flexible materials so your toes don't feel constricted. "Shoes are an investment," Dr. Koshimune says. "They protect feet from the elements, but they are also the connection between the body and the earth. If the foot has better posture, then the rest of the body will also have better alignment."

Stryker Completes Acquisition of Wright Medical Group
Stryker announced it has completed the previously announced acquisition of Wright Medical Group NV, according to a company press release.
The release noted Stryker completed the acquisition of Wright Medical on Nov. 11, 2020, following the completion of the cash tender offer for all outstanding ordinary shares of Wright Medical by Stryker BV. The acquisition consisted of a series of mergers implemented pursuant to the agreement and plan or merger by and among Stryker, Stryker BV, Wright Medical, Wright Luxembourg SA, Wright Medical Ltd., and Stryker Unite Ltd.

Front-desk embezzlement schemes are not uncommon and they are typically the result of loose protocols and lack of oversight. Reduce your financial risk by tightening cash controls using these proven practices.
5. Randomly audit no-shows and cancelled appointments to make sure they are valid.

In one surgical practice, we uncovered a front-desk staffer deleting appointments using the reason code "no-show" for patients who were seen and paid in cash. How did we catch it? An inordinate number of no-shows prompted us to pull charts.

In several, we found visit notes for patients who supposedly did not show up for their appointment when they actually did. Not only had the practice lost the patient's payment, but because the staff person had made these encounters disappear, none or these services had been billed to insurance either.

Query: Non-Skilled Nursing Homes
Can I bill an E/M office visit code for a patient seen in a non-skilled nursing home setting?

Response: Yes, you can. The place of service would be 32. The rules for when you can bill a nursing home visit are no different than when seeing your patient in a skilled nursing facility (SNF) or office. You have to document history, exam, and decision-making and choose the appropriate level of service based upon that documentation. If billing a procedure at that same encounter, then the E/M has to be “significant and separately identifiable” from the procedure performed, just like in the office setting.

Encouraging results for painful diabetic neuropathy patients using low-frequency spinal cord stimulation (SCS) and who are refractory to conventional care were reported by Erika Petersen, MD, of the University of Arkansas for Medical Sciences in Fayetteville, and colleagues. There was a significant difference between the treatment groups with respect to achieving ≥50 percent pain relief and without a worsening baseline neurological deficit. At a three-month follow-up, there were also differences noted in lower-limb pain scores, responder rates, and investigator-assessed sensory improvements. You should take a C&S. I have found on several occasions when antifungals agents aren't working, it is usually an underlying bacterial infection. Erythromycin lotion twice daily for a couple of weeks seems to help.

We have had similar cases over the years. For starters, you need to rule out a bacterial infection and treat it accordingly. After that, separate the toes with strips of gauze and teach the patient to do the same daily. One medication that we have used with great success is Formula 7 Gel. It’s a bit pricey but does the job.

Over many years of practice, I have had similar situations a few times. If a C&S shows no growth for fungi, yeast, or bacteria, I have recommended to the patient to buy a spray can of Arrid Extra Dry antiperspirant and spray a small amount between the toes at bedtime and in the morning after shower. This has worked several times.

Sorry, Dr. Samuel, but I reject this statement: "The Council allowed programs to add more residency spots or gave new program credentials that may not be ready to provide the diversity and complexity of a solid training program." You offered no proof.
With that said, I agree with you that the last thing this profession needs is another college of podiatric medicine. It can be anticipated that hospitals currently hosting programs will close, creating again a shortage of residency slots. Another college will make things worse.
