NY Podiatrist Discusses Stress Fractures
 
Repeated stress (like the continuous pounding of running) can cause a tiny break in the bone, also known as a stress fracture. “The metatarsal bones, which make up the front part of the arch, are a common area for stress fractures with the second and third metatarsals being affected most often,” says Loretta Logan, DPM, an associate professor at the New York College of Podiatric Medicine & Foot Center of New York. “The pain is typically felt at a specific and localized spot, usually on the top portion of the bone.” Sadly, your stress fracture might not be obvious at first, which puts you at risk for making it worse: “The pain may be mild at first, but it can intensify with time if you don’t take care of it.”

“Stress fractures take about six to eight weeks to heal and are routinely treated with rest, ice, compression, and elevation,” Logan says. Rest is key here: “Ice and NSAID medication may assist in early days, but the fastest pathway to healing is through relative off-loading of the fracture site.” To be clear, that means no running until your doc says so. Some doctors may even prescribe a walking boot to help take some load off the affected area. The stakes are high: A stress fracture can turn into a fracture-fracture (or full break) if you aren’t careful.

NH Podiatrist’s Annual Shoe, Boot, and Sock Drive Begins
 
After collecting more than 420 pairs of footwear during her annual drive last year, a local podiatrist is hoping to collect even more shoes, boots and socks this year to help homeless individuals and others in need. Billie Bondar, DPM has started collecting items through her annual holiday drive. Bondar said she greatly appreciates community members helping other people in need each year, as she said she believes that level of human-to-human support is an admirable and defining trait of New England.

“It is so important to get this going early for the donations because the temps are dropping, and providing footwear for the winter season ahead needs to be accomplished now,” said Dr. Bondar.

MACRA UPDATE
Vast Majority of Docs will get a Pay Bump under MIPS—But it Won't be Much
 
Most doctors and other clinicians did okay in the first year of the Merit-based Incentive Payment System (MIPS). More than 90% of the doctors and clinicians who participated in MIPS will receive a pay bump based on their performance in 2017, said CMS administrator Seema Verma in a blog post about the program implemented under MACRA in which clinicians receive either a positive, negative, or neutral payment adjustment based on their performance. However, it won't be much.
 
Even those with the highest performance scores only earned a 1.88% positive adjustment. That left many physicians disheartened. Overall, 95% of participants avoided a negative payment adjustment, Verma said. That leaves only 5% of participants with a negative 4% payment adjustment. 71% earned a positive adjustment and an adjustment for exceptional performance and received a 1.88% adjustment, 22% earned a positive payment adjustment only and received a .20% adjustment, and 2% received a neutral adjustment (no increase or decrease).

Individuals’ Use of Online Medical Records is on the Rise
 
It has taken years, but healthcare consumers are now becoming comfortable with the notion of accessing their medical records online. In fact, the use of portals and other mechanisms to access medical records is rising, according to the Office of the National Coordinator for Health Information Technology. And the pressure will rise in coming years, with the 21st Century Cures Act including provisions intended to improve patients’ access to and use of their electronic health information.
 
More than half of individuals have been offered access to records. Researchers from ONC’s Office of Technology—Vaishali Patel and Christian Johnson—say individuals’ access to online medical records increased by 24 percent between 2014 and 2017.

Dr. Mildred K. Dixon was one of the most decent human beings ever. She was a "mensch", honest, and gracious. She was one of a kind who dedicated her life to caring for veterans. I know that she was recognized by the Department of Veterans Affairs in the past for her efforts. She was a bright star for our profession. 
 
She will be missed. May she rest in peace.

Our profession lost a real champion for podiatric medicine when we lost Dr. Mildred K Dixon.  I have known her my entire career in podiatry as just a sincere and friendly person who would always be there to help anyone in need. She will always be a special person and one to remember. In her days in practice, she overcame many racial issues that she confronted, particularly in her community during those early years of practice. We should all have her energy for the good of the profession. I want to extend my deepest sympathy to her loving family whom I have also met on several occasions.

Injectable collagenase may be helpful for plantar fibromatosis, although neither robust nor long-term studies are available for reference. My personal approach is as follows:
 
1. Confirm the diagnosis with MRI, ultrasound, or percutaneous needle biopsy prior to treatment;
2. Daily BID application of a compounded mixture of 5% lidocaine, 5% diclofenac, and 15% verapamil (EBM);
3. Stretching of the plantar fascia (night splint and/or active stretch);
4. Avoidance of hard insoles or rigid orthotics. 
 
My experience (for many years) with the utilization of this technique for non-aggressive type fibromatosis has been nothing short of excellent.

I’m taking an opposing position than most of the previous posts. I think there is a place for altruism, but I believe podiatrists must be pragmatic. I don’t think that the decision to perform multiple procedures in one session based on one variable is appropriate. Podiatrists and healthcare providers dependent on third-party payors have been financially battered with no real potential recourse. So, if patients depend on a third-party paying for their care, then by implication, they should also expect the care their payors are willing to pay.
 
Podiatrists have overhead responsibilities. If those obligations can’t be met, then I don’t see how this benefits anyone. I am not a proponent of putting patients at increased risk, but I am also sensitive to the financial predicament put on our providers.
