MI Podiatrist Enpowers Diabetic Patients Through Education

Diabetes is the leading cause of non-traumatic amputations in the U.S., resulting in about 84,000 lower limb amputations each year and it's almost completely preventable. At Dr. Crystal Holmes' podiatric practice in the University of Michigan Health System, she treats and helps prevent diabetes-related foot complications, such as diabetic neuropathy, or nerve damage, wounds, and infections. Education and early intervention are critical.

Proper athletic wear is a lesson the people who are part of "Walkable Wilson" are learning as they continue their nine weeks to better health. There are about 55 people enrolled in the nine week program. Proper footwear can make or break walking as a healthy habit.

Dr. Kendall Blackwell, podiatrist, says about 30 percent of all of his patients come to him with ill-fitted shoes. Blackwell analyzes his patients' walk and sends them to certain athletic shoe stores to get a properly fitted shoe for walking or any other exercise they may want to do. Pronation is what Blackwell is looking for in his patients.

Retired Podiatrist Authors Book Based on True Life Tragedy

A Father’s Anguish” (published by iUniverse) was inspired by the rape of author Raymond Doyen’s daughter and the search for her abuser. While the police search for a serial rapist, so too does the victim’s father. Growing frustrated, the father finds his prey and kills him. The police must now switch gears from hunting a rapist to finding a killer. The detectives have mixed feelings about Stratton — they too have daughters. Doyen’s novel explores the psychology of abusers, of their victims, and of the family of victims. It examines sexual assaults, obsessive behavior and family love. “A Father’s Anguish” brings awareness to the crime of rape, the ordeal that these women must endure, and the legal complexities that do not always support the victim.

QUERIES (NON-CLINICAL)

Query: Whitewash Check Fraud

I am curious to know if anyone on this forum has experienced whitewash check fraud. For years, I have used Online Update and Reconciliation for my checking accounts. A few weeks ago I did an update to find approximately ten checks had been paid from my business account all slightly under three hundred dollars. I immediately went to the bank, closed the account, and opened a new account. Account numbers then had to be changed at EFTPS and PayCycle. Fortunately, the bank reimbursed the money the next day.

For years, I have used a combination of Online BillPay through the bank and standard voucher paper checks. The checks were managed and printed with cash management software I have used since the eighties. The perpetrators had apparently scanned one of my paper checks made out to one of my payees. The checks that cleared the bank looked exactly like one of my paper checks including my exact signature. The only difference was that the bad check did not contain my telephone number. The checks were all cashed in the Ft. Worth area.

I have many business payees scattered throughout the country and there is no way of knowing how the check was altered or who did it. The checks that cleared the bank all had duplicate numbers in my check register made out to existing but different payees. If the bad guys printed and passed ten checks, then they could have printed hundreds. Since this happened, I have only written two paper checks. Payees are now paid through the bank BillPay system which does not show my account number. Those interested might check this website.

RE: Post-op Fracture Blister (Tip Sullivan, DPM)

When you are performing an arthrodesis, when there is good bone to bone aposition, "fracture" blisters are rare but not impossible. A "necrotic" eschar has more to do with the handling of tissues, incision placement, and/or capillary perfusion of the surgical site. In a 60 yo, apparently healthy patient, even with good peripheral circulation of the major pedal vessels, capillary insult will most likely occur because of thinning of her dermis due to age.

I would strongly advice against a contralateral surgical correction while the wound is still present, more so due to the fact that sufficient time has not been allowed for the fusion sites to heal completely.

Based on the appearance of the wound with eschar and fibrous tissue present in the wound, this looks ischemic to me, I do not see any granulation tissue present. I would recommend non-invasive Doppler studies to evaluate perfusion and possible vascular consult before I considered operating on the other side.

RE: Painful Callus, Unresponsive to Conservative Care  (Megan Lawton, DPM)

I believe Dr. Lawton was looking for guidance for a neurolytic type treatment for these lesions.  About every five years, I have a patient with similar lesion(s). They are excruciatingly painful on minimal palpation. Is this the 'neurovascular corn' we learned about years ago, one that somehow has gathered a nerve ending into it and is unrelentless?  Is this a 'porokeratosis'?  What it is, is rare and a challenge to the doctor.

What I have offered the patients in the past, with 75% success rate, is a '"changectomy", where I excise the lesion, with minimal sutures.  Yes, a lesion does return because the osseous and biomechanical factors will reassert themselves. But the result is a great improvement in the level of tenderness. I was planning on performing  the neurolysis  injection on my last patient with these lesions, but he had significant co-morbidities that precluded this treatment.

I understand fully the concerns on the scope of practice issue, poor reimbursement and the burden of increased paper work - all road-blocks for change.

In my opinion, these concerns are unfortunately...

RE: Edwin Probber, DPM

The passing of Ed Probber is a tremendous loss for the podiatry profession. Dr. Probber was a truly gifted podiatric surgeon and teacher of modern percutaneous minimally invasive surgical techniques. Dr. Probber loved curing his patients' deformities by operating through tiny punctures without the need for any lengthy incisions, sutures, or internal fixation devices. He was truly happy to see his MIS techniques finally being adopted by nearly every other surgical specialty. Ed, we will miss you.

It is with sadness that I join my collages in mourning the loss of  Edwin Probber, DPM. He was a visionary and his genius and wit were unsurpassed. His masterful surgical expertise and purist innovations, including publishing Standards of Care, are a lasting legacy in patient care. He taught thousands of podiatrists and singlehandedly changed the face of modern podiatry. His impact on our profession is immeasurable and not yet fully realized.

He walked tall as a proud podiatrist and advocated that we follow the professional model of dentistry. As founder of the AAFAS, he tirelessly promulgated minimal invasive podiatric techniques and procedures. I'm grateful, like so many podiatrists, for having been his student and friend. May peace and love surround his loving wife Ellen and children Barry, Pam and Scott.

Joanie and I were very saddened with the news that Ed passed away. Not only did Ed Probber teach thousands of podiatrists minimal incision surgery techniques, but he taught the "philosophy of practice," and instilled personalities in every podiatrist who took his course. Each person chose how they did surgery, but all of them learned lessons of practice that they never could learn from any other podiatrist.
 
In my opinion, he was "The Father of Office-Based Surgery." Ed was not only a very down-to-earth person, but one of the funniest people that I ever had the pleasure of knowing. Ed will be missed by many, but personally, I will always have a place in my heart for Ed Probber. He molded my beliefs and enhanced my entire practice life. Thanks Ed for giving me everything you had.
