VA Podiatrist Observes Kristallnacht Remembrance Ceremony
 
Limor Glazer Schwam, DPM is a symbol of the American dream. For three decades, she has been an active Richmonder as a podiatrist, a teacher of podiatric surgery, and a member of numerous professional boards and efforts in the Jewish community. She and her husband, Gary, have raised two daughters and succeeded through hard work — and remembering where they came from. On Sunday, Dr. Glazer Schwam stood before a socially distant crowd of roughly 50 people at the Emek Sholom Holocaust Memorial Cemetery in Henrico County. On the anniversary of Kristallnacht — Nov 9-10, 1938; the night of broken glass and a harbinger of worse years to come — she shared how the Holocaust tore apart both sides of her family. Her courage to tell her story is a reminder of the need to recall, remember, and act.

“What is our part?” Dr. Glazer Schwam said. “To keep their stories alive; invest in Jewish historical education; behave and live as if my family members are your family members because they are.” At Auschwitz, seven pages of “Glazer” names are among the dead. Glazer Schwam’s parents escaped the persecution that killed 6 million Jews during the Holocaust.

"Hybrid” Schuster Biomechanics Seminar Draws Record Attendance 
 
NYCPM’s eighth annual Richard O. Schuster, DPM, Memorial Biomechanics Seminar was offered on November 7th and 8th as both an on-site conference and online, and most of the record 150 attendees chose to attend virtually. A majority of the presentations were made virtually as well, by notable biomechanics experts located across the country and internationally; six speakers chose to present at NYCPM. The seminar was coordinated and chaired by Joseph C. D’Amico, DPM, a professor and past chair of the department of orthopedics and biomechanics at NYCPM, and was made possible by a significant contribution from a grateful patient of Dr. D’Amico who was cured of his persistent heel pain.

Dr. Jinwook Song from Korea presented a retrospective study of almost 500 cases of internal tibial torsion that were treated conservatively, entitled “It’s Time to see Internal Tibial Torsion from a New Perspective: a Conservative Treatment of 482 Patients with ITT.”  Dr. Song noted an extremely high success rate with a new AFO-type transverse plane bracing device that neutralizes muscle imbalances from the genicular region distally. His conclusion was that ITT may not be the reason for in-toeing gait in children or adults diagnosed with ITT, but rather a “pseudo-lack of malleolar torsion” with the responsible etiology lying in muscular imbalance and can be very successfully treated.

PRACTICE MANAGEMENT TIP OF THE DAY
Reducing Medical Practice Embezzlement Risk with Cash Controls - Part 4
 
Front-desk embezzlement schemes are not uncommon and they are typically the result of loose protocols and lack of oversight. Reduce your financial risk by tightening cash controls using these proven practices.
 
1. Separate the "change fund" from "petty cash." There is a difference.
  • The change fund is an amount in small bills that's always the same - say, $200. This is the money staff use to make change for patients who pay in cash. Every day when they balance to the penny, the amount is counted out and kept in the drawer for change-making the next day.
  • The petty cash fund is a small account from which you borrow for small purchases. Each transaction is logged on a "chit", and ultimately posted as an expense in your bookkeeping system. When the petty cash fund is low, the manager replenishes it, and records this in the bookkeeping system too. If you don't track petty cash separately from change, you risk an easy-to-play financial shell game that never matches money spent with specific transactions. And it's difficult to spot missing money if there are no controls for reconciling transactions in the first place.

QUERIES (CLINICAL)
Query: Treatment of Interdigital Maceration
 
I have a few patients with excessively macerated skin between their toes - usually seen in the 3rd and 4th interspaces. There is typically no evidence of tinea infection. This appears to be caused by medial contraction of toes, which in turn causes very tight interspaces to become chronically moist. 
 
I have tried toe spacers, lamb's wool, etc. with some limited success. I have also tried gentian violet in years past, but it's very messy & stains. I am curious what my colleagues are doing to treat chronically macerated interspaces?

RE: Therapeutic Shoe Breaking News

CGS, the JB and JC DMEPOS Contractor, announced on November 5, 2020 a long-awaited update to the Therapeutic Shoe Policy. This update, would for the first time, permit the documentation of a nurse practitioner or physicians assistant (NP/PA) working under the “incidence to” rule. But this is subject to some restrictions. It is not clear how soon the LCD itself will be updated. Auditors at various CMS agencies are currently being educated on this matter now and that expects to be completed in about six weeks. How this new update will affect claims submitted with a DOS prior to November 5, 2020 is unclear at this time. Noridian (DME MAC A and D) has not yet posted this on their website, but since the policies are equally enforced, there is no doubt the posting from Noridian will be  forthcoming soon.
 
I cannot stress enough that for now in states where NP and PA can work independently and bill under their own NPI, this ruling may have little effect and, in fact, create more confusion. This update precludes the inclusion of documentation of an NP or PA working independently. In states where NP or PA can work independently, it is advisable to check with the NP or PA to ensure that their documentation fits the incident to guidelines.

RE: MIPS and the COVID-19 Exception

With some research, I learned that there is now an "Extreme and Uncontrollable Circumstances Exception" for 2020. Using COVID-19 as the circumstance (prevented collecting data and impacted performance/cost measures), this will allow us to claim an exemption from all 4 categories (Quality, IA, PI, and Cost) for MIPS 2020. This will result in a neutral payment adjustment for 2022. This is no small exception since a neutral adjustment this year requires 45 points (up from 30). Realistically, it will take 85 points to see any positive adjustment.

I just read that another college is being opened. I am not sure how to respond to this. A few things are going through my head like, “Are you kidding me” or “seriously?” Just a few years ago, we had a residency crisis. Now, it is reported that we have plenty of residencies. How did that happen? The Council allowed programs to add more residency spots or gave new program credentials that may not be ready to provide the diversity and complexity of a solid training program. This was just to get the graduates placed and avoid legal issues while ‘selling’ a profession, only to not have needed post-grad training to actually practice?
 
We don’t have uniform training in general and from what I hear and observe, adding more residents to existing programs limits the hands-on time and overall case load for the residents. The other issue that is quite clear, having been involved with training residents for close to 30 years is that, not everyone is going to be a...
