OH Podiatrist Discusses Diabetic Peripheral Neuropathy
According to podiatrist Thomas Vail, DPM, "Neuropathy can be a rather scary aspect of diabetes because patients may not be able to feel pain. If you can’t feel an injury or sore, it could lead to a serious infection. People with diabetes have a harder time healing from infections, and even a minor sore or blister could ultimately lead to amputation. It’s important to try to prevent nerve damage before it happens and to take extra precautions if you do experience symptoms."

"I recommend patients with peripheral neuropathy never go barefoot because of the risk of injuries. People with peripheral neuropathy should see a podiatrist regularly to help catch any changes in their foot health early," says Dr. Vail.

CA Podiatrist Keynotes at Swiss Limb Preservation Symposium
Dr. David G. Armstrong, Professor of Surgery and Director of the Southwestern Academic Limb Salvage Alliance (SALSA) at Keck School of Medicine of University of Southern California (USC) became the first podiatric surgeon invited to keynote the 8th Balgrist Symposium on the Diabetic Foot. His lecture spanned the history and future of surgical, medical, and mechanical management of the high-risk foot and limb preservation.

Delegates from Switzerland and across Europe participated in the biennial event. The symposium also included a visiting professor program in the Balgrist's Department of Orthopaedic Surgery, where Armstrong presented to assembled physicians, surgeons, and trainees.

PRESENT e-Learning Systems Runs its 50th Conference
PRESENT e-Learning Systems (PRESENT) has just run its 50th CME medical conference for podiatrists. The company, founded by podiatrists Alan Sherman, DPM and Michael Shore, DPM to deliver a standardize online lecture education program for podiatric residents in 2003 began doing live conferences in 2007 and has grown into one of podiatry’s largest education providers. PRESENT holds Superbones Superwounds West and East, Superbones Superwounds East Residency Education Summit Midwest and East events, and Desert Foot. PRESENT ran its first PRESENT Treasure Hunt Conference for the Los Angeles County Podiatric Medical Association this year.

Along the way, PRESENT became a CPME-accredited CME provider and one of the largest providers of online CME to podiatrists. PRESENT provides an online “Lecture Hall” of over 300 current CME lectures available to podiatrists all over the world. Risk of Second Melanoma Following Cutaneous Melanoma Diagnosis
This retrospective analysis of the SEER database found that patients with prior cutaneous melanoma (N=169,841) had greater odds of developing a second primary cutaneous melanoma (standardized incidence ratio[SIR], 8.17), ocular melanoma (SIR, 1.99), oral melanoma (SIR, 6.87), and vaginal/exocervical melanoma (SIR, 10.17) than the general population.
These findings suggest that, in addition to monitoring for second primary cutaneous melanomas, physicians should monitor patients with a history of a primary cutaneous melanoma for non-cutaneous melanomas.

Query: Bundling of Forefoot Surgery
Our doctor performed surgery on the right foot that included a Lapidus bunionectomy with a 2nd hammertoe arthroplasty, a 2nd metatarsal head osteotomy and capsulotomy of the 2nd metatarsophalgeal joint. I have billed this case as follows:
CPT 28297 RT, CPT 28308 RT/59, CPT 28270 RT/59, CPT 28285 59/T6
Insurance is stating the CPT 28308 and CPT 28270 as inclusive codes and not reimbursable. The tenotomy and capsulotomy were separate incisions and on separate toes from the bunion.

Response: First off, separate incisions is not a criteria for payment of any surgery. In order to get to the bone to do the metatarsal osteotomy, you had to incise the capsule and then repair it (possibly some minor capsular work performed as well) as part of your closure. So the two procedures can be bundled together. The question is whether the work you did at the joint was more than expected/average. This goes back to your documentation. If you felt the capsular work was above and beyond the norm, then you need to file an appeal documenting why you think it should be reimbursed separately.

I received an email message yesterday from Humana Military, who handle the claims for the military we see locally. They said under no circumstance should we advise CBD products to the active military. They may test positive for a banned substance and would be subject to disciplinary and legal action. This is something we need to be aware of if we treat active military members.

I have been using CBD oil in my practice for about 3.5 years now with excellent results. Utilizing CBD systemically is the best bet at managing your patient’s pain (along with a topical CBD liniment to relieve spot areas of pain). The wonderful thing about CBD is that it’s stoichiometry is strikingly similar to endogenous endocannabinoids (endorphins and neurotransmitters) that bind to receptor sites in the human body that open up the pathways of repair, detoxification, replacement, and replenishment (effectively supporting homeostasis).
This is why it relieves so many symptoms in addition to pain. It’s important to note that all CBD is not created equal. There happens to be a lot of garbage out there in the marketplace. One must research the product thoroughly. I utilize products from Canbiola. Their CBD is a combination of pure, organic CBD isolate with full spectrum terpenes. It is prepared via sound wave nanotechnology for maximum sub-lingual absorption and a predictable dose. Just be aware that CBD is metabolized by Cytochrome P450 and because it is natural, will displace certain medications thereby increasing their half-life with the potential for drug side-effects. I have yet to experience this but definitely use with caution with new generation anti-coagulants.
