Peeling Feet Most Commonly Caused Fungal Infections: NY Podiatrist
According to Rebecca Pruthi, DPM, the most common reason patients have peeling feet is a fungal infection (like athlete's foot)—although they might not always realize it. "A lot of times it just presents itself as peeling skin and patients don't have the itchiness, so they don't know it's a fungal infection," Dr. Pruthi says. So if your skin starts mysteriously peeling all of a sudden, get it checked out sooner rather than later. Once you have a fungal infection on your skin, it can easily infect your toenail. Pruthi warns can be very difficult to get rid of.

"What I find that happens is that the skin infects the nails, the nails re-infect the skin, back and forth, back and forth. So whatever presents itself first, you want to knock it away," says Dr. Pruthi.

Judge Denies FL Podiatrist New Trial in Video Voyeurism Case
Circuit Judge Daliah Weiss denied Scott Strolla’s motion for a new trial after a jury found him guilty of video voyeurism, though one juror admitted to doing outside research during deliberations. “Though there was juror misconduct, it was not prejudicial to (Strolla),” Weiss wrote in the Oct. 29 denial. The 53-year-old foot doctor will not get a new trial for the video voyeurism charge but is scheduled to begin his retrial Jan. 13 for two counts of sexual battery. Weiss declared a mistrial when jurors could not reach a verdict in those two charges during the September trial.

Query: Fiberglass Coding
Is Q4038 the correct code to bill for the fiberglass casting supply? How do I bill for fiberglass under DMERC?

Response: HCPCS Level II code Q4038 is defined as: Cast supplies, short leg cast, adult (11 years +), fiberglass.
The HCPCS Level II “Q” codes which define casting materials are not billed to the Durable Medical Equipment Regional Carrier (i.e. DMERC). They are billed to the Medicare Administrative Carrier, the standard Part B Medicare Carrier. Even if the application of the cast (i.e. cast application CPT code) is included in the reimbursement for the CPT code/procedure that necessitated the application of the cast, more often than not, the cast supplies are separately reimbursable.

RE: The Passing of Richard G. Macdonald, DPM
Dr. Richard G. Macdonald, 82, a long-time central Illinois resident, passed away on October 31st. He graduated from the Illinois College of Podiatric Medicine in 1960. Entering the Army, he became the eighteenth podiatrist commissioned in its history. During his career, he was clinical instructor in the Department of Family and Community Medicine, University of Illinois College of Medicine Peoria, and published numerous articles in the field.

He was affiliated with Peoria hospitals, OSF Saint Francis, Methodist and Proctor, and was a consultant for the VA Out-Patient Clinic. Concerned about the education of podiatrists, he served on the Alumni Board of Directors at the Scholl College of Podiatric Medicine at Rosalind Franklin University.

I recently purchased Quantum Rub at a seminar. It contains polarized CBD, menthol, and various herbs and oils including vitamin E. My wife has used it after our trips to the gym. She said that it helps with generalized soreness, not strong pain. Naturally, this is anecdotal evidence. It seems all rubs contain menthol for their cooling effect. Many can contain ingredients that produce warmth and new evidence shows it may speed healing. As to the claim of polarization, it is unproven. Ingested CBDs in the form of pills and gummies may be taken off the market due to lack of standards. I personally prefer Voltaren gel on a limited basis, as there is some systemic absorption.

CBD topical preparations appear to be very helpful for symptomatic treatment of musculoskeletal disorders (e.g. arthritis, tendinitis ). I offer this has an alternative to those patients in whom oral NSAID therapy is not appropriate or concerning, e.g.: renal concerns, cardiac concerns, anticoagulation therapy, history of GI pathology). Typically, high NSAID risk patients are given the choice of topical NSAIDs, CBD topical, or no treatment. I personally use the CBD products from EBM pharmacy. The results have been excellent. I prescribe CBD topical daily.

Thank you Drs. Frykberg, et al. for completing the study on topical oxygen therapy for the treatment of diabetic foot ulcers. The study showed that that those who used the topical oxygen therapy device had 41.7% of their ulcers heal as compared to 13.5% who used a sham device. This is significant.
In our practice, we have used topical oxygen therapy to treat diabetic ulcers for many years and have had great success. In two cases where there was no ability to surgically revascularize the lower extremities and the surgeons expected the patients to go on to BK amputations, we were able to save those legs using topical oxygen therapy. The only problem with it is that the only insurance company (in NY) that pays for the device is Medicaid. Patients are not given the opportunity to pay out-of-pocket, although one supplier has been willing to supply some of my patients with the modalities for free.
I hope that Dr. Frykberg and his team of researchers will be able to convince Medicare and other carriers to cover this limb-saving modality.

I was very saddened to learn of Allen's passing. It's a terrible shock to read a classmate's obituary. More years have passed than I'm sure our class cares to admit, but I clearly remember Allen as a hard-working, intelligent student, who always managed to smile and laugh and lend a helping hand. May his memory be a source of blessing and comfort to his family. Rest in peace.
