NY Podiatrist Discusses Prevention of Foot Pain

"About 75% of people have foot pain at some time in their life, especially as they age," says Dr. John F. Connors, a board-certified sports podiatrist in New York City. "It can be a big problem that hinders mobility, balance, and quality of life. Women, in particular, have more foot-related pain than men--especially women who wear high heels, narrow-toed shoes, sandals, or ill-fitting footwear."

Losing weight also may help prevent pain down the road, Dr. Connors says. "One pound on your body can amount to as much as five pounds of pressure on your feet. Less body weight means less stress and shock on your feet."

TUSPM Honors PA Podiatrist at Gallery of Success Awards

Leonard Portnoy, DPM has been named as one of the awardees of Temple University Gallery of Success Awards. Each year during homecoming, Temple University selects two alumni from each of the 17 schools and colleges for this distinction. Temple University has over 260,000 living alumni.

OH Podiatrist Organizes Candy Exchange Party

The 12th annual Halloween candy exchange party for children with diabetes was held at the offices of Achilles Foot and Ankle Surgery,  on Sunday, Nov. 1. Unlike other Halloween parties, this event is designed for children who have diabetes and, thus, cannot eat the typical sugared candies that other youngsters enjoy after an evening of trick-or-treating.

Dr. Bruce G. Blank, podiatrist and organizer of the annual party, explained, "Halloween is a fun holiday, but not necessarily fun for children with diabetes. For the most part, they can't eat the treats they get." To give those youngsters an alternative, he said, "We have the children bring the sugared candy that they get on trick-or-treating night to our office. We have a costume party and, at the end of party, they give us the sugared candy and we give them snacks and treats that they can eat, that are more appropriate for their conditions."

In turn, Blank said, "The candy that they give us is then donated to the Soup Kitchen of Greater Wheeling. "It's been a fantastic event," Blank said of the holiday tradition. "Children enjoy it; families enjoy it. We have a lot of fun with it. We look forward to it every year."

QUERIES (CLINICAL)

Query: Post-op Fracture Blister
 
60ish y/o healthy female had surgical reconstruction with arthrodesis of 1st MTPJ and 1st met/cuni, right foot. She developed a large fracture blister post-op which resulted in a small proximal dehiscence, which has never been clinically infected. She appears to be healing steadily without any more problems. There is still a 2.5cm X 1cm superficial wound which is meticulously cared for, and is steadily improving without antibiotics. There is no exposed hardware or appearance of infection.

The patient has very little pain. A "C" arm shows very stable fusion sites. The patient understands the complication and is generally happy with her results. My question is: As long as the right foot with a clean open wound is isolated from the left in the OR, does anyone see a problem with repairing her left foot before her right foot is 100% epithelialized?

 

Fracture Blister, Current

Generally, I do not repair the contralateral foot until the person is 100%, but in this particular case, given that the patient will be independently ambulatory, I believe that there could be some consideration for the patient's request. Besides using as atraumatic technique as possible, does anyone have any pearls on avoiding fracture blisters in patients who have a history of developing them?

Query: Concierge Services

Does anyone provide concierge services for their staff members? A concierge service provides a wide variety of tasks and errands for employees, including such convenience services as picking up dry cleaning, or waiting at a person’s home for a  repairman or a delivery. Concierge services can be provided by someone hired for that purpose (one practitioner hired his retired father-in-law) or outsourced to an independent contractor. It's a terrific perk for employees.

RE: 1st MPJ Closed Reduction in 13 Year Old Boy (Michael Tritto, DPM)

Since there appears to be good alignment of the hallux proximal phalanx' growth plate, but with compression of the base upon the 1st metatarsal head due to the injury, I would apply a monolateral external fixator to re-align the anatomical structures with slight distraction at the MPJ, allowing for repair of the intersesamoidal ligament. Maintain alignment with the fixator for at least 6 weeks, remove it, then start ROM exercises as tolerated. Unfortunately, this type of injury would require him to avoid playing football for the season.

The joint is not properly reduced. I would like to see a "non-splinted" lateral view. It looks like the hallux is still not relocated properly in the sagittal plane at the 1st MPJ. This may require an O.R.I.F. with a 0.62 K-wire for 6 weeks, the same period to attempt to allow the tibial sesamoid to heal.
