VA Podiatrist Interviews Biomechanist About Foot Orthosis Therapy in Cleats
Recently, during a segment on his podcast Fit Foot U, Ben Pearl, DPM interviewed Matt Gooch, MS. CPed, Vice President of Product and Innovation at Superfeet, about the challenges of providing foot orthosis therapy in cleated footwear. The conversation covered a variety of topics detailing what makes cleated footwear such a difficult shoe environment for athletes and clinicians to optimize. Beyond traditional fit considerations, the roles that kinesthetic awareness, plantar sensitivity, and supination resistance play in the way that different levels of athletes experience custom foot orthoses and OTC insoles were discussed. Here is a clip from the podcast.
|
|
Matt Gooch |
Pearl said that podiatrists are better prepared to help patients if they understand what is available as footwear and OTC footbed solutions in the retail space. Gooch stated that at no time have there been as many high performing running shoes across all the brands. He also noted that the trends for lighter foams with higher stack heights and lower heel to forefoot ratios have left a gap in older patients with balance issues who are using them for walking and running.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
3 Lessons from the Trenches
Lesson #3: Answer all questions
Another good example is calling special patients at home to check on their condition or answer any questions they may have. Examples of these special patients include those who have been recently discharged from the hospital, patients undergoing one-day procedures under anesthesia or sedation, patients with complications or side-effects, and patients with lingering questions about their treatment or condition.
While the busy doctor's schedule may not allow such calls to all special patients on any given day, someone in the office, such as a medical assistant, can call patients late in the day to make sure they are clear about how to take their medications, how to manage their wounds and dressings, and when to make the next appointment. This covers most questions patients will have, and a call from the office is very reassuring.

| CODING & BILLING Q&As FROM CODINGHELPLINE.COM | |
Query: Fracture Care
I have an elderly patient who fell and broke her ankle and foot. She suffered a trimalleolar fracture with a fracture of the cuboid, 1st, 2nd, 3rd, and 4th metatarsals and a Lisfranc's fracture dislocation. She has many medical problems including going through chemotherapy radiation therapy and immunotherapy. She is going to be treated conservatively with immobilization. How would I bill the fracture care?
Jeffrey Klein, DPM
Response from Codinghelpline.com: A question one must ask to answer what you described is do you feel that you stabilized the trimalleolar fracture and the Lisfranc's fracture? If so, then you bill for fracture care. If you had to do closed reductions with or without manipulation, then select those 90-day global procedure codes. If you feel the fractures are not yet stabilized, then bill the appropriate E/M, x-rays if you performed them, and immobilization DME coding if you dispensed the device.

RE: Medicare Part D
From: Lloyd Smith, DPM
The changes to Medicare and the implementation of Part D were promulgated under the George Bush administration. Part D, in particular, benefits insurers and pharma. The limits on Part B are shared by all providers and were designed to prevent a Medicare bankruptcy.
The $2,000 cap for Part D is a step in the right direction. Allowing Medicare to negotiate drug prices is long overdue. Only Congress can alter the Part B formula.

From: Ivar E. Roth DPM, MPH
First, Dr. Rappaport is correct. ACFAS should either prohibit ads for MD foot and ankle Orthos, or ACFAS should contact the equivalent of ACFAS in the MD world and tell them either establish a quid pro quo or no more ads will be allowed.
The solution for your unequal pay is simple. Start your own practice and make as much as you want from your own efforts. Depending on others never really works out anyway.
