VT Podiatrist Explains How Othotics Improve Back Pain
 
To scholars of evolution, it isn’t necessarily surprising that our backs hurt. After all, we are the only known animal to stand upright, and our backs are responsible for maintaining our posture against the ceaseless force of gravity. Yet, what is surprising is that the origins of lower back pain may have less to do with our backs, and more with our feet. Dr. Howard Dananberg, a world-renowned podiatrist, believes orthotics may be our best bet for bucking back pain. The average individual walks anywhere from 2,500 to 15,000 steps a day, which can make for good exercise. However, those who have issues with their feet, such as flat-footedness or high arches, also tend to change their gait to compensate, which surprisingly, applies the most stress to the muscles of the lower back.

In fact, it’s not the impact (as is common in running injuries), that causes the problem, but the actual swinging of the leg that strains the musculature. Dr. Dananberg and his colleagues found that for 84% of those with end-stage (i.e. “incurable”) lower-back pain, shoe orthotics could improve or relieve their pain. Dananberg also found that those with orthotics extended their hips almost 50% more, which meant less pressure on the muscles of their lower back (and 50% less pain).

MA Podiatrist Recognized for 30 years of volunteering at the Boston Marathon
 
Jonathan Kaplan, DPM of Wellesley, MA was honored by the Boston Athletic Association (BAA) for thirty years of service volunteering at the Boston Marathon. Twenty other volunteers were recognized for their service at a luncheon and received a medal.

Dr. Kaplan has been chief of the podiatry team for the past ten years, managing his podiatry team of fifty to sixty doctors, residents, and podiatry students. He started volunteering in the medical tent at the finish line for the Boston Marathon when he was a podiatry student at the New York College of Podiatric Medicine.

Wearing Bad Shoes Is Like Having Too Much Dessert: NY Podiatrist
 
According to a new British survey conducted by the College of Podiatry, it only takes one hour and six minutes of wearing uncomfortable shoes to feel pain, with ailments ranging from blisters to muscular problems. And heels are not the only culprit; even flip-flops can increase your risk for stress fractures. Dr. Hillary Brenner, a podiatric surgeon in NYC and spokesperson for the American Podiatric Medical Association (APMA) said, wearing shoes is like dieting, adding that you have to count the hours you wear them.

Desert Foot 2018 Diabetic Limb Salvage Conference Supports New Technology
 
The Desert Foot 2018 Multi-Disciplinary Limb Salvage Conference convened in Phoenix again this year, focusing on new technology for the care and preservation of the diabetic lower extremity. The event comprised 4 days of cutting edge lectures, 17 hands-on workshops, and an abundance of industry events. Conference Co-Chairman Charles Andersen, MD, Lawrence Lavery, DPM, MPH and Lee Rogers, DPM led an esteemed faculty of 52 clinicians and researchers, educating hundreds of clinicians from around the country. Derek Hughes of 3M was honored as the Grand Sponsor of PRESENT Education for 2018, whose generous support allows PRESENT e-Learning Systems to deliver the PRESENT Residency Education Online weekly curriculum to hundreds more podiatric residents across the country.

The winners of the Desert Foot 2018 Abstract/Poster Competition were Jaminelli Banks, DPM for her study, “Use of Remote Temperature Monitoring to Identify Pre-Ulcerative Lesions for DFU Prevention and Early Detection: A Case Series of Four Patients” in the Resident category, and Paul Savage, DPM for his study, “Evaluation of ceragenin CSA-44 for acceleration of healing of chronic wounds associated with type 2 diabetes” in the Resident category. The winner of the 2018 Paul Brand Award for excellence in limb salvage leadership is esteemed general surgeon Fermin Rafael Martinez De Jesus, MD of Mexico City, Mexico. Next year’s Desert Foot Conference will take place Dec 4-7, 2019 at the Sheraton Grand Phoenix, Phoenix, AZ.

Digital Radiology Saves Money and Saves Lives: CA Podiatrist
 
With the right technology, clinicians at Adventist Health White Memorial's (AHWM) Center for Limb Preservation are saving the organization money while saving patients’ limbs. The team sees on average of 40 to 60 patients a day. The goal: catch complications early and avoid amputations. The team has a dedicated FDR D-EVO Suite II on the premises. Charles Ananian, DPM., staff podiatrist, Center for Limb Preservation and Advanced Wound Care, AHWM, said the sophisticated, world-class technology is a key contributor to the rapid diagnosis and treatment of patients and better overall outcomes.

“Normally what would happen is we send them into radiology maybe tomorrow and see them maybe the next day and by then, the problem could have compounded and become worse,” said Dr. Ananian. But with the FDR D-EVO Suite II onsite, “Now I see them at 4 o’clock, we send them right down the hall to get an x-ray here, I have the image in a few minutes, and I get to come up with a treatment plan for that patient in real time.”

CMS Finalizes 2019 Rules - Part 2

2. CMS has added an additional low-volume threshold exemption to MIPS for next year. To be excluded, providers or groups need to meet at least one of the following conditions:
 
• Have $90,000 or less in Medicare Part B allowed charges for covered professional services.
• Provide care to 200 or fewer Part B-enrolled patients.
• Provide 200 or fewer covered professional services under the PFS.
 
The minimum period for each performance category remains unchanged, so quality and cost stay at 12 months while improvement activities and promoting interoperability remain at a continuous 90-day period.

The lack of studies on this subject may be due, in part, to it being classified as a “rare” disease.  I am not sure if it is really that rare. Diagnosis codes may be used to track disease entities and there is no unique code for Ledderhose disease.
 
I have been using serial ultrasound-guided injections of hyaluronidase mixed with triamcinolone acetonide for about 20 years with very good results. I would be happy to share my protocol with any interested podiatrist.

RE: Dealing with Pay Reductions for Multiple Procedures

There is actually a CMS rule that addresses performing procedures on multiple days to increase reimbursement. It is in Chapter I (Page 8) General Correct Coding Policies for National Correct Coding Initiative Policy Manual for Medicare Services. “Physicians shall not inconvenience beneficiaries nor increase risks to beneficiaries by performing services on different dates of service to avoid MUE or NCCI PTP edits”. 
 
Even though this is CMS/Medicare policy, many insurance companies utilize these guidelines.

The monumental steps Dr. Dixon made to advance our profession are remarkable. On several occasions, I had the opportunity to spend time with her, and she never once spoke of any challenges, only positively of her career and life. I was in awe to see the tribute she was given at her 100th birthday celebration in her home town in Alabama. This event was sponsored by National Park Services for which she dedicated much of her time. Kudos to her daughter Janet who made it possible for Dr. Dixon to travel and be at many national podiatry events in her later years. For all the younger podiatrists reading this, please reflect on those such as Mildred, who sacrificed to build a strong and solid foundation for our wonderful profession.
