VA Podiatrist Interviews Physician Electromagnetic Therapy Expert
Ben Pearl, DPM interviewed William Pawluk, MD, MSc, an expert and author on electromagnetic therapy. Podiatry has been using electromagnetic therapy in the form of bone stimulators for several decades. The body has neurons and other tissues like bone that respond to electromagnetic charges like a battery. Dr. Pawluk stated that electromagnetic energy effects are governed by the laws of physics. Applying Faraday's law, we know that the intensity of the magnetic field pulse and its rise time to reach its peak determines the amount of energy the body produces. The higher the gauss intensity and the faster the pulse reaches its peak, the more energy is produced by the body to stimulate healing.

Another law of physics that applies to Pulsed Electro-Magnetic Fields is Newton’s inverse square law that governs the natural, rapid drop off in magnetic field intensity with distance from the source of the magnetic field. By using the inverse square law, you can calculate the power level needed to have enough depth of penetration to get the desired effects at the target tissue.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
Six Steps to Ensure Your Future Practice Success
6. Share the importance of continuity of care. There’s a strong argument to be made that impersonal, episodic care costs more in the long run. Make sure your patients know that consistent care is cost-effective care.

Query: Tungiasis (Sand Flea Disease)?
I heard from a young man this afternoon, quite terrified, who has been on the beaches of Southern Mexico over the last week or so. Starting last Friday, he started feeling unwell with fevers and generalized discomfort in addition to a sore throat. He has been ill since then, routinely taking medications for fevers and discomfort. He noticed sores on his feet a few days ago, which seem to have erupted and are getting worse. He just went to an Urgent Care in Sacramento on his way back to Colorado. He was given a diagnosis of tungiasis (sand flea disease) as he was in the sand in Southern Mexico when this eruption occurred.
|
|
Tungiasis (Sand Flea Disease)? |
I suspect this may be a secondary reaction to infection/sickness/enterovirus which caused the skin eruption; however, I don't have much experience with either one of these conditions and have not seen this type of presentation lately. I suspect hand, foot, and mouth disease and believe observation is the best course of treatment currently as he was placed on antibiotics and seems to be improving clinically over the last day or so. Can anyone give suggestions or recommendations on what this likely is and the best course of treatment?

Query: Retirement and Purchasing a Malpractice “Tail”
I have been semi-retired since 2016 and plan to fully retire at the end of this year when I close a satellite office I work at one day a week. My practice is general podiatry and routine foot care on diabetic patients and patients on qualifying anticoagulants. I do not do any wound care or surgery with the exception of simple ingrown toenails, biopsies, etc. I would like some advice on if it is necessary to purchase a tail on my malpractice policy. I currently have a minor, part-time non-surgical policy. Any thoughts on this subject would be appreciated.

From: Ivar Roth, DPM, MPH, Mark Weaver, DPM
Congratulations to Dr. Wenstrup. He is a shining example of the caliber of our profession and a fantastic representative. I sincerely hope he can continue to shine the light on our profession and help us achieve our parity goals. I hope someone from APMA governance will read this and contact him to strategize on our behalf. We need all the help we can get to highlight this small but very needed profession.
Ivar Roth, DPM, MPH, Newport Beach, CA
I am proud to be one of the first to congratulate Dr. Brad Wenstrup on his Congressional victory. Brad you are a good man. What great exposure you have given to our podiatry community! Dr. Wenstrup, you have never let our profession down. From your military service to the Republican baseball practice massacre (where you were a medical and podiatric hero bravely saving lives under fire!) to the multiple positive medical bills you have supported in Congress.
Mark Weaver, DPM, Ft. Myers, FL

| RESPONSES/COMMENTS (PM NEWS JURY VERDICT REPORTER) | |
From: Chris Robertozzi, DPM
Dr. Grife makes some valid points about how attorneys for the plaintiff and defense get paid. That can in itself be a lengthy discussion. I am a little confused, however, about other statements that he made, all of which revolve around the word "lie". Webster defines a lie as an “intentionally false statement or impression”. That is completely different from the fact that experts are retained to provide their opinion based upon the facts, in Dr. Grife’s own words. He asks, is the expert lying who under oath in court testified that the only way a patient could have tarsal tunnel is if the patient was confined to a wheelchair? The answer is yes.
You can go to any podiatric or orthopedic book on tarsal tunnel and it will list other reasons such as...
Editor's note: Dr. Robertozzi's extended-length letter can be read here.

Nurse Accused Of Amputating Man's Foot Without Permission
A Wisconsin nurse was charged with elder abuse after police said she amputated the frostbitten right foot of a dying nursing home patient without his permission. Even more shocking is her alleged motive for surgery on the 62-year-old man.

Not the way to make a bronze foot! |
Mary K. Brown, 38, told co-workers she planned to bronze the man’s right foot and display it in her family’s taxidermy shop with a sign saying, “wear your boots kids,” according to a police affidavit cited by The Washington Post.
