CA Residents Volunteer at Diabetes Seminar
The Scripps Podiatric Surgical Residency team in San Diego, California volunteered at the Taking Control of Your Diabetes (TCOYD) seminar this past weekend. The seminar is in its 20th year, and its purpose is to heighten awareness of diabetes and provide tools for those affected to better manage it.

Foot screenings were performed for approximately 100 Type 1 and Type 2 diabetic individuals. Participating were Drs. Derek Smith, Ben Cullen, Michael Crawford, Ryan Lee, Bruce Hutchinson, Alayna Puccinelli, Troy Moghadam, Pamela Hong, and Brittany Rice.

Bako Solutions to Sponsor National Park Day Event
The American College of Foot & Ankle Pediatrics (ACFAP) has announced that Bako Integrated Physician Solutions has agreed to provide an unrestricted educational grant in support of the 1st Annual ACFAP Podopediatric Seminar. Bako’s grant will be used in part to support a group outing to the Everglades National Park, a “National Park Day” that will become a tradition at each annual meeting. ACFAP President, Dr. Louis J. DeCaro, hopes that the event will not only provide attendees with a unique national park experience each year, but at the same time will create an “invigorating forum” for getting together and sharing podopediatric knowledge.

The 1st Annual ACFAP Seminar will take place March 27-28, 2015. The CME event on Saturday, March 28th will take place at Barry University School of Podiatric Medicine and will feature leading authorities on pediatric foot and ankle conditions. It will cover topics ranging from pediatric flatfoot, equinus, sports medicine, surgery, pathology, and practice management.

Regular and Vigilant Foot Care Can Reduce Amputations: APMA President
A diabetes diagnosis can be daunting, but a simple attitude adjustment can make a world of difference in how well you fare while living with the disease. By proactively taking steps to monitor key health indicators, experts agree that it’s possible to prevent some of the most severe risks of diabetes, including lower limb amputation. “The CDC says the occurrence of diabetes-related foot and lower-leg amputation has decreased by 65 percent since 1996,” says American Podiatric Medical Association (APMA) President Dr. Frank Spinosa. “Working together, podiatrists and their patients with diabetes can reduce the number of amputations even more.

People with diabetes may be less aware of cuts or wounds on their feet due to the nerve damage related to their disease, Spinosa points out. “Regular and vigilant foot care can help catch problems before they develop into a health crisis.”

MEETING NEWS |
450 Attend Goldfarb Clinical Conference
The 42nd Annual Goldfarb Clinical Conference took place November 6-9 in Valley Forge, PA. The theme of the conference this year was "collaborative care", with an emphasis on diversity among the speakers.

In addition to many podiatrists, the faculty included a fellowship-trained hyperbaric medicine doctor, a doctor of physical therapy, a yoga instructor, a naturopathic doctor, and a vascular surgeon. There were 450 attendees and many exhibitors. The conference also included a coding workshop and a risk management update.

Query: Working as a Locum Tenens
I am thinking of selling my practice, but want to continue to do some part-time work. If I were able to do some locum tenens work, I'm not sure of the guidelines such as rate of pay, billing, and time being able to be spent at one practice. Would I be able to stay at one office once per week for an indefinite amount of time under a locum tenens arrangement?

Editor's note: PM News does not supply legal advice: Paymemt arrangements are negotiated between the practice and the locum tenens, generally on a per diem basis (§30.2.11.). Here are the Medicare Guidelines for locum tenens: If a physician is absent for a limited period of time for vacation, disability, continuing education, etc, you may bill Medicare for services performed by a locum tenens physician under the regular physician’s NPI as long as the following conditions are met.
• The regular physician must be unavailable.
• The locum must be compensated on a per diem or similar fee for time basis.
• The Medicare beneficiary seeks to receive the services from the regular physician.
• The regular physician cannot bill for the services of a locum tenens physician for a continuous period of longer than 60 calendar days.
If, after returning to work for a brief period of time, the regular physician must be absent again, the same locum tenens physician may be re-hired, and a new 60-day period begins. If a physician is absent longer than 60 days without returning to work, the locum tenens must be credentialed and enrolled as you would do if this were a new physician.

Query: Tendon Repair with Use of GraftJacket
Please advise as to the most appropriate code(s) for repair of a partially torn posterior tibial tendon with use of GraftJacket.

Response: The use of GraftJacket does not influence the tendon repair code you choose (in this case, CPT 28200 - repair, tendon, flexor, foot; primary or secondary, without free graft, each tendon). There is no specific code for the application of GraftJacket in tendon repair. Typically, unless you get authorization for additional allowance pre-op, you might very well find that there is no extra allowance for its use in reinforcing a tendon repair.
Some surgeons have added a "-22" increased procedural services modifier to the repair code. Others have billed the unlisted foot/toe code, CPT 28899. I can't tell you if they have been successful.

I have had to fuse the first ray on several occasions basically from IPJ to TN joint. If you are positive that all conservative efforts have been exhausted, then fuse the first MTPJ. Make sure you use some sort of biomechanically-sound accommodation in the shoe post-op – most commonly a simple rocker bottom or metatarsal bar which can be added to most shoes with ease and is fairly cosmetically acceptable.

I noticed that Dr. Albright feels like an MRI scan for the clinical situation presented is essential. I would respectfully disagree. Given the scenario presented and if the pain was relieved on the short term by intraarticular injection of local anesthetic I would feel very comfortable with a decision of fusion. I am a believer in technology assisting us in our diagnosis and treatment choices but I am a bigger believer in common sense and clinical experience.
I think the “essential” aspect of getting an MRI in this case is essential only for our malpractice carriers. I can see a point in our future that we may be actually penalized by insurance companies or our government for requesting an expensive diagnostic procedure on fairly routine cases.

Dr. Ribotsky makes it sound like every patient who has bilateral plantar fascitis should use two night splints. I have treated thousands of patients with plantar fasciitis, many bilateral, and almost every patient who is able to sleep with a night splint finds it very helpful. I always start with one night splint and when they come in for a follow-up in a few weeks, I offer a second one. I have found that greater than 90% of the patients do NOT want to use a second night splint, even if the first one is helping significantly.
I tell patients with bilateral plantar fasciitis to use the splint on the leg that has the more severe plantar fasciitis and, then when that foot is the same as the other foot, to alternate legs each night. Patients seem to rather have their fasciitis get better a little more slowly than sleep with two night splints.
90% of the patients I treat for plantar fasciitis are NOT seniors. Perhaps Dr. Ribotsky has an older population and they sleep more soundly, or maybe people in Florida just sleep better than New Yorkers, but I always ask knowing what the patient will say and still get a laugh at the look on their faces at the thought of sleeping with two night splints.
